Prevalence of ischaemic heart disease risk factors in male populations aged 45-54 years in Warsaw and Bratislava. Part IV: Relationship between dietary habits and IHD risk factors.

The aim of the study was to investigate the relationship between IHD and nutrition. 404 occupationally active men in Warsaw aged 45-54 years, and 634 men in Bratislava were screened. The dietary habits of both populations were evaluated on the basis of their dietary history. Although no significant correlation between the daily frequency of basic meals consumption and the prevalence of hypercholesterolaemia, overweight and hypertension was found, a tendency to higher prevalence of these risk factors among people consuming only 2 meals daily was observed in both populations. In the Bratislava population, which was characterized by a lower frequency of basic meals consumption, higher consumption of meat, meat products and fat, a higher prevalence of overweight, hypercholesterolaemia and hypertension was found. The authors conclude that the influence of unproper dietary habits characteristic of industrialized countries, on the health of their populations cannot be completely neglected.

Changes in the dietary habits of male workers in Warsaw plants over a period of four years.

At the Polish Trial on Ischaemic Heart Disease Prevention begun in 1976-77 the dietary habits of 5,484 male workers aged 40-59 years working in 14 Warsaw plants were assessed by 24-hour recall. After two and four years of follow-up in 1978-79 and 1980-81 this study was repeated in random samples of the previously screened population still in employment in these plants i.e., 652 men (response rate after two years=70.4%) and 574 men (response rate after four years=65.0%). In the initial screening the mean energy value of the reported daily food ration was 2,700 kcal/11,296 kJ. This energy derived 13.6% from protein, 37.1% from fat, and 49.3% from carbohydrate. After two years the mean energy value of the daily food ration as well as the amounts of the daily consumed fat and carbohydrate and vitamin B1 had decreased significantly. The amounts of the remaining nutrients were not significantly changed. After four years only the mean daily consumption of vitamin C was decreased significantly. The mean values of the total energy value of the food ration as well as of the amounts of nutrients were not significantly different. On the other hand, differences were found in the amount of food products--the reported consumption of cereals and bread as well as animal fat increased significantly, while the consumption of meat and meat products, vegetable fats, fruits and sugar decreased.

Prevalence of ischaemic heart disease risk factors in male population aged 45-54 years in Warsaw and Bratislava. Part III: Smoking and overweight.

The purpose of the study was to determine the prevalence of such ischaemic heart disease (IHD) risk factors as cigarette smoking and overweight, and the degree of IHD risk in the Polish and Slovak populations. The investigated population consisted of men aged 45-54 years; 2944 of them were from industrial plants in Warsaw, with response rate of 88.4%, and 698 from plants in Bratislava (response rate: 64.5%). 60.4% of the Warsaw group were cigarette smokers, with average daily consumption of 12.7 cigarettes; the respective values in the Bratislava group were lower (46.4%; 9.2 cigarettes). The mean body weight and the prevalence of overweight were higher in Bratislava (79.6 kg; 25.6%) than in Warsaw (77.9 kg; 20.8%). The IHD risk evaluated according to the multilogistic function was higher in Bratislava. It is concluded that the difference in the IHD death rate found in these cities could to a high extent be explained by the difference in the prevalence of risk factors.

Prevalence of diabetes mellitus, coronary heart disease and hypertension in the families of insulin dependent and insulin independent diabetics.

During an epidemiological study concerning the fate of diabetics in Warsaw, 2,356 subjects (aged 35-68 years with duration of diabetes mellitus of 3-11 years) were investigated with particular relevance to the presence of diabetes mellitus, coronary heart disease, and hypertension in their parents and siblings. Diabetics were classified into the following groups: insulin dependent, insulin independent nonobese, insulin independent obese, and a group in whom the distinction between insulin dependence and insulin independence was unclear. The findings in these groups were compared with the frequencies of these diseases in a random sample of general population. There was an excess of diabetes in close relatives of all the diabetic groups. This was highest for insulin independent non-obese diabetics. There was no difference in the prevalence of coronary heart disease and hypertension in close relatives of insulin dependent diabetics when compared with the general population, but these were twice as prevalent in close relatives of the insulin independent non-obese group. Obese insulin independent diabetics reported a similar excess of coronary heart disease and hypertension in siblings, but the excess was less marked in parents. The prevalence of these diseases in families of probands with unclassified diabetes was intermediate between the other two groups. The results demonstrate an aggregation of diabetes mellitus with coronary heart disease and hypertension in families of insulin independent non-obese diabetics. This provides further evidence for heterogeneity in diabetes mellitus.
