[The effect in vitro of permanent and variable magnetic fields on immunocompetent blood cells from patients with hemoblastoses and non-Hodgkin's malignant lymphoma].

Immunologic parameters were studied in 54 patients with myeloma multiplex (MM), acute leukosis (AL), non-Hodgkin's disease (NHD) before and after in vitro exposure of these patients' blood to weak constant magnetic field (CMF) and CMF in combination with variable magnetic field (VMF). Blood irradiation with CMF over 60 min in AL and MM patients brought about in some cases enhancement of the expression of +CD3, +CD4 and CD8 together with augmentation of the immunoregulatory index. At the same time phagocytic activity of leucocytes got increased as did digesting capability and index of completeness of phagocytosis, i.e. exposure to CMF prevents a blockade of receptors for neutrophilic complement, increases their phagocytic activity and antibody-dependent cellular cytotoxicity. In studying immunocompetent cells of NHD patients' blood, T-helpers were found out to be moderately decreased, with T-suppressors to a greater extent so. Study of CMF and VMF effects on MM and AL patients' blood has shown an immunocorrective action thereof though to a lesser extent so.

[Prognostic value of some biochemical and immunologic factors in the management of multiple myeloma].

The results of examination of 58 patients in the course of multiple myeloma (MM) are discussed. Prognosis for LDH, beta2-m and FNO-alpha administration was evaluated versus immunochemical pattern, tumor mass and response to different chemotherapeutic regimens. High prognostic value of serum levels of LDH, beta2 and FNO-alpha in MM patients was observed. The method may be useful in carrying out chemotherapy and detecting recurrences and chemoresistance.

[The in-vitro effect of continuous and pulsed magnetic field on the colony-forming ability of bone marrow cells from hematologic patients].

The investigation was concerned with the effect of magnetic field on the colony- and cluster-forming potential of myelo- and monocytopoietic precursor-cells in different blood diseases. Bone marrow from 72 patients was exposed in vitro to low continuous magnetic field and to intermittent continuous and pulsed one--from another 22 patients. The ADMT-Magnipulse source was used. Magnetic field proved a biologically-active factor capable of changing the clone-forming properties, proliferative and, possibly, differentiating potential of hemopoietic precursor-cells.

[The efficacy of polychemotherapy programs in treating multiple myeloma patients].

AIM: To compare efficiency of the programs MCVP, VCAP and ARA-COP in the treatment of multiple myeloma (MM) as regards completeness of the response, duration of the remission and toxicity. MATERIALS AND METHODS: A total of 41 MM patients entered the study (27 females, 14 males, age from 41 to 72 years, MM duration from 1 month to 8 years). 16, 10 and 15 patients were treated according to MCVP, VCAP and ARA-COP programs. RESULTS: Both in the resistant and primary patients the response was the highest to ARA-COP treatment. The remission or stabilization was achieved in 93.4% of patients. VCAP program was less effective. However, clinicohematological remission was achieved in 50% of patients. This program is rather heart toxic. MCVP program was the least effective. Survival was followed up in 16 patients (10 MCVP, 3 VCAP and 3 ARA-COP patients). The survival was 20-62, 16-36, 23.6-64.8 months for ARA-COP, VCAP and MCVP, respectively. CONCLUSION: ARA-COP program proved most effective of the three programs both in primary and drug-resistant patients. VCAP and MCVP programs are less effective but can be used in primary management of MM patients.

[The efficacy of treatment with native lymphoblast alpha-interferon (Wellferon) in patients with chronic myeloleukemia].

Data are submitted on treatment with vellferon of patients with chronic myeloid leukemia (CML) in the chronic phase of the illness. The data obtained suggest that vellferon has a pronounced therapeutic effect in CML patients. The drug has a positive effect on both the clinical- and hematological status and cytogenetic changes in bone marrow cells. After 3-month treatment of patients with vellferon the sizes of the liver and spleen returned to normal as did the peripheral blood leucocyte and platelet counts. Two patients revealed a minimum cytogenetic response to therapy, one patient achieved a complete cytogenetic remission, and only 1 patient failed to demonstrate a cytogenetic response. A six-month therapy was associated with a complete clinical-and-hematological remission in 4 (80%) patients, with its duration ranging between 4 to 5 months, all patients maintaining a minimum cytogenetic response to vellferon treatment. Thus, treatment with vellferon permits obtaining not only a clinical and hematological but also a cytogenetical response to therapy in a major proportion of CML patients.

[Effect of patients' gender on the course of chronic myeloid leukemia].

Chronic myeloid leukemia course was evaluated versus sex in 271 patients. Chronic stage involved more pronounced leukocytosis, thrombocytosis and splenomegaly in females, the latter showing higher susceptibility to anemia. As a result, treatment has to deal with a greater mass of tumor. A relatively longer survival time in males (44 and 42 months, respectively) suggest a higher effectiveness of therapy in such patients. When diagnosed, leukocytosis, thrombocytosis, enhanced splenomegaly and anemia should be regarded as factors of unfavorable prognosis.

[Effect of various chemotherapy regimens on survival and cause of death in patients with multiple myeloma (retrospective study)].

A retrospective analysis of survival time and causes of death has been undertaken in 109 patients treated for multiple myeloma (MM) at the Hematologic Clinic of the Russian Research Institute of Hematology and Transfusiology in 1979-1995. Group I included cases of mono- and group II-polychemotherapy (PCT). Survival in patients with MM increased significantly (from 27 to 43 months) after PCT. The main causes of death in MM patients of group I were chronic renal failure and infectious complications, while in group II-intoxication syndrome including blood toxicity, toxic hepatitis, etc. In cases of this grave disease, the effectiveness of therapy appeared to depend on both main and supporting treatment. Of great importance was profuse hydration using diuretics, active rheologic preparations, plasmapheresis and other counter-complication means. Following the administration of immunity correction drugs (thymaline, tactivin, immunoglobulin), infectious complication incidence in group II dropped from 22 to 8.3%. It is suggested that the effectiveness of MM therapy is determined by stage-by-stage approach, course cycling of PCT and adequate evaluation of its criteria. PCT courses should be supplemented with infusion detoxication measures, particularly, in cases of highly aggressive PCT schemes, and procedures of immunity correction, hemo-component therapy and plasmapheresis.

[A new method for evaluating the rheological properties of the erythrocytes in surgical patients with endogenous intoxication].

An examination of 30 patients against the background of using both the operative and conservative methods of treatment was performed in order to study the rheological properties of blood and selection of the most informative tests for diagnosis of microcirculatory disturbances in diseases of the gastrointestinal tract and gastroduodenal area. The authors propose methods of investigation of deformability and viscosity of erythrocytes in patients with the surgical diseases in question. The methods are simple, quite handy for any surgical hospital and polyclinic laboratories. In addition to being used in investigations of the aggregative ability of erythrocytes and viscosity of the whole blood, these methods can be of use in express diagnosis of rheological disorders. The endogenous intoxication in patients with the diseases in question was found to be accompanied by deep rheological disturbances. The deformability of erythrocytes and their viscosity index which can pass ahead of the shifts in other parameters characterizing the hemocoagulative and rheological properties of blood are changed. A conclusion is drawn that the parameters of the erythrocyte deformability and viscosity can be an additional criteria of the adequacy of therapy used for patients with the diseases in question.

[The importance of studies of the glycosaminoglycans in the peripheral blood leukocytes of patients in the diagnosis of the phases of chronic myeloleukemia].

To elucidate new diagnostic markers of chronic myeloid leukemia (CML) phases, we investigated quantitative and qualitative composition of glycosaminoglycans (GAG) of leukocytes from peripheral blood of 72 patients. Chronic CML phase was characterized by elevated GAG levels (2 times compared to normal values), weakening of anionic properties of chondroitin sulfate (CS) and high amount of heparan sulfate (HS). In CML transformation in the progressive phase overall concentration of GAG grew still higher, GAG fraction composition changed. In the blast crisis there was a sharp fall in the overall GAG, new electrophoretic fractions emerged. In the myeloid variant of the crisis an additional GAG component appeared (GAG-m), whereas in the lymphoid variant another component was found (GAG-1). It is suggested that the number and composition of GAG in peripheral blood leukocytes may serve markers of CML phase.

[Hemorrhagic and neurological complications in aplastic anemia patients].

Blood coagulation and rheology were studied with regard to severity of hemorrhagic syndrome and neurological complications in 44 patients with aplastic anemia (AA). Most of these patients appeared to have DIC-syndrome with serious rheological shifts. The patients suffered from hemorrhagic diathesis, nasal and gingival hemorrhages. With AA progression the severity and frequency of hemorrhagic complications increased. Neurological examination revealed in 9 of 12 AA patients organic affections of the nervous system with EEG evidence of bioelectrical shifts. In some patients neurological complications were registered prior to hemorrhagic. Therefore, to diagnose hemorrhagic complications in AA patients it is necessary to assess in details their hemocoagulation, blood rheology and neurological status.

[The effectiveness of reaferone in chronic myeloid leukemia assessed with ultrasonic and histomorphologic examination].

The effectiveness of reapherone administration for treatment of chronic myeloid leukemia has been studied using sonography of the liver and spleen as well as morphometry of histological patterns of the bone marrow. The study group included 17 patients (chronic disease-16; progressing disease-1). Ten patients received reapherone and 7-myelosan. Dynamic evaluation of scanning evidence for liver and spleen proved instrumental in assessing the effect of reapherone therapy of chronic myeloid leukemia. Similar tendencies were identified by sonography and morphometry applications: reapherone-treated patients revealed decreased liver and spleen, foci of fibrosis dissolving while the morphometric findings pointed to the decreasing indices of collagenous fibrosis and megacariocytic source of the bone marrow. It is suggested that reapherone acts upon the hemopoietic tissue indirectly-via endosal cells of the bone marrow stroma.

[Is the cytogenetic verification of chronic myeloleukemia always obligatory?].

To elucidate feasibility of accurate diagnosis of chronic myeloid leukemia (CML) without cytogenetic and molecular-genetic investigations as well as to specify CML diagnostic criteria, clinicohematological parameters were compared in two groups of patients: with Ph'-chromosome and/or rearrangement of fragment bcr (group 1), with unknown karyotype in whom detection of bcr fragment rearrangement was not made. Clinicohematological parameters in both groups were close in absolute value and underwent parallel changes in the course of leukemia progression. In group 1, patients in progressive and blastic phase compared to patients in chronic phase had a 14-fold increase in the number of additional cytogenetic anomalies. In patients with tumor transformation fragment bcr underwent rearrangement according to type B2A2. Thus, the diagnosis of typical CML variants is feasible without detection of Ph'-chromosome and/or rearrangement of bcr fragment. It is especially true and essential for patients in the chronic phase. The data obtained provide more accurate diagnostic criteria of CML.

[Neurologic complications of multiple myeloma].

The investigation was concerned with concomitant nervous disorders involved in multiple myeloma (MM) and evaluation of the role of neurologic symptoms in primary diagnosis of myeloma. The study included 37 patients in most of whom pain was the first syndrome of MM as well as various neurologic symptoms. The latter were caused by compression of the reticular cells of the roots of spinal nerves, separate nerves and cerebral and spinal cord tissue by malignant infiltrates. Pronounced and moderate changes in the bioelectrical activity of the cerebral cortex were identified by electroencephalography in 14 patients. Brain compression by direct contact or due to metastasis formation was shown to be mostly responsible for said differences which was confirmed by X-ray examination of skull bones and pathological liquor studies. The brain appeared to be particularly susceptible to that pathology in those patients who had not received special therapy for a long time. Among other factors were paraproteinemia and uremia-related intoxication. Early diagnosis of neurologic complications in MM is absolutely key in selecting adequate therapy.

[The prognostic significance of the beta 2-microglobulin level of the blood serum in multiple myeloma patients].

Pretreatment and treatment content of beta 2-microglobulin in blood serum was measured in 52 patients with multiple myeloma stage IIA, IIB, IIIA and IIIB. The above concentrations before treatment correlated with the disease stage, rose in the exacerbation, remained unchanged in stabilization, fell in effective therapy and returned to normal values in remission. However, in pronounced renal insufficiency (stage IIIB) the remission beta 2-microglobulin levels remained high. A 6 mg/l concentration proved critical for survival. The survival median at pretreatment beta 2-microglobulin content above 6 mg/l was 14 months, under 6 mg/l--49 months. Routine estimation of beta 2-microglobulin levels is necessary for better control over multiple myeloma treatment, timely change of cytostatic drugs.
