Survey of neuromuscular disorders in Lebanon. I. Profile of the genetically determined muscular dystrophies.

The clinical and laboratory profiles of 130 cases of muscular dystrophy in Lebanon collected over 35 years (1947-1982) are presented. Data collection included hospital records; registries in the department of pathology, hospital laboratories, electron microscopy facility and electromyography laboratory; and records from a private hospital neurosurgical unit. The yield from each of these sources is presented. The data was fed into an Apple II computer and analyzed using thirty seven parameters. The results are discussed and compared with three other similar studies from India and America.

Neurogenic pulmonary edema and missile emboli.

A case of missile emboli to a major intracranial vessel resulting in fulminant pulmonary edema is reported. A discussion of the etiology of neurogenic pulmonary edema (NPE) is presented. The cause is thought to be a marked increase in pulmonary artery pressure, due to a massive catechol release in the CNS, and possibly secondary to post-capillary sphincteric constriction. Missile emboli to the cerebral arteries should be removed in order to avoid distal embolization and infarction. The case reported appears to represent the first documented case of missile emboli causing NPE.

Zollinger-Ellison syndrome associated with a renal gastrinoma in a child.

Renal gastrinoma has not been previously reported. A 12-year-old boy with Zollinger-Ellison syndrome was found to have a renal tumor. No other tumor was detectable by imaging techniques, and selective venous sampling for gastrin showed a significant renal vein to vena cava gradient. Nephrectomy was performed, and examination of the tumor showed typical histologic features of an endocrine tumor. G cells were apparent by electron microscopy, and immunoperoxidase staining for gastrin, neuron-specific enolase, and chromogranin were positive. The gastrin content was unusually low for gastrinomas: 128 pg/g. Following nephrectomy, fasting gastrin and secretin stimulation testing were normal. Basal acidity was reduced by 60% but remained elevated at 39 mmol H +/h (hydrogen ion per hour). We speculate that renal gastrinoma may be characterized by uniquely poor gastrin storage and that curative resection of all gastrinoma tissue may not necessarily be associated with immediate complete suppression of hyperacidity.

The effect of aliphatic alcohols on certain vitamins of the B-complex group in the liver of the rat.

The effect of the oral administration of a series of aliphatic alcohols (methanol to butanol) in addition to isopropanol on the liver contents of thiamine, riboflavin, pyridoxine, niacin, and pantothenic acid was investigated in rats. It was found the tested alcohols caused a significant decrease in the liver contents of the studied vitamins. This effect was observed after daily oral administration of the tested alcohols in doses of 1 and 2 mg/kg for 7 days. The diminishing effect of alcohols on the liver contents of vitamins was directly proportional to the dose administered.
