The role of technetium-99m methoxyisobutylisonitrile scintimammography in diagnosis of breast cancer in patients with mammographically dense breasts.

The purpose of this study was to assess the contribution of techentium-99m methoxyisobutylisonitrile (Tc-99m sestamibi) scintimammography in the diagnosis of breast cancer in 32 female patients with indeterminate mammographic probability of malignancy because of mammographically dense breasts. All the breast masses were removed and histopathological diagnoses were obtained in all cases. The results showed that scintimammography with Tc-99m sestamibi was positive in 21 patients (20 true-positive, 4 false-positive) and negative in 11 patients (7 true-negative, 4 false-negative). The diagnostic sensitivity, specificity and accuracy rates were 83%, 88% and 84%, respectively, in the differentiation of malignant from benign breast masses in the patients with mammographically dense breasts. The detection of malignant breast tumors by Tc-99m sestamibi scintimammography was independent of the density of the breast tissue. In conclusion, Tc-99m sestamibi scintimammography appears to be a useful diagnostic method for the detection of breast cancer in patients with non-diagnostic mammograms because of mammographically dense breasts.

Technetium-99m-tetrofosmin scintigraphy in the prediction of chemotherapy response of untreated malignant lymphomas and comparison with other prognostic factors.

The aim of this study was to retrospectively predict the chemotherapy response of untreated malignant lymphomas using technetium-99m tetrofosmin (Tc-TF) scintigraphy and to compare the technique with other prognostic factors. In this study, before any chemotherapy, 25 patients with malignant lymphomas (ML) underwent Tc-TF scintigraphy. Scintigraphy was performed 10 minutes after intravenous injection of 20 mCi Tc-TF to calculate the tumor-to-normal (T/N) uptake ratio. In the first 1-2 years after complete chemotherapy, the response was evaluated by clinical and radiological methods. Compared with the 10 ML patients with poor chemotherapy response, the 15 ML patients with good response had a significantly higher mean T/N uptake ratio (3.23 +/- 0.56) versus 1.18 +/- 0.11). All of the 15 patients with good response had positive scintigraphic results whilst the 10 patients with poor response had negative scintigraphic results. When compared with other prognostic factors, Tc-TF scintigraphy was the tool to predict the chemotherapy response of ML patients.

Detection of cervical lymph node metastases in nasopharyngeal carcinomas: comparison between technetium-99m methoxyisobutylisonitrile single photon emission computed tomography and computed tomography.

The aim of this study was to compare the effectiveness of technetium-99m methoxyisobutylisonitrile (Tc-99m MIBI) single photon emission computed tomography (SPECT) and computed tomography (CT) of head and neck in evaluating cervical lymph node (LN) metastasis in nasopharyngeal carcinomas (NPC). Fifty NPC patients with suspected cervical LN metastases underwent Tc-99m MIBI SPECT and CT of the head and neck to evaluate cervical LN metastases. Among the 50 patient.s cervical LN lesion were confirmed by biopsy histopathological results in 40 patients. For 22 LN lesions with discordant results between Tc-99m MIBI SPECT and CT Tc-99m MIBI SPECT could correctly detect 5 metastatic and 7 benign LN lesions, while could correctly detect 5 metastatic and 6 benign LN lesions. Tc-99m MIBI SPECT and CT could correctly detect all of the 18 metastatic LN lesions. Tc-99m MIBI SPECT showed a better specificity but a lower sensitivity for detecting cervical LN metastases in NPC when compared with CT. The combined use of Tc-99m MIBI SPECT and CT could increase the accuracy, compared with the single use of either Tc-99m MIBI SPECT or CT, to detect cervical LN metastases in NPC.

Using radionuclide esophageal emptying test to evaluate pneumatic dilatation effects for achalasia.

BACKGROUND/AIMS: Radionuclide esophageal emptying test using a solid meal is a simple, noninvasive, and physiologic test of esophageal function. METHODOLOGY: In this study, we evaluated esophageal emptying using an isotope-labeled solid meal (an egg salad sandwich labeled with 99mTc-MAA) in the assessment of pneumatic dilatation treatment in achalasia. Twenty achalasic patients (12 males, 8 females, age: 51.4 +/- 13.0 years) underwent esophageal emptying measurement before and after pneumatic dilatation. After treatment, the dysphagic symptoms of all patients improved. RESULTS: In comparison with the pretreatment test, the posttreatment retention fraction at 1, 5, 10, and 15 min were significantly lower (P < 0.001). CONCLUSIONS: The results suggest that radionuclide esophageal emptying test using a solid meal remains a useful objective study of esophageal function and may have an important future role in the follow-up evaluation of treatment for achalasia.

Value of whole body 18F-fluoro-2-deoxyglucose positron emission tomography in the evaluation of recurrent cervical cancer.

To evaluate the clinical value of whole body positron emission tomography (PET) with 18F-fluoro-2-deoxyglucose (FDG) in recurrent cervical cancer, the records of 20 patients who underwent FDG-PET scans were reviewed to detect local recurrence, pelvic/para-aortic lymph node metastases and distal metastases. The final diagnosis was based on operative, histopathological findings or clinical follow-up for longer than one year. FDG-PET accurately detected 18 patients with recurrent diseases (12 patients with local recurrences, 16 patients with pelvic lymph node metastases, 14 patients with para-aortic lymph node metastases and 4 patients with distal metastases of other sites). However, 2 patients with local recurrences had false- negative FDG-PET results as well as 1 patient without local recurrence and 1 patient without pelvic lymph node metastases who had false-positive FDG-PET results. The overall sensitivity and specificity of FDG-PET for patients were 90% and 100%, for local recurrence were 86% and 92%, for pelvic lymph node metastases were 100% and 94%, for para-aortic lymph node metastases were 100% and 100% and for distal metastases were 100% and 100%, respectively. In conclusion, whole body FDG-PET is a useful diagnostic tool in the evaluation of recurrent cervical cancer. It appears to be promising for detecting recurrent cervical cancer, lymph node metastases and distal metastases.

The use of dual phase 201Tl thyroid scan for equivocal fine-needle aspiration results in cold thyroid nodules.

We assessed the usefulness of dual phase 201Tl thyroid scan for equivocal fine-needle aspiration (FNA) biopsy results to differentiate malignant from benign lesions. The findings of the dual phase 201TI thyroid scan were compared with those of surgical histopathology for 27 patients with cold thyroid nodules. The FNA results were assessed and classified by experienced pathologists equivocal ENS results. Dual phase thyroid scan, including an early image and a delayed image, were acquired 10 minutes and 3 hours, respectively after mCi (74 MBq) of 201TI was injected intravenously. Dual phase 201Tl thyroid scan findings were visually interpreted as positive or negative results. Based on the final diagnoses of the surgical histopathology results, the dual phase Tl thyroid scan showed a sensitivity, specificity, and accuracy of 100%, 90%, and 96%, respectively. We concluded that dual phase Tl thyroid scan was very helpful in differentiating between benign and malignant thyroid nodules when FNA results were equivocal.

Whole body positron emission tomography with 18F-fluoro-2-deoxyglucose for the detection of recurrent ovarian cancer.

Although many cancers can be detected by whole-body positron emission tomography (PET) with 18F-fluoro-2-deoxyglucose (FDG), there has been limited clinical experience with FDG-PET for the detection of recurrent ovarian cancers. Therefore, the aim of this study was to evaluate the clinical value of FDG-PET in the detection of recurrent ovarian cancer. Whole body FDG-PET scans were performed on 24 women who had previous histories of ovarian cancer and treatment with surgery and chemotherapy. All patients also underwent physical examination, laboratory testing of serum CA-125 level and pelvic-abdominal-chest computed tomography (CT) or magnetic resonance imaging (MRI). The results of FDG-PET scans were correlated with serum CA-125 level, CT/MRI and operative pathology results. The diagnostic sensitivity was 90.9%, 90.9% and 90.9%, specificity was 92.3%, 76.9% and 46.2% and accuracy was 91.7%, 83.3% and 66.7% for FDG-PET, serum tumor marker of CA-125 level and CT/MRI in detecting recurrent ovarian cancer, respectively. FDG-PET is a useful diagnostic tool in detecting recurrent ovarian cancers with high specificity as compared with the serum tumor marker CA-125 level and the conventional CT/MRI morphological imaging methods.

Comparison of technetium-99m tetrofosmin and gallium-67 citrate scintigraphy for detecting malignant lymphoma.

The aim of this study was to compare the value of technetium-99m tetrofosmin (Tc-TF) scintigraphy with that of gallium-67 citrate (Ga-67) scintigraphy for detecting malignant lymphoma. In this study, 50 patients with malignant lymphoma underwent Tc-TF and Ga-67 scintigraphy before receiving any therapy. Tc-TF scintigraphy detected malignant lymphoma in 44 (88%) patients, but was false-negative in 4 cases of infradiaphragmatic malignant lymphoma and in 2 cases of malignant lymphoma with chemotherapy resistance. Ga-67 scintigraphy detected malignant lymphoma in 45 (90%) patients, but was false-negative in 3 cases of low-grade non-Hodgkin's lymphoma and in 2 cases of malignant lymphoma with bone marrow involvement. There was no significant difference in sensitivity between Tc-TF and Ga scintigraphy. However, a combination of Tc-TF and Ga-67 scintigraphy detected malignant lymphoma in all 50 patients (100%). We conclude that it is necessary to combine Tc-TF and Ga-67 scintigraphy to accurately detect malignant lymphoma.

Detection of esophageal carcinoma using single-photon emission computed tomography of thallium-201: a preliminary report.

The aim of this preliminary study was to evaluate the potential usefulness of thallium-201 (Tl-201) in detecting esophageal carcinoma. A total of 15 patients with esophageal carcinoma were arranged for Tl-201 single-photon emission computed tomography (SPECT) of the chest. Thirteen of the patients had epidermoid carcinoma and two had adenocarcinoma. Meanwhile, 7 normal controls without any history of esophageal disease also accepted Tl-201 SPECT of the chest for comparison. From the 15 patients, Tl-201 chest SPECT detected esophageal carcinoma in 13 (86.7%) but not in 2 (13.3%) patients with epidermoid carcinoma. In contrast, all 7 normal controls (100.0%) had negative results of Tl-201 chest SPECT. Our study showed that the Tl-201 chest SPECT scan could be considered as a non-invasive useful imaging method in detection of esophageal carcinoma.

Development of the Chinese nutrition screen (CNS) for use in institutional settings.

OBJECTIVES: To develop and validate a nutritional screening tool (CNS) for elderly Chinese subjects in the institutional setting. DESIGN: Using the MNA as a guide, a questionnaire was developed appropriate for the Chinese health care system, diet, food customs and culture, using physician assessment for comparison. PARTICIPANTS: 200 men and 200 women aged 65 years or older, approximately equally distributed by age between two cities in China (Hong Kong and Shanghai), were recruited from hospitals and old age homes for the reliability study. 340 men and 527 women were recruited for the validity study. RESULTS: The CNS compared with physician assessments based on two groups, normal or at risk with less than normal nutritional status, had kappa coefficients of 0.5 overall and were as high as 0.8 in Shanghai. CNS was able to identify about 90% of all persons with normal nutritional status and had about a 60% chance of correctly identifying a person at risk with a less than normal nutritional status. CONCLUSION: The CNS can be used in a 2-group classification to identify those who have a normal nutritional status (CNS > or = 21). Those who do not fall into this group should have their nutritional status evaluated in greater detail (CNS score < or = 20). The applicability of screening tools may vary depending on the site and the population characteristics.
