[The optimization of bone repair in open comminuted fractures of the long bones].

Clinical and experimental researches were conducted to study the possibilities for enhancing the efficiency of treatment of open composite comminuted and crushed fractures of long bones with the help of repercussive action of electrets. The results of roentgenomorphometric, clinical and biomechanical researches have shown the efficiency of electrets that could shorten the healing time of the infected wounds, stop the inflammatory reaction and promote the repairing process of bones. The application of electrets using the repercussive methods in complex treatment of complicated open fractures in cases with multiorgan and concomitant injuries creates favourable conditions for the healing process. This method is simple, atraumatic and economical.

[A systemic approach to the surgical treatment of degenerative-dystrophic diseases of the hip joint].

Based on the experience gained with the surgical treatment of over 900 patients with different forms of degenerative dystrophic diseases of the hip joint the current principles of the choice of different types of surgical interventions were defined. As far as the patients with aseptic necrosis of the head of the femur are concerned, the main problem in this case lies in the early diagnosis of osteonecrosis. The surgical treatment is specified by the leading factor of the pathogenesis at every stage of the development of the process (vascular disorders, changes in the biomechanism of the joint). The planning of operations in patients with dysplastic coxarthrosis rests on the consideration of 8 basic factors that influence the disease by means of factorial analysis and determination of the prognostic estimations of the treatment efficacy.

Femur Head Necrosis

[The use of electromyostimulation for preventing and treating hypodynamic disorders in gunshot injuries to the extremities].

It was found out during the study of multichannel electromyostimulation (EMS) influence upon human organism and the adequacy of its physical load to adoptive and compensative possibilities of cardiovascular system that that method was effective for prophylaxis of disorders and rehabilitation of nervous and muscular system, and locomotor apparatus after a short period of hypodynamia. The EMS increases the working capacity of nervous and muscular apparatus, mass of muscles and their contraction strength due to improvement of nervous and muscular conductivity and blood supply of the extremity. The efficiency of EMS decreases considerably when degenerative changes are taking place.

[Supratrochanteric osteotomy of the hip in adults with dysplastic coxarthrosis].

On the basis of complex assessment of the results of supratrochanteric osteotomy of pelvis (30 operations) and osteotomy of pelvis with the subsequent intertrochanteric osteotomy (24 observations) in patients with dysplastic coxarthrosis at mean terms of observation, equalling to 90 months, have been determined efficiency and indications for these operations. Indications for pelvic osteotomy are formed on the basis of roentgeno-anatomic interrelations in hip joint, where the decisive part is played by the angle of vertical inclination of the cotyloid cavity and degree of coating of the head of the femur. Intertrochanteric osteotomy following the pelvic osteotomy is indicated at preserved pain syndrome, resulting from coxarthrosis, subsequent to the muscular system restoration.

[The general and local treatment of wounded patients with gunshot fractures at stages in the medical evacuation].

The article discusses the pathogenetical approach to an early prophylaxis of local disorders of blood circulation in the cases of gunshot fractures of limbs. The authors emphasize the negative influence of the post-traumatic edema of muscles inside osseous-fascial bursas. On the background of blood losses and centralization of blood circulation edema causes the decompensation ischemia of tissue, which leads to the appearance of centres of secondary necrosis. An early recovery of optimal tissue blood circulation is propounded as one of the goals during the primary surgical management of wounds. A rational sequence of immediate surgery, which was practically tested is proposed. Basic methods of operations on limb segments are enclosed.

Combined Modality Therapy

[Internal osteosynthesis in pseudarthrosis without inter-fracture tissue dissection].

UNLABELLED: In the paper is presented the experience of operative treatment of 149 patients with false joints of bones, which were subjected to internal osteosynthesis with preservation of inter-fracture tissues. The operation was carried out in the absence of real danger of suppuration. The most frequently operated were the patients with tibia pseudarthrosis (72.5%), fixed by means of a plate. Adhesion has been achieved with 96.7% of patients. COMPLICATIONS: osteomyelitis (1), skin necrosis (2), fractured fragment dislocation under the action of contractor compression application (2). In the process of treatment of 439 patients by means of internal osteosynthesis with dissection of inter-fracture tissues, separation of bone fractured fragments and medullar cavity opening, adhesion has been achieved with 87.2% of patients.

[Principles of the treatment of patients with combined injuries and multiple fractures].

Based on an experience with 597 patients the authors have formulated main principles of treatment of poly-traumas. Nine groups of patients were established depending on the severity of polytrauma. In 42.6% of patients with polytrauma urgent surgical procedures were necessary. Operations on bones in the early period were justified but for saving the patient's life as a component of antishock measures. It should be performed rapidly, atraumatically and without blood loss. The application of apparatuses of external fixation on the basis of wires is thought to be expedient at this period.

Abdominal Injuries

[Theoretical and clinical bases of the method of osteosynthesis by screws in oblique and spiral fractures of the tibia].

The results of experiments with axial loading of human tibia preparations used for simulation of oblique (spiral) fractures and different methods of osteosynthesis by screws revealed that higher strength was ensured by the method of bone connection with the screws directed perpendicular to the segment axis (4.1 times more than in case of screws directed perpendicular to the plane of fracture). Explanation of the obtained facts necessitated development of more precise biomechanical model of deformation. Practical recommendations have been formulated.

Biomechanical Phenomena

[Results of using decompression, denervation and fenestration (DDF) in the treatment of patients with coxarthrosis of different etiologies].

The authors present their results of treatment of coxarthrosis of various etiology (dysplastic, idiopathic, posttraumatic and aseptic necrosis of the femoral head) by means of an operation of denervation, decompression and fenestration on 35 hip joints in 31 patients with the follow-up period up to 6 years. The proposed operation is especially effective at the early stages of development of coxarthrosis; in the patients with aseptic necrosis of the femoral head the prognosticated remission may reach 10 years. Low traumatism and short duration of the operation with the preservation of the anatomic unity of the femur and the possibility of early activization allows to recommend this method for the treatment of idiopathic coxarthrosis in the elderly people with associated diseases when conservative treatment is hopeless and an extension of surgical intervention is impossible.
