Choledochal cyst and neoplasm: a comprehensive review of 106 cases and presentation of two original cases.

Choledochal cysts are prone to complications: cholangitis, biliary cirrhosis, portal hypertension, lithiasis, rupture, pancreatitis, and carcinoma. The coincidence of choledochal cysts and neoplasia ranges from 2.5 to 26 per cent. One hundred six cases of choledochal cysts with neoplasms have been collected from the literature. We have tabulated the results of 68 cases found to have a neoplasm at the initial laparotomy and of 38 patients with pristine choledochal cysts treated electively who subsequently developed a neoplasm. Two original cases are presented. The primary site of neoplasia was not confined only to choledochal cysts. There appears to be a propensity for malignancy to develop anywhere in the biliary tract or gallbladder or pancreas in conjunction with the choledochal cyst. Accompanying choledochal cysts is a high incidence of an anomalous relation at the pancreaticobiliary junction with subsequent malignancy formation. A pathogenetic basis is postulated.

Pseudocholinesterase--a clinical assessment.

Serum pseudocholinesterase (PC) levels were analyzed retrospectively against serum albumin, total lymphocyte count, and hematocrit using 4 to 15 data sets in 17 seriously ill surgical patients who received total parenteral alimentation for 4--21 days. A statistically significant correlation was found between PC and serum albumin but not between PC and total lymphocyte count or hematocrit. Low PC levels are found with acquired nutritional deprivation and in certain abnormal genetic states. When the use of succinylcholine is contemplated, PC levels should be measured in patients who have a low serum albumin or have an acquired nutritional deficiency.

Hyperplastic Gastropathy. Analysis of 50 selected cases from 1955-1980.

Hyperplastic gastropathy remains a rare condition. Fifty cases of hyperplastic gastropathy (adhering to rigid pathologic and clinical criteria) were analyzed from the English literature for the past 25 years. This was compared to a similar review of 50 cases published 25 years ago. A clearer pathologic classification-mucous cell hyperplasia, glandular cell hyperplasia and mixed mucous-glandular cell hyperplasia is recommended. The etiology and pathogenesis remain obscure. The clinical characteristics and medical and surgical therapy are discussed. The course appears uncertain as does the relationship to gastric carcinoma.

Polypoid villous adenoma of the stomach. A case report.

Clinical symptoms, endoscopic findings and radiographic features of a polypoid villous adenoma of the stomach are described. A working classificaiton of gastric epithelial polyps and a table of differential diagnoses are presented. The clinical significance of this lesion is discussed.
