Reconstructive surgery of claw hand in leprosy.

32 hands in 31 known cases of leprosy were operated using the Paul-Brand technique for the intrinsic minus deformity. 29 hands revealed good results whereas remaining 3 also had definite improvement in their function and cosmesis. Bunnel's opponens plasty was performed in 7 cases to restore back the power of opponens in the thumb. Meticulous pre and postoperative management proved to be very necessary.

Arteriographic evaluation of vascular changes in leprosy.

Two hundred and eighty-four arteriograms were performed in the hands and feet 178 leprosy patients. Narrowing and tortuosity of the digital vessels, especially in the presence of bone absorption, were constant features. Digital tufts revealed signs of ischemia. However, in the presence of infection, hyperemia was a definite observation. Diminution in the caliber of the digital arteries and thinning or absence of the vascular end loops prove that ischemia plays a vital role in the production of various lesions in leprosy.

Muscle transfer for abduction paralysis of the shoulder in poliomyelitis.

Muscle transplantation in deltoid paralysis in poliomyelitis has been practiced only sporadically and considered ineffective. Trapezius transfer to humeral shaft with fascia lata extension was performed to restore abduction power of the shoulder in 24 patients between 4--10 years of age. It is recommended that the muscle re-education program and bracing be continued for a sufficiently long time so that the muscle may develop enough strength to function. If a forward subluxation of the joint develops and persists, a Nicola procedure is effective. The functional results obtained in a follow-up of 1--3 years has been encouraging in these children, especially when the accessory muscles were not paralyzed.

Complete rotation of the leg with associated deformities in poliomyelitis. A case report.

This is a report of a 24-year-old man with a heretofore unreported 180 degrees rotation deformity of the right leg on the thigh with the calf and heel in front and the patella and toes behind along with other associated deformities of both lower extremities in poliomyelitis. The right knee was flail and the popliteal pulsations could be felt anteriorly. The rotation deformity was successfully corrected by arthrodesis of the knee without any neurovascular damage. After correction of the other deformities, an erect posture was achieved by this patient.
