[Molecular biology investigation of respiratory viruses as a factor of infectious complications in hemoblastosis and myelodepression].

Communicable respiratory viruses as a causative factor of infectious complication in hemoblastosis and myelodepression were investigated in 51 patients (aplastic anemia--3, multiple myeloma--10, different patterns of acute leukemia--16, chronic leukemia--8 and non-Hodgkin's lymphoma--14). Our clinical evidence obtained with the aid of polymerase chain reaction featured genomes of adenoviruses, influenza A and B viruses, respiratory-scintillating virus and coronaviruses. On the whole, respiratory viral infections were detected in 27 (52.9%) patients: adenoviruses--23.5%, coronaviruses--13.7%, influenza A and B--5.9% and respiratory-scintillating virus--3.9%. In many cases, herpes was associated with viral respiratory infection. That pathology was most often triggered by severe neutropenia induced by chemotherapy.

[Clinical biological features of mixed myeloid diseases].

AIM: To characterize patients with mixed myeloid neoplasias with proliferation of neutrophils, platelets and eosinophils. MATERIAL AND METHODS: Examination and treatment results were analysed for patients with atypical myeloid leukemia (n = 4), myelodysplastic syndrome (MDS, n = 1) and thrombocytosis, MDS and eosinophilia (n = 1). The examination included morphological, histological, cytogenetic and molecular tests. RESULTS: One patient with atypical chronic myeloid leukemia was prior diagnosed to have primarily MDS with a typical aberration of chromosome 5. Two other patients had an initial morphological picture of resistant anemia with blast excess, signs of myeloproliferation and extramedullary hemopoiesis. One and two months after the first examination they received transfusions of erythrocytic mass. Just then they were found to have splenomegaly and leukocytosis due to proliferating and maturating forms of neutrophils. The course of the disease in patients with MDS, thrombocytosis and normal karyotype and in patients with MDS, eosinophilia and combined chromosomal breaks including translocation (3;12)(q21;p13) was characterized by resistance to standard programs of polychemotherapy and transformation into acute myeloblastic leukemia. CONCLUSION: In some cases atypical CML is a stage of a natural course of MDS. Some MDS variants with eosinophilia and thrombocytosis should be referred to the group of mixed myeloid neoplasias.

[Anticytosine therapy for primary myelodysplastic syndrome].

A pilot study of anticytocine therapy (PCD) was carried out in 9 patients with primary myelodysplastic syndrome (MDS). After 12 weeks, 2 patients (22%) showed hematological response cutting down the need for washed-out erythrocyte transfusions by half or more. MDS progression at different stages was reported in 4; transformation to acute myeloleukemia--2. PCD treatment is indicated in MDS patients, aged over 65, with a less than 10% blast level of the bone marrow, and without any risks of tumor progression.

[Neuroimmunopathological mechanisms of preclinical epileptogenesis and their correction by cycloferon].

The peculiarities of immunologic processes and immunocorrecting potential of Cyclopheron were studied in 62 patients with preclinical epileptogenesis. Using clinical, EEG and biochemical examination, expression of epileptogenesis was estimated as regressed, compensated, increasing, decompensated. Preclinical stage of epilepsy was accompanied by disturbance of immunological brain reactivity the expression of which was correlated with a level of compensative epileptogenesis manifestation. The decrease of the latter is followed by intensive disintegration of neuroimmune interactions characterized by immunodeficiency of mixed type with significant presentation of neuroimmune disturbances. Therefore, a use of Cyclopheron, as a medication with multilevel immuno-correcting action, is expedient at initial stage of epilepsy.

[The in-vitro effect of constant and pulsating magnetic field on immunocompetent blood cells of hematologic patients].

Immunologic characteristics were studied in 103 patients with multiple myeloma, acute leukemia, chronic lymphocytic leukemia and non-Hodgkin's disease following in vitro exposure of blood to a low-intensity static field (SF) and alternating field (AF) or pulsating magnetic field (PF). In a SF-AF study of multiple myeloma, a 30 min exposure had a positive effect on expression of tumor cells and T-cell markers and stimulated the regulatory function of T-lymphocytes. With SF-AF and PF application alternating, the expression of both +CD3 and +CD4 and the +CD3/+CD4 ratio increased suggesting the lowering of immunological deficiency. In acute leukemia, a combined application of the magnetic fields had an effect on the helper activity of the T-lymphocyte subpopulation. The phagocytic activity of leukocytes increased significantly while their digestive ability rose to a moderate degree.

[The action in vitro of a permanent and an alternating magnetic field and of cytosar on the colony- and cluster-forming properties of bone marrow cells from hematologic patients].

The bone marrow of hematological patients was studied as were effects of cytozar at different dilutions on the bone marrow cells. The analysis of the secured results showed the magnetic field to be a powerful biologically active factor capable of changing clonogenic properties, proliferative and differentiative potential of hemopoietic cells precursors. A long exposure brings about a decline in the colony-forming activity of bone-marrow cells, that is to say, with prolongation of time of the exposition the magnetic field becomes an unfavourable factor for the leukotic cell culture development. Similar results were obtained with cytozar in the culture while studying the colony-forming capability of bone-marrow cells. Hence it appears that action of magnetic field under long exposition to it, combined exposure to magnetic fields, are, in some cases, comparable to effects of cytozar. It is possible that concurrent prescription of cytozar together with magnetization of blood in patients with hemoblastoses should permit enhancing the efficacy of treatment. Employment of weak magnetic fields is a satisfactory and worth-while therapeutic attempt to be made in a multimodality treatment of patients with hemoblastoses.

[Low doses of alpha-interferon preparations in the treatment of patients with chronic myeloleukemia].

AIM: To compare the effects of low-dose alpha-interferons with those of cytostatics (hydroxyurea or myelosan) on survival of patients and duration of chronic phase of chronic myeloid leukemia (CML). MATERIAL AND METHODS: 107 CML patients were divided into two groups. 28 patients (15 males and 13 females) aged 17-59 entered group treated with alpha-interferon drugs. 79 control patients (35 males and 44 females) aged 15-79 received standard chemotherapy (hydroxyurea or myelosan). RESULTS: 3-year survival in the study group and controls was 94 and 67.5%, respectively. 5-year survival--70.8 and 28.9%, respectively. The survival medians for the groups were 66 and 48 months, respectively. 36 months after CML diagnosis, chronic phase of the disease still continued in 90.2% of the study group patients and in 53.4% of patients on chemotherapy. 54 months after the diagnosis the chronic phase was registered in 56.5 and 24% of patients, respectively. The medians made up 54 and 39 months, respectively. CONCLUSION: Treatment of CML with low-dose alpha-interferons increases duration of the CML chronic phase and survival of CML patients.

[Diagnostic and prognostic significance of some laboratory indices of cerebrospinal fluid in multiple myeloma].

beta 2-microglobulin (beta 2-m) and certain other characteristics of liquor have been followed in the course of multiple myeloma (MM) versus severity of the disease and its response to chemotherapy, to establish the diagnostic and prognostic value of these indices. Lymphocytes and plasma cells were found in most of 17 patients under study, who were resistant to chemotherapy and relapsed; they revealed regular rise in liquor pressure and cytosis. Similarly, raised concentrations of beta 2-m were registered. However, there was no correlation between blood serum-beta 2-m levels and that in cerebrospinal fluid (CSF) nor between CSF-beta 2-m concentration, on the one hand, and such parameters as CSF-total protein, blood serum-beta 2-m and pathologic cytosis, on the other. In advanced MM, CSF-beta 2-m levels were, as a rule, abnormally higher. Our data suggest that, in addition to assaying CSF-total protein, severity of cytosis and liquor pressure as well as morphological identification of detected cells, beta 2-m levels be followed in the course of the disease in MM patients, to make therapeutic correction possible. Moreover, CSF-beta 2-m level dynamics can be used as another factor of prognosis.

[Alpha-interferons in the treatment of patients with chronic myeloid leukemia].

Chronic disease duration and survival have been investigated in three groups of patients suffering chronic myeloid leukemia (CML). The first group included 13 patients on alpha-interferons 6-9 mln MU/24 h (mean dose--48 mln MU/week). 31 patients received 2 mln MU/m2/24 h; mean weekly dose--15 x 10(6) MU. Standard chemotherapy was given to another 79 patients (group III). Actual survival and chronic disease duration were computed after Kaplan-Meyer: 4-year survival in group I--88%; group II--85.6% and group III--54%. Five-year survival in patients who had received standard or lower doses of alpha-interferon was 78.7%; chemotherapy alone--28.9%. Median survival in alpha-interferon-treated patients was 66 months; chemotherapy--48 months. After standard alpha-interferon, chronic disease three years after CML diagnosis was in 87% of those treated with standard alpha-interferon, 89% of those receiving lower doses of the drug and 53.4% of chemotherapy-treated patients. After 4 years, chronic disease was registered in 75.5% (alpha-interferon)--74.8% in group I and 72.9% in group II, and in 34.4% of patients treated with myelosan or hydroxyurea. Median chronic stage duration after interferon was 51 months and 39 months in group III, hence, both standard and lower doses of alpha-interferon prolong chronic disease and improve survival in CML patients.

[Alpha-2a-interferon (Reaferon) in the treatment of patients with multiple myeloma].

Sixty-six patients with multiple myeloma were divided into four groups: treatment with alpha-2a-interferon (reaferon) 3-5 mln MU/m2 alone (group); alpha-interferon + pulsed therapy with dexamethasone (group II); reaferon at high or low dose + different regemens of polychemotherapy (group III), and polychemotherapy followed by therapeutic support with melfalan and prednisone (MP) (control). Patients in group I untreated earlier with MP showed positive response in 80% while only 20% were in partial remission. Chemoresistance was broken in 86% (complete clinico-hematological remission--18%) following administration of high doses of alpha-interferon in conjunction with cytostatics. Reaferon + dexamethasone modality proved less effective. After chemoresistance was broken in 83%, tumor process reached a plateau. Low-dose reaferon was less effective, too, with complete remission never being reported. When administered in high doses as support therapy, it proved fairly effective, with median response being double that registered at low dosage or chemotherapy + MP. Whenever high-dose alpha-interferon tolerability is poor, dosage can be decreased; the patients may continue on low dosage or switch to MP support.

[Diagnostic and prognostic significance of blood-serum ceruloplasmin, acetylcholinesterase and total proteolytic activity in patients with multiple myeloma].

The study included 82 patients with multiple myeloma (MM) to evaluate the diagnostic and prognostic significance of correlation between of levels of ceruloplasmin (CP), acetylcholinesterase (ACE) and total proteolytic activity (TPA) in blood serum and immunochemical pattern, tumor mass and response to chemotherapy. It was shown that CP, ACE and TPA determination may be used as additional markers to confirm therapeutic benefits, to timely detect relapse and to verify resistance to chemotherapy. It was also demonstrated that resultant decrease in CP and TPA and increase in ACE levels are reliable indicators of changes developing in MM course and remission fulfillment. High values for CP and low ones for ACE point to pathological activity. Both diagnostic and prognostic significance of the indices has been shown. The highest levels of CP and TPA in blood serum were identified in cases of A-myeloma and myeloma of Bence Jones while ACE concentrations in those patients were lower than in G-myeloma which correlated with median survival. Application of said assays might improve diagnosis, management and prognosis of MM.

[The rheological properties of the peripheral blood in acute diffuse suppurative peritonitis and their changes during infusion therapy using Modegel].

The rheological properties of blood and deformability of erythrocytes were investigated in 85 patients with acute diffuse peritonitis. It was found that higher viscosity of blood mainly with slow shift rates were observed in the reactive and toxic phases of the disease. In the reactive phase these changes can be considered as adaptational, in the toxic phase--as pathological ones. Lessened viscosity in the terminal phase of the disease is an unfavorable prognostic sign.
