[Cerebral intraventricular and/or intraparenchymal hemorrhages in full-term newborn infants. Long-term outcome].

BACKGROUND: Intraventricular and parenchymal hemorrhages are rare in full-term neonates. Their prognosis is still under discussion. POPULATION AND METHODS: 6 full-term newborns admitted between January 1985 and December 1988 developed intracranial hemorrhage: 3 had intraventricular hemorrhage and 3 parenchymal without intraventricular hemorrhage. The study includes the results of a questionnaire on the pregnancy and delivery, clinical, ultrasonographic and CT scan data. The long-term neuro-developmental outcome was assessed when the children were 4 to 7 year-old. RESULTS: All the patients were given phenobarbitone for 1 month to 1 year (one patient was given phenobarbitone plus phenytoin). One patient, aged 1 month, required insertion of a valve, because hydrocephalus developed. One patient had difficulties at school with developmental retardation; another patient developed minor difficulties. The 4 other patients had normal motor and mental development with good scholastic performances. CONCLUSIONS: The prognosis for intraventricular and parenchymal hemorrhage seems better in full-term than in pre-term infants.

[Fulminant pneumococcal septicemia in a splenectomized child despite vaccination and chemoprophylaxis: necessity for education of the entourage].

A splenectomized three-year-old developed fulminant pneumococcal septicemia despite immunization and chemoprophylaxis. The course was rapidly fatal. Fulminant pneumococcal septicemia mainly affects splenectomized individuals and is associated with very substantial mortality. Immunization prior to splenectomy and daily prophylactic oral penicillin have partial preventive efficacy. Other useful measures include carrying a health status card and inpatient antimicrobial therapy in the event of fever. The need for these precautions should be repeatedly discussed with the child's parents and physician.

[Cutaneous larva migrans in travelers].

Fifteen cases of cutaneous larva migrans (CLM) diagnosed in transcontinental travellers over a period of 4 years in the Department of Tropical Medicine in Hospital Clinic i Provincial of Barcelona were reviewed. The frequency of this disease as an imported pathology is on the increase and is related to the greater mobility of travellers today. Tourism is the main motive of the traveller and the geographic origin of the cases is cosmopolitan. The use of open footwear in 14 of the 15 patients is pointed out. The single lesion on the foot was the most frequent site. Thiabendazole was effective in 14 patients as was albendazole used in one patient. The secondary effects of thiabendazole, principally gastrointestinal disorders, headache, and dizziness are underlined.

[Intratubular neoplasm adjacent to germ cell tumor of the testis].

Because intratubular germ cell neoplasia (IGCN) is considered to be a precursor of testicular cancer, precise identification of this tumor type is important. The present study was undertaken to determine the cytological features of IGCN, its histological classification, and its incidence relative to other invasive germ cell tumors. We studied the biopsy specimens from a similar number of patients. The biopsy material was fixed in formol and embedded in paraffin. Sections were processed and analyzed with the hematoxylin-eosin and PAS stainings methods with and without diastase. IGCN classification was performed following the criteria recommended at the International Symposium on Testicular Cancer (Minneapolis, 1980). IGCN was recognized in 22 (26%) of the 85 biopsies and presented the following characteristic features; large cells with hyperchromatic nuclei, one or more nucleoli and abundant cytoplasm. Glycogen was present in most of the lesions but not in controls. In 12 patients (54.5%), the cytological anomaly was observed in the periphery of the tubules without the characteristic features of germ cell tumors. This lesion type was termed "unclassified". In 5 cases (22.7%), the tubule lumen was filled with atypical cells with round nucleus, scant chromatin, prominent nucleolus and scant cytoplasm. This variety was termed "intratubular seminoma". In 2 cases (9%) the intratubular lesion revealed large cells with pleomorphic nucleus and central necrosis. Direct quantification of coproporphyrins and uroporphyrins in urine by derivative synchronous fluorescence spectroscopy.

The second-derivative synchronous fluorescence spectroscopic technique is applied to the simultaneous and direct assay of coproporphyrins and uroporphyrins in human urine. This technique resolves the overlapping conventional spectra, obviating the need for pre-analysis sample separation techniques, and measurements can be made in a single scan. The amplitudes of the derivative peaks are linearly related to uroporphyrin (0.4 to 300 micrograms/L) and coproporphyrin (0.4 to 250 micrograms/L) concentrations. The detection limit for both porphyrins is 0.1 microgram/L. Analytical recoveries range between 98 and 101%. Within- and between-assay CVs are reported. Results for 24-h urine specimens correlated well with those obtained by an extraction-ion-exchange chromatography combined method. The proposed method is inexpensive and requires no sophisticated detection equipment.

Simultaneous and direct determination of pyridoxal, pyridoxal-5'-phosphate, and pyridoxic acid in serum by derivative synchronous fluorescence spectroscopy.

Simultaneous and direct assays of pyridoxal, pyridoxal-5'-phosphate, and pyridoxic acid in human serum are described. The method applied is based on the reaction of these compounds with beryllium in an ammoniacal medium to yield highly fluorescent derivatives. Overlapping of conventional fluorescence spectra is resolved by using second-derivative fluorescence spectroscopy, thus making the use of separation techniques unnecessary. The proposed method is simple (only beryllium and an ammoniacal buffer are needed to develop fluorescence), rapid (the derivative formation is instantaneous and serum treatment only requires deproteinization), and inexpensive (no sophisticated detection equipment is necessary, any conventional modern spectrofluorimeter being adequate for use). The analytical recovery achieved was of about 96% for pyridoxal, 97% for pyridoxal-5'-phosphate, and 100% for pyridoxic acid. Measurements were carried out in a single scan.

Simultaneous determination of epinephrine and norepinephrine in urine by derivative synchronous fluorescence spectroscopy.

The combination of synchronous and derivative fluorimetry, potentially useful in resolving mixtures that present strongly overlapping conventional fluorescence spectra, appears suitable for determining epinephrine and norepinephrine in urine. We describe a simple, rapid, and inexpensive method for simultaneous assay of these compounds in a single scan of urine specimens from normal adults. The catecholamines are extracted from the sample on cation-exchange columns and subsequently determined by formation of trihydroxyindoles. Analytical recovery is about 94% for epinephrine, 91% for norepinephrine, with a CV of approximately 5%. No sophisticated detection equipment is necessary, a conventional modern fluorimeter being adequate.

Dimethyl sulfoxide therapy in bronchiolitis.

The action of a medicinal spray that contains anti-inflammatory and bacterio-static drugs integrated with the penetrating and potentiating agent DMSO has been evaluated clinically in 60 infants with acute respiratory obstruction. A dose of 2 ml of this spray is applied in the posterior pharynx and the tonsil region; 1 to 4 applications are made, according to the clinical evolution of the case. Two groups were chosen at random; 60 patients were paired off, and one of each pair received the treatment with DMSO spray. All the patients received ampicillin (50-100 mg/kg body weight). The following clinical results were observed. First there was an immediate recovery, after an average lapse of 30 min; improvement of sensorial involvement was observed in 80% of the cases, reduction of the intercostal retraction in 75%, reduction of polypnea in 76%, and transformation of the obstructive syndrome into a catarrhal syndrome in 80% (with fluidification of the secretions, which were expelled more easily by the upper air tracts). There was also a deferred or maintenance effect, which evaluated in comparsion with the control group that received similar treatment except for the nebulizations with DMSO spray. A sequential design, based on whether or not it was necessary to use the croupette, demonstrated the superiority of the DMSO spray over the control treatment in the bronchiolar process; in the group treated with DMSO spray it was not necessary to use the croupette. The sequential design based on the effect of this therapy on the general condition and the tabulated clinical factors permitted the inference that the therapeutic responses are more favorable in the group treated with DMSO spray than in the control group. In the sequential design, the line of significance is cut at the fifteenth pair (an error of 0.05 and P equals 0.95%). Since the application is easy, there are no toxic side effects and in view of the favorable results in the clinical evolution of the acute respiratory obstructive processes, we consider the use of this therapeutic spray very useful and beneficial in bronchiolitis.
