Dual mechanisms of potentiation of murine antigen-specific IgE production by cyclosporin A in vitro.

Concomitant administration of cyclosporin A (CsA) with Ag has been shown to augment the production of Ag-specific IgE in vivo. We demonstrate that addition of CsA also markedly potentiated Ag-specific IgE in vitro. Low doses of CsA (3 and 10 ng/ml) added at the time of culture initiation selectively enhanced Ag-specific IgE but not IgA or IgG1 production, whereas higher doses (30 ng/ml) suppressed production of all the isotypes. Augmented IgE production was found to correlate with enhanced production of IL-4 and diminished production of IFN-gamma. Delayed addition (after 2 days) of low doses of CsA to Ag-stimulated cultures did not potentiate IgE production, even though CsA differentially affected levels of IL-4 and IFN-gamma. CsA enhanced Ag-mediated cognate T/B interaction was not affected by neutralizing doses of anti-IL-4, suggesting Ag-mediated lymphocytic "synapses" may be inaccessible to anti-IL-4. The effect of CsA on Ag presentation was determined by pulsing peritoneal exudate cells, spleen cells, or primed B cells with Ag and low doses of CsA before incubation with primed splenocytes. Enhanced Ag-specific IgE responses were detected with no effect on IL-4 or IFN-gamma levels. Thus, our study indicates that CsA potentiation of Ag-specific IgE response is due to cumulative action of CsA on two independent pathways: first, CsA differentially modulates IL-4 and IFN-gamma levels during the early phase of cognate Th2/B cell interaction; and second, CsA directly affects APC and IgE isotype-specific amplifying cellular components without apparently affecting the secretory levels of IL-4 and IFN-gamma. Dual mechanisms of CsA-potentiated IgE production are consistent with the hypothesis of two-tiered T cell regulation of Ag-specific IgE responses.

Genesis of host IgE competence: perinatal IgE tolerance induced by IgE processed and presented by IgE Fc receptor (CD23)-bearing B cells.

A murine model for studying life-long IgE tolerance was previously developed in this laboratory by perinatal IgE injection into neonates. Herein, we demonstrated that normal and immortal CD23+ B cell lines presented processed IgE via CD23-mediated endocytic pathway and triggered perinatal IgE tolerance. The observations were as followed: (a) CD23 on normal B cells or B cell hybridomas mediated IgE-dependent perinatal IgE tolerance and total IgE deficiency; and lack of either antigen-specific IgE or total IgE did not correlate with elevated levels of autologous anti-IgE in individual mice; (b) IgE tolerance-inducing capacity of CD23+ B cell hybridomas was augmented by treatment with antigen-IgE complexes or interleukin 4, and significantly inhibited by anti-CD23 prior to IgE pulsing; (c) antigen-IgE complexes were endocytosed and degraded in acid hydrolases-containing vesicles; and IgE tolerance was abrogated by treating IgE-pulsed 17A11 at 4 degrees C or 20 degrees C followed immediately by fixation, and by treating IgE-pulsed 17A11 with metabolic inhibitors that elevated intracellular pH of the endocytic vesicles. In conclusion, this study suggested that one pivotal step of genetic control of IgE responses may be exercised at the early developmental stage of T cells of the IgE lineage, and that CD23 may facilitate capture of endogenously secreted IgE, and mediate endocytic processing and presentation of self IgE epitope(s), and thus contribute to the genesis of host IgE competence.

Effects of lithium and haloperidol on human sperm motility in-vitro.

Two psychotropic drugs, lithium and haloperidol, were evaluated for their in-vitro effects on sperm motility using a transmembrane migration method. Sperm motility was measured either immediately after semen had been mixed with the drug or after a 2 h incubation period at 37 degrees C. Lithium inhibited human sperm motility in a dose-dependent manner with an EC50 of 10 mM when the semen-lithium mixture had been incubated. Sperm motility was increased to 127% of control when semen had been incubated with 0.027 microM haloperidol; this concentration was within the therapeutic range.

Evidence of direct activation of adenosine A1 receptor by 5'-adenosine monophosphate in isolated guinea pig atrial myocytes.

The electrophysiological effect of 5'-adenosine monophosphate (AMP) was examined in isolated atrial myocytes of guinea pig. Membrane potential and ionic currents were measured by the tight-seal, whole-cell patch-clamp technique. AMP caused the shortening of atrial action potential in a dose-dependent manner. In voltage-clamp experiments, AMP (3-10 microM) caused the activation of IKACh as well as the decrease in basal ICa. Prolongation of action potential duration caused by isoproterenol (20 nM) was antagonized by AMP (10 microM). Isoproterenol (20 nM) occasionally caused the sustained rhythmic activity and the subsequent application of AMP (10 microM) terminated it. AOPCP (10 microM), which inhibits 5'-nucleotidase and hence prevents breakdown of AMP, did not significantly attenuate the effect of AMP on shortening action potential. However, the application of adenosine deaminase (1 U/ml), which deaminates adenosine to inosine, partially reversed the shortening of action potential caused by AMP (10 microM). These results indicate that (1) AMP could activate IKACh and decrease basal ICa simultaneously, and antagonize the isoproterenol-stimulated action potential prolongation; and (2) the observed electrophysiological effect of AMP in whole-cell preparation is attributed to AMP per se as well as its degradative product, adenosine.

Construction of a genomic DNA library of the Toxoplasma gondii ZS2 strain, screening of specific clones, and DNA diagnosis of toxoplasmosis.

We have constructed a genomic DNA library of the Toxoplasma gondii ZS2 strain and isolated a specific cloned DNA sequence from this organism. The restriction map of this cloned 1.1-kb DNA fragment was analyzed. Southern and dot-blot analyses showed that the 32P-labeled DNA fragment hybridized to parasite DNA, to DNAs from peripheral blood leukocytes and the thymus of baby pigs that were artificially infected with T. gondii, and to DNAs of T. gondii-positive anencephalic and hydrocephalic fetuses. It did not hybridize with DNA from controls, (i.e., normal human and baby pig peripheral blood leukocytes, spleen of normal mice, Plasmodium falciparum, Pneumocystis carinii, and pBR322). As few as 100 T. gondii parasites or 500 pg of purified DNA from T. gondii can be detected by dot-blot hybridization. This probe method was specific and sensitive, and has been used successfully in detecting various clinical cases of toxoplasmosis with T. gondii.

[The strength and functional performance of patients with Duchenne muscular dystrophy based on natural history].

Duchenne muscular dystrophy (DMD) is a disease of progressive muscular weakness and wasting. This study is designed to evaluate the muscle strength and functional performance of patients with DMD during the natural progression of this disease. This study comprises a sample of 35 subjects who were confirmed to have DMD. Manual muscle testing (MMT) was used to evaluate muscle strength, the Brooke functional scale to rate the motor function of the upper extremity, and the Vignos functional scale to rate the motor function of the lower limbs. The results showed a significant positive correlation between age and the decrement in strength, i.e. a one year increment in age led to a 3.9% decrease in the average muscle strength. The strength loss always occurred in a typical pattern proceeding from the lower extremities to the upper extremities, and from the proximal to the distal parts. There was a significant negative correlation between muscle strength and both the Brooke and Vignos functional scales. These findings may suggest that decreased muscle strength results in a progressive worsening of the functional performance of the extremities. In addition, the Brooke scale is significantly correlated with the Vignos scale through the natural course of DMD. The high percentage (90.5%) of subjects without the long leg braces needed for ambulation indicates an inadequate rehabilitation care for DMD patients in this country.

[Muscle histochemical changes in rats swimming under high temperature].

The aim of this study was to investigate the effects on muscle tissues of increasing amounts of exercise under normal and high temperatures. An experimental swimming test was applied to completely exercised rats. There were 42 adult male rats divided into 4 groups, including normal control rats, rats which swam to death under room or high temperature conditions, and rats swimming under high temperatures. Immediately after the exercise, the soleus (SOL) and extensor digitorum longus (EDL) muscles were removed by surgical excision. All specimens were frozen in isopentane cooled to -70 degrees C in liquid nitrogen. Cryostat cross sections were cut and stained using histologic and histochemical staining methods to observe pathological changes. The major changes included interstitial edema, muscle fiber atrophy, muscle fiber necrosis, hypercontracted fibers, and ragged red fibers. The rats exercised to death under high temperatures had more apparent changes than those not exercised to death. These results indicate that pathological changes in muscle will be brought about by increasing the amount of exercise and environmental temperature. Since there are more fast twitch fibers in the EDL than in the soleus, pathological changes in muscle tissues are more severe in the EDL.

[Diagnosis of toxoplasmosis by polymerase chain reaction].

Based on the partial sequences of the specific DNA cloned fragment from T. gondii (ZS2 strain) a specific primer of the oligonucleotide for the Toxoplasma gondii DNA sequence has been designed and synthesized in our laboratory. The method of the DNA diagnosis for toxoplasmosis by polymerase chain reaction (PCR) has been established. A specific amplified band was shown in the PCR products from DNAs of T. gondii and seven manifold terata. The DNAs from the peripheral blood leukocytes of fifty normal individuals and seventy-five patients as infants with hepatitis syndrome and pregnant women with previous abnormal birth histories were diagnosed by PCR. Among the seventy-five diagnosed cases, ten were positive. The normal individuals all were negative. Using 32P-cloned T. gondii specific DNA fragment as probe and Southern blot assay, the results showed that the probe only hybridized to the specific amplified DNA bands, but did not hybridize to the amplified DNA products of negative cases. Our PCR method is a rapid, highly specific and sensitive one for detecting toxoplasmosis as compared with DNA probing, immunoassay and animal inoculation.

[Brainstem auditory evoked potentials in diabetes mellitus].

It is well-known that diabetic patients develop peripheral and autonomic neuropathy, and recent review has also suggested the occurrence of central pathway abnormality in diabetics. In this article, we conducted the BAEP study on 61 cases of NIDDM and 11 cases of IDDM. Peak latency, interpeak latency (IPL) and peak amplitude of BAEPs were analyzed in each case. For further correlation, the motor and sensory nerve conduction velocities of median nerve, the blood sugar, the serum HbA1c were measured. Two nondiabetic groups, age and sex matched with NIDDM and IDDM groups, were used as control. In NIDDM group, the results showed prolongation of all peak latency and IPL except peak latency of wave II and wave IV in the left side and bilateral IPL III-V. There was no statistically significant amplitude difference between NIDDM and age-matched control group. The result of IDDM group revealed prolongation of all peak latency and IPL, except the right IPL III-V. As for amplitude, waves III and V in the right side and waves I and V in the left side were reduced as compared with the age-matched young control group. There was no statistically significant difference in all peak latencies and IPLs between NIDDM and IDDM groups. In both groups of NIDDM and IDDM, the MNCV and SNCV of median nerve were significantly delayed in conduction. The prolongation of III and V peak latency had a linear correlation with their amplitude reduction. In conclusion, both peripheral and central conduction dysfunction occur in both IDDM and NIDDM patients.

[Autonomic nerve preserving operation for rectal carcinoma: preliminary postoperative urinary function of 15 cases].

The autonomic nerve preserving operation was performed on 15 patients with rectal carcinoma (9 men and 6 women), from August 1990 to February 1991. The average age was 57.2 years ranging from 28 to 70 years. Low anterior resection was performed on 6 patients having middle rectal carcinoma. Seven Colonic anastomosis and two abdominoperineal resections were performed for lower rectal carcinoma. The average operation time was 4 hours and 22 minutes. The average blood loss was 1560 cc. Urinary functions were evaluated pre- and post-operatively. None of the patients needed re-catheterization postoperatively. Urodynamic studies showed no significant differences. We recommend this operation for rectal cancer because it prevented the postoperative autonomic dysfunction without affecting the curability.

Chronic progressive external ophthalmoplegia with NADH-CoQ reductase deficiency: report of a case.

A thirty-two year old female had chronic progressive external ophthalmoplegia (CPEO), exertional fatigue, dysarthria, dysphagia, and bilateral hearing impairment. Histochemical stains, obtained from the right vastus lateralis, showed ragged-red fibers and wide-spread abnormalities in the number, size, and the structure of mitochondria under electronomicroscopic examination. A biochemical analysis showed a low activity of NADH-cytochrome C reductase, NADH dehydrogenase and a normal activity of succinate cytochrome C reductase and cytochrome C oxidase. This data suggests a specific defect in the NADH dehydrogenase of complex I (NADH CoQ reductase). We believe that this is the first biochemically defined mitochondrial myopathy reported in Taiwan and provides additional evidence for the existence of biochemical heterogeneity in mitochondrial disorders of CPEO.

Hypothalamic amenorrhea in a case of mitochondrial encephalomyopathy.

A 26-year-old female with myoclonus epilepsy associated with ragged-red fibers is reported. Clinically, she had myoclonus epilepsy, cerebellar ataxia, a bilateral neurosensory type hearing loss, retinitis pigmentosa and short stature. She also presented with primary amenorrhea and poor development of secondary sexual characteristics. Endocrinologic studies revealed that hypothalamic dysfunction was the most plausible cause of her primary amenorrhea. Magnetic resonance imaging showed marked dilatation of the third ventricle indicating thalamic and hypothalamic degeneration. We conclude that hypothalamic dysfunction may be one of the characteristic features of mitochondrial encephalomyopathies.

Adult-onset, autosomal dominant, rimmed vacuolar myopathy with limb-girdle distribution--a preliminary report.

Adult-onset, autosomal dominant myopathy with limb-girdle distribution in a Chinese family is reported. Nine members were affected with characteristic proximal weakness and pelvifemoral-preceded scapulohumeral involvement. The facial, extraocular and bulbar muscles were spared. Rimmed vacuoles were the most obvious characteristics in the myopathologic study of the biopsy specimens from four patients. In a review of the previous literature, we found three reported families with clinical and myopathologic features identical to ours, and these families are viewed as a distinct subgroup of myopathy.

A possible role of heat shock proteins in human sperm motility.

The testicular spermatogenic stem and seminiferous tubular cells selectively synthesize heat shock proteins (Hsps) during heat stress. Hsps, synthesized from testicular cells and leukocytes, are identical in molecular masses as well as chemical properties. In this study, we induced the Hsps from leukocytes and investigated their in vitro effects on human sperm motility. Semen samples were divided into two parts, washed and unwashed. The whole blood was heated in 43 degrees C for 15 minutes for induction of Hsps. A trans-membrane migration method was used to examine the effect of heated blood plasma on human sperm motility. The main heat-induced proteins of leukocytes were detected by 2-D electrophoresis and Coomassie blue stain. Leukocytes treated by heat produced a large amount of Hsp72 and Hsp80, while only a small amount was observed in that of non-heated leukocytes. The heated blood plasma inhibited motility of washed sperm in a manner that was dose-dependent. In the presence of seminal plasma fluid, however, the inhibitory effects of heated plasma on human sperm motility could not be observed. It was concluded that the heat-induced substance(s) from leukocytes, which being highly possible the Hsps, interfered the mobility of wash human sperm and the inhibition might be antagonized by seminal plasma.

[Pattern shift visual evoked potentials in diabetes mellitus].

Functional exploration of the optic pathways with pattern shift visual evoked potentials (PSVEP) has been rapidly accepted as a non-invasive method of investigation of diabetics. For this article, we conducted the PSVEPs study on 46 cases of NIDDM and 13 cases of IDDM. The peak latency, interpeak latency and evoked amplitude of P100 were analyzed in each case. For further correlation, the motor and sensory nerve conduction velocities of the median nerve, blood sugar, serum HbA1c, and duration of DM were measured simultaneously. Two nondiabetic control groups which matched the age and sex of the NIDDM and IDDM groups were used for comparison. In the IDDM group, the results showed prolongation of all peak and interpeak latencies (IPL) except the peak latency of N75 on the right side. The P100 amplitude was reduced as compared with the age-matched young control group. The interocular P100 latency difference (ILD) was not statistically significant between the IDDM group and the age-matched control group. The results of the NIDDM group revealed prolongation of all peak latencies and IPLs. The P100 amplitude and ILD showed no statistically significant difference between NIDDM and the age-matched control group. The prolongation of N 75 peak latency exhibited a linear correlation with duration, HbA1c and median nerve SNCV.

Baclofen intoxication: report of four cases and review of the literature.

Four cases of baclofen intoxication are reported, with a review of 33 cases from the literature. Analysis of these 37 cases suggests that there are two types of baclofen intoxication syndrome. Patients with acute intoxication present with four major clinical manifestations: encephalopathy (disturbance of consciousness and/or seizure), respiratory depression, muscular hypotonia, and generalized hyporeflexia. Patients with chronic intoxication present with hallucinosis, impaired memory, catatonia, or acute mania. The acute intoxication syndrome has a faster onset, shorter duration, more severe clinical manifestations, and higher incidence of seizures than the chronic intoxication syndrome. Baclofen intoxication, although it may cause grave encephalopathic manifestations and electroencephalographic findings, has a benign outcome if actively managed.

Effects of aircraft noise on hearing and auditory pathway function of airport employees.

The effects of aircraft noise on hearing and auditory pathway function were studied in 112 airport employees, both by audiometry and brainstem auditory evoked potentials (BAEPs) to evaluate cochlear function and to verify the possibility of retrocochlear involvement. Employees were divided into five groups according to their daily jobs. Group A was made up of 23 maintenance workers, Group B of 20 firemen, Group C of 24 policemen, Group D of 34 airline ground staff, and Group E of 14 civil servants. The typical audiogram pattern of noise-induced hearing loss (NIHL) was a dip at 3 or 4 kHz and moderate hearing loss in the frequency range of 6 to 8 kHz. The results of audiograms in this study revealed the prevalence rate of high-frequency loss in all employees was 41.9%. The incidences of NIHL were highest in the groups of maintenance workers (65.2%) and firemen (55.0%), who are almost continuously exposed to aircraft noise. As for the BAEPs, both click threshold and latencies showed that the impairment was most severe in the groups of maintenance workers and firemen. There was prolongation in central conduction time, shown mainly in intervals of I-V and III-V. This suggests that involvement of the central auditory pathway, especially between the pons and midbrain, is present. In summary, the degree of auditory damage coincided with job patterns. Furthermore, damage of both peripheral cochlear organs and the central auditory pathway by high-frequency aircraft noise exposure was confirmed.
