Unusually large cisterna chyli: US and MRI findings.

We report a rare appearance of cisterna chyli appearing as a giant cystic structure on routine abdominal ultrasonography (US). Diagnosis was established with color Doppler US and after magnetic resonance imaging. This report describes an unusual appearance of the cisterna chyli that radiologists should be familiar with, especially on the routine conventional gray-scale US to avoid mistaking it for a pathologic condition.

MR imaging of fetal abnormalities.

Our purpose was to evaluate the capability of ultrafast single-shot fast spin-echo MR imaging to assess normal fetal anatomy and abnormalities of different fetal organ systems. Fetal MR imaging was performed prospectively in consecutive 40 pregnant women because of abnormal findings or suspected fetal abnormalities on prenatal US. No statistically significant difference between US and MR imaging was found for the detection of abnormality in any organ system. MR imaging was slightly superior to US with regard to cerebral abnormalities only. In four (10%) of 40 fetuses, additional information provided by MR imaging altered counseling. However, MR imaging of the extremities-face and soft tissues was limited because of the lack of real-time information.

Urachal remnants in asymptomatic children: gray-scale and color Doppler sonographic findings.

PURPOSE: We undertook this study to evaluate the frequency of visualization and the sonographic appearances of urachal remnants (URs) in asymptomatic children. METHODS: One hundred eighty-two children without any urachus-related symptoms underwent sonography of the prevesical region and urinary bladder with high-frequency transducers. RESULTS: A UR was visualized in 180 (99%) of the children. The URs had a mean length +/- standard deviation of 13 +/- 5 mm. Most of the URs were ovoid, and most demonstrated a moderate protrusion into the bladder cavity. The URs with a central echogenic area were larger than those without central echogenicity. In 36 (61%) of the 59 URs that were evaluated with color Doppler sonography, vascular signals were demonstrated within the lesion. The URs with internal vascular signals were significantly larger than those without internal vascular signals. CONCLUSIONS: The gray-scale and color Doppler sonographic characteristics of URs seem to be related to their size and their degree of involution rather than to the age of the child. We think that URs are present in almost all children. URs should be considered normal findings if they are asymptomatic and their length is not significantly greater than 22.5 mm, the 95th percentile in our study.

Prenatal sonographic detection of nasopharyngeal teratoma.

We present the case of a 34-year-old pregnant woman who had an elevated maternal serum alpha-fetoprotein level and sonographic findings of a semisolid mass protruding from the fetus's oral cavity. The large, heterogeneous mass filled the oropharynx and nasopharynx. Abnormal Doppler waveforms were detected in the umbilical artery of the fetus, who died in utero. Postmortem examination revealed a nasopharyngeal teratoma.

Portal venous system aneurysms: report of five cases.

Until recently aneurysms in the portal venous system were considered to be very rare lesions. This opinion has largely been changed by the increasing number of cases reported in recent years. In this paper we report the cases of five patients with portal venous system aneurysms, including one with splenic vein aneurysm. One patient had associated portal hypertension. The reexamination of two patients 2 years later showed no change in the aneurysms. The sonographic features and related literature are reviewed. In the light of this series and the information in the literature, we recommend that portal venous system aneurysms should no longer be considered exceptionally rare entities.

Regression of splenic vein aneurysm following resolution of splenomegaly.

Aneurysms of the portal venous structures have rarely been reported. We present a case of a splenic vein aneurysm that appeared and regressed in parallel with splenic size during the course of a systemic infection in a patient with leukemia. The aneurysm was demonstrated with B-mode and color Doppler sonography and computed tomography. After appropriate therapy, the spleen diminished in size. This decrease was accompanied by regression of the aneurysm. Color Doppler sonography enables the noninvasive detection, diagnosis, and follow-up of splenic vein aneurysms.

Carotid steal: report of ten cases.

The blood flow may be diverted from the external to the internal carotid artery via the carotid bulb in the absence of flow in the common carotid artery. We aimed to investigate the prevalence, hemodynamics, and clinical features of this condition. Reviewing the records of color duplex ultrasonographic examinations, we found carotid steal in 12 carotid arteries of 10 patients. In three patients flow in the ipsilateral external carotid artery was bidirectional. The steal had been demonstrated angiographically in six patients. The cause of the phenomenon was atherosclerosis, Takayasu arteritis, or trauma. Awareness of this pathway of collateral circulation may contribute to diagnostic work-up and expand management alternatives.

Image-directed color Doppler ultrasonography of kidney in systemic lupus nephritis.

We evaluated intrarenal arterial waveforms with image-directed color Doppler ultrasonography in 21 patients with systemic lupus erythematosus (SLE). The systolic-diastolic ratio, resistive index, and pulsatility index were compared with serum creatinine levels, creatinine clearance, and quantitation of urinary protein excretion, as well as with histopathologic scores of specimens obtained from 9 patients who underwent renal biopsy. Doppler parameters were in the normal ranges in all patients, without showing significant correlation with any of the histopathologic scores or laboratory parameters except creatinine levels. We conclude that image-directed color Doppler ultrasonography is of no practical value in the evaluation of lupus nephritis during the early stages of the disease.

Image-directed color Doppler ultrasonography in the evaluation of superficial solid tumors.

Image-directed color Doppler ultrasonography (ICDUS) studies of 86 patients with superficial solid masses yielded significantly lower resistive index (RI) values in acute inflammatory lesions, but no significant difference between the maximum systolic flow velocities (S) of the patient groups with malignant, benign, and acute inflammatory lesions. When analyzed separately, the malignant soft-tissue tumor subgroup was shown to have significantly higher mean RI compared to that of the malignant node subgroup. We conclude that RI may be useful in the differentiation of acute inflammatory masses from other pathological entities. Malignant soft-tissue tumors, especially sarcomas, may have different Doppler features from those of carcinomatous tumors.

Image-directed and color Doppler ultrasonography in the diagnosis of postbiopsy arteriovenous fistulas of native kidneys.

Using image-directed and color Doppler ultrasonography (ICDUS), we examined 65 patients with single kidney biopsy and diagnosed one arteriovenous fistula (AVF) in each of 8 kidneys. Three of them were associated with pseudoaneurysms. Three of the patients with AVF who presented with macrohematuria underwent angiography. Therapeutic percutaneous embolization was performed in 2 of them. The remaining 6 patients were followed up with ICDUS. All the lesions had disappeared at the end of a 6-month period. We conclude that ICDUS is an easy and noninvasive imaging technique in the diagnosis of postbiopsy native renal AVFs.

Case report: computed tomography findings in lipoid proteinosis: report of two cases.

Lipoid proteinosis (Urbach-Wiethe disease (Urbach, E and Wiethe, C, Lipoidosis cutis mucosae, Virchows Arch. Patholog. Anat., 273, 285-319 (1929)) is a rare generalized disease with autosomal recessive inheritance. It most often involves the skin and mucosal membranes of the aerodigestive tract; but also involves the central nervous system, lung, lymph nodes and striated muscles. We present the computed tomography findings in the cranium and larynx of two siblings with lipoid proteinosis.

Intrarenal Doppler findings of upstream renal artery stenosis: a preliminary report.

To diagnose 60% or greater diameter-reducing stenosis of the renal artery (RAS), color Doppler imaging (CDI) and angiography were performed on 44 hypertensive and 16 normotensive cases. In this prospective, double-blind study we evaluated the related intrarenal waveform changes. In both the right and left kidneys of normotensive cases, at the level of interlobar arteries, the results indicated a symmetric finding in peak systolic/end diastolic velocities (S/D), pulsatility index (PI) and resistive index (RI) ratios. The results in the stenotic kidneys among the hypertensive group indicated that index values were significantly lower in the stenotic kidneys than those of the contralateral kidneys, the kidneys of control and nonstenotic hypertensive cases. To quantify this observation we suggest "perfusion indexes," which require more studies on a greater number of hypertensive cases. It was concluded that measuring the index values of intrarenal arterial waveforms is easier and more accessible in diagnosing RAS than obtaining those of the main renal artery.

Color-Doppler imaging in acute scrotal disorders.

Eighteen patients who referred to our hospital with acute scrotal pain and swelling were evaluated with color-Doppler imaging (CDI) after the initial clinical examinations. Five of these subjects were considered to have testicular torsion as diagnosed with CDI. These diagnoses were surgically confirmed, and appropriate treatments were performed. CDI findings of 13 patients were in accordance with inflammatory pathologies (epididymitis and/or orchitis). These patients received medical treatment. The CDI diagnoses of all patients were in agreement with the final clinical diagnoses. We conclude that CDI, which is a practical, rapid and high-resolution technique, promises to be the leading and decisive scanning method in acute scrotal disorders.
