Cytokeratin fragment 19 and squamous cell carcinoma antigen for early prediction of recurrence of squamous cell lung carcinoma.

Sixty patients with squamous cell carcinoma (SCC) of the lung, including 25 cases with recurrence and 35 cases without recurrence 1 year after operation, were enrolled in this study. The serial serum levels of cytokeratin fragment 19 (CYFRA 21-1) and SCC antigen were measured before operation and 1 week, 1 month, 3 months, 6 months, 9 months, and 12 months after operation for early detection of recurrence. The results revealed that 1) mean serum values of CYFRA 21-1 were significantly higher at early and any times after operation in 25 patients with recurrent SCC when compared with 35 patients without recurrent SCC; and 2) mean serum values of SCC antigen were significantly higher until 9 and 12 months after operation, in 25 patients with recurrent SCC when compared with 35 patients without recurrent SCC. We conclude that CYFRA 21-1 is a better marker than SCC antigen for early prediction of SCC recurrence in the lung.

Tissue polypeptide-specific antigen and carcinoembryonic antigen for early prediction of recurrence in lung adenocarcinoma.

Fifty patients with lung adenocarcinoma, including 20 cases with recurrence and 30 cases without recurrence 1 year after operation, were enrolled in this study. The serial serum levels of tissue polypeptide-specific antigen (TPS) and carcinoembryonic antigen (CEA) were measured before operation and at 1 week, and at 1, 3, 6, 9, and 12 months after operation for early detection of recurrence. The results revealed that: 1) mean serum values of TPS were significantly higher at 1, 3, 6, 9, and 12 months after operation in 20 patients with recurrent adenocarcinoma, when compared with 30 patients without recurrent adenocarcinoma; 2) mean serum values of CEA were significantly higher at 9 months and 12 months after operation in 20 patients with recurrent adenocarcinoma, when compared with 30 patients without recurrent adenocarcinoma. We conclude that TPS is a better marker than CEA for early prediction of adenocarcinoma recurrence in lung within 1 year after operation. However, a longer follow-up study should be encouraged.

Monitoring the effect of anticoagulants on left atrial thrombi in patients with rheumatic heart disease: assessment with 111In-oxine-labelled platelet heart scintigraphy and transoesophageal echocardiography.

111In-oxine-labelled platelet heart scintigraphy (In-plt) was used to evaluate the activity of left atrial thrombi detected by transoesophageal echocardiography (TEE) in 16 cases of rheumatic heart disease. The effect of anticoagulants on the therapeutic intervention of these patients was evaluated by In-plt and TEE. In-plt was performed in all patients in serial pre- and post-anticoagulant therapy studies with anterior, 45 degrees left anterior oblique and left lateral views taken daily from the second to sixth day after injection of labelled platelets. After discontinuing anticoagulants for at least 1 week, only nine cases of left atrial thrombi were detected among the 16 cases by initial In-plt. After anticoagulants were again used for at least 3 weeks, all nine positive cases became negative on repeat In-plt. These results suggest that the diagnostic sensitivity of In-plt is lower than that of TEE. However, In-plt is more suitable and useful for evaluating the activity of left atrial thrombi after antithrombotic agent therapy and for monitoring the effect of anticoagulant therapy on left atrial thrombi in patients with rheumatic heart disease.

The role of rhinoscintigraphy in the evaluation of nasal mucociliary clearance function in patients with sinusitis.

Nasal mucociliary clearance is one of the homeostatic systems of the respiratory mucosa. Various rhinopathic conditions may involve alterations in nasal mucociliary clearance. The aim of this study was to assess the clinical role of rhinoscintigraphy in the evaluation of nasal mucociliary clearance function of sinusitis patients in comparison with a healthy control group. A single droplet of 99Tcm-labelled macroaggregated albumin (99Tcm-MAA) was deposited on the floor of the nasal meatus about 1 cm behind the mucocutaneous junction, and its course was followed with a gamma camera. The nasal mucociliary clearance was presented as the velocity (mm x min(-1)) of nasal mucociliary transport of the 99Tcm-MAA droplet. A significant difference in nasal mucociliary clearance was observed between the two groups, with a mean of 4.28+/-1.38 mm x min(-1) (4.3+/-1.35 mm x min(-1) in the right nostril, 4.27+/-1.42 mm x min(-1) in the left nostril) for the controls; and 1.8+/-0.64 mm x min(-1) (1.79+/-0.65 mm x min(-1) in the right nostril, 1.82+/-0.63 mm x min(-1) in the left nostril) for the patients with sinusitis. Rhinoscintigraphy is a safe, easy, quick and reliable imaging method for evaluating the ciliary activity of nasal mucosa and the nasal mucociliary clearance function in patients with sinusitis.

Detection of esophagitis by 99mTc-methoxyisobutylisonitrile chest SPECT.

UNLABELLED: Esophagitis is a common complication of gastroesophageal reflux disease (GERD). Unfortunately, an accurate diagnosis of esophagitis usually requires invasive endoscopy. The purpose of our study was to evaluate the use of noninvasive 99mTc-methoxyisobutylisonitrile (MIBI) chest SPECT to detect esophagitis in GERD patients. METHODS: Forty GERD patients who underwent endoscopy for diagnosis and grading of esophagitis according to the Savary-Miller grading system were included in this study. 99mTc-MIBI chest SPECT was performed on all patients to detect and grade esophagitis. RESULTS: On the basis of the endoscopic findings, the sensitivity, specificity, and accuracy of 99mTc-MIBI chest SPECT to detect esophagitis were 100%, 77%, and 95%, respectively. The correlation between the Savary-Miller grading system and 99mTc-MIBI chest SPECT grading was found to be good. CONCLUSION: Noninvasive 99mTc-MIBI chest SPECT has excellent sensitivity and good accuracy as a screen for esophagitis.

Unusual bilateral peritoneopleural communication associated with cirrhotic ascites: detected by TC-99m sulphur colloid peritoneoscintigraphy.

Hydrothorax is an infrequent but well-recognized complication in patients on continuous ambulatory peritoneal dialysis (CAPD) and patients with cirrhotic ascites. It is usually found on the right side; an incidence on both sides is rarely reported. We describe the scintigraphic diagnosis of unusual bilateral peritoneopleural communication in a patient with cirrhotic ascites.

Electrophysiological profile after inward rectifier K channel blockade by barium in isolated rabbit hearts. Altered repolarization and unmasked decremental conduction property.

AIMS: The aims of this study were to define the changes of cardiac conduction properties after selective IK1 blockade in isolated hearts. METHODS AND RESULTS: Intracardiac recording and stimulation in Langendorff-perfused rabbit hearts were used to define cardiac conduction properties after Ba2+, a well known IK1 blocker. Ba2+ prolonged the corrected QT interval and decreased, or even abolished, the amplitude of the T wave. The conduction time through the sinoatrial (SA interval), the atrioventricular node (AH interval) and His-Purkinje system (HV interval) were not significantly changed. However, at higher atrial pacing rates the HV interval was lengthened by Ba2+ in a frequency-dependent manner. The recovery curve of the His-Purkinje system (the relationship of H2V2 to V1H2) was changed by Ba2+ to a prematurity-dependent pattern (AV nodal behaviour). Such a response was neither observed with 4-aminopyridine (Ito blocker) nor with d-sotalol (IK blocker). The atrial and ventricular refractory periods as well as the relative refractoriness (measured by the difference between relative refractory period and the absolute refractory period) of the ventricular tissue were increased by Ba2+. CONCLUSION: The electrophysiological profile of selective IK1 block in isolated hearts revealed altered repolarization of cardiac tissue. The decremental conduction properties, which are normally present in the atrioventricular node (with a paucity of IK1), were unmasked by IK1 block in the His-Purkinje system. The T wave, particularly its amplitude, is more closely related to IK1 than to Ito.

Expression of the Brazil nut methionine-rich protein and mutants with increased methionine in transgenic potato.

A cDNA encoding the methionine-rich (19 mol% Met) protein in Brazil nut was placed under the regulation of CaMV 35S promoter and nopaline synthase terminator and introduced into the potato cultivar Russet Burbank via Agrobacterium-mediated transformation. To further enhance the Met content in the transgenic plants, chimeric genes containing four mutant constructs, BoxIa (with 5 additional Met), BoxIIa (2 additional Met), BoxIaIIa (7 additional Met), and BoxIIa2 (7 additional Met), were also generated by sequence modifications of the cDNA and transferred into potato. Analysis of the microtubers and leaves of the transgenic potato plants revealed, in general, with the exception of the BoxIIa2, the presence of mRNA transcripts of the expected size and the correctly processed Met-rich 9 kDa subunit polypeptides. The expression levels in the leaves among the various constructs and individual transgenic plants varied between <0.01% and 0.2% of total protein. The corresponding expression in the tubers was usually 2- to 4-fold lower than in leaves. In the case of BoxIIa2, which contains two tandem repeats of the BoxIIa mutant sequence, a larger (10.5-11 kDa) polypeptide was detected. These findings demonstrated that it is feasible to exploit the variable region of the Brazil Nut 2S protein for enhanced Met contents and perhaps for other desirable properties.

Comparative direct electrophysiological effects of propofol on the conduction system and ionic channels of rabbit hearts.

1. Propofol, an intravenous anaesthetic agent, can affect cardiac conduction but the ionic mechanisms have not been well defined. 2. This study measured the direct effects of propofol on the cardiac conduction system by using intracardiac recording/stimulation in Langendorff-perfused rabbit hearts. The underlying ionic mechanism was elucidated by using the whole-cell voltage clamp on rabbit isolated atrial and ventricular myocytes. 3. Propofol prolonged significantly the AV conduction (AH) interval at a clinically relevant concentration (3 microM). This AH interval prolongation was dose-dependent (3 to 100 microM). At higher concentrations, the AV nodal Wenckebach cycle length and its refractory period were also prolonged (10 to 100 microM). In addition, the conduction through the His-Purkinje system (HV interval) and the atrial tissue (SA interval), as well as the spontaneous cycle length, were lengthened dose-dependently (30 to 100 microM). 4. In isolated ventricular myocytes, Na current was decreased dose-dependently by propofol. In part this was due to a negative-shift of the steady-state voltage-dependent inactivation and a slowed rate of recovery from inactivation. The INa suppression by propofol was frequency-dependent. Propofol also blocked the ICa. The ED50 for peak current inhibition was 6.9 +/- 0.9 (n = 6) and 8.3 +/- 1.5 microM (n = 7) for INa and ICa, respectively. 5. The transient outward potassium current (Ito) of atrial myocytes was suppressed with an ED50 of 5.7 +/- 0.8 microM (n = 11), which was only partly caused by a left-shift of the steady-state inactivation. The inward rectifier K current (IK1) of the ventricular cells was reduced somewhat by propofol. 6. In summary, propofol can cause direct dromotropic and chronotropic effects on the cardiac conduction system, especially the atrioventricular node. These changes can be attributed, at least in part, to its direct dose-dependent suppression of the cardiac ICa, INa and Ito. Special concerns in the use of propofol anaesthesia for cardiac patients and the therapeutic antiarrythmic potential of propofol-like compounds are addressed.

Age-related propofol effects on electrophysiological properties of isolated hearts.

We compared the direct effects of propofol on the conduction system of neonatal (< 7 days) and adult (> 3 mo) rabbit hearts using intracardiac recording/ stimulation in Langendorff-perfusion and autonomic blockade. At a low concentration (3 microM), propofol caused a slight but significant lengthening of the atrio-ventricular (AV) conduction interval of the adult hearts, but not of neonatal hearts. At a higher concentration (10 microM and above), propofol significantly prolonged the AV conduction interval in a frequency-dependent manner in both neonates and adults. The AV Wenckebach cycle length was also lengthened, with the change more significant in the adults. However, with concentrations of propofol up to 100 microM, the neonatal hearts frequently (9 of 13 experiments) progressed to complete AV block, which was not observed in the adults. Conduction through the atrial tissue (SA interval) and the His-Purkinje system (HV interval), as well as the spontaneous heart rate, were all slowed by propofol at 30 microM or above. However, the lengthening of SA interval was more pronounced in the neonates, and only in the neonate was the atrial refractory period prolonged by propofol at 10 microM and above. We conclude that 3 microM propofol produces no significant direct effects on the neonatal cardiac electrophysiological properties, although AV conduction of the adult heart may be suppressed at this concentration of propofol. At higher concentrations, age-related propofol effects were demonstrated in the AV node and the atrial tissue.

[Effects of dingduwan on electric activities of gastrointestinal smooth muscle and effect of gastrointestinal motility].

Dingduwan (DDW) is a Chinese herbal medicine. Method of modified synchronous recording was used to study the electric activities of gastrointestinal (GI) smooth muscle and the effect of GI motility treated by DDW. A dosage of 3.5 g/kg for ingestion in rats markedly increased the average amplitude of GI slow wave, the total amplitude of GI motion, the rate of GI slow wave and intestinal motion (P < 0.05), but it had no significant effects on the frequency of gastroelectric slow wave and gastric motility (P < 0.05). DDW in the dosage of 5.2 g/kg for ingestion markedly promoted the recovery of intestinal intussusception in mice (P > 0.05). The results showed that DDW markedly increased the intestinal electricity and motility more than that of stomach.

Interaction of fibroblast growth factor with turkey embryonic myoblasts and myogenic satellite cells.

1. The interaction of fibroblast growth factor (FGF) with receptors on clonal-derived turkey embryonic and posthatch muscle cells was compared using saturation isotherms. 2. At least two binding sites, including a high affinity receptor and sites of low affinity, which are likely heparin sulfate proteoglycans, were observed on both embryonic myoblasts (EM) and myogenic satellite cells (SC). 3. The FGF binding affinities (Kds) were similar between SC and EM. Receptor Kds were also similar between SC derived from turkeys both selected and unselected for rapid, growth and skeletal muscle accretion rates.
