Secondary extradural spinal tumours in children.

Secondary extradural spinal tumours are very rare, but they comprise most of the extradural neoplasms seen in infancy and childhood. Thirty-nine patients with this type of secondary tumour were treated in our hospital between 1965 and 1991. The diagnosis was proved by biopsy in every case. Following surgical decompression, all 35 survivors had radiotherapy and/or chemotherapy. Their outcome is reported.

Shear strength of composite bonded porcelain-to-porcelain in a new repair system.

This study assesses the repair procedure for porcelain fracture and involves fitting and bonding a porcelain material over the original damaged crown. To simulate the situation, a test sample was designed to simulate composite bonded porcelain-to-porcelain. A total of 120 samples was prepared and the shear bond strengths were tested. Three composite resins, two porcelain combinations, and two storage-time intervals were used. Significant differences were found among the three types of composite resins. Porcelain combinations did not affect the results. Furthermore, the mean bond strength was higher at 45-day storage than at 90 days.

Optic nerve glioma presenting as a huge intrasellar mass. Case report.

We report the case of a twelve-year-old boy who presented with visual impairment and headache and was found to harbour a purely intrasellar cystic pilocytic astrocytoma originating from proximal left optic nerve. The mass was explored transcranially and decompression of the optic apparatus and subtotal resection of the cyst wall was accomplished. The patient received post-operative radiotherapy and stays symptom-free after two years. A review of the literature revealed six cases of optic pathway astrocytomas associated with cyst formation. Our case appears as the first case of an optic glioma to present as an intrasellar mass.

Adult medulloblastoma presenting as a large neck tumor. Case report.

Extension of neuroepithelial tumors outside the neuroaxis is uncommon and usually follows surgical intervention. We report a patient with medulloblastoma of the right cerebellar hemisphere with extracranial extension presenting as a large neck tumor and discuss the clinical and pathological features of this case.

Primary intraventricular oligodendroglioma.

Oligodendroglioma, usually found in the cerebral hemispheres may invade the ventricles, however primary intraventricular oligodendroglioma is quite uncommon with only a limited number of cases reported in the literature. A 39-year-old woman harbouring a primary oligodendroglioma of the left lateral ventricle was treated by subtotal removal of the tumour through a left frontal transcortical transventricular approach. The clinical, radiological and surgical details of the case are presented in correlation with the current data on oligodendrogliomas. Benefits of postoperative irradiation are stressed.

Unusual location of histiocytosis X. Presenting as a petrous apex syndrome.

The authors describe a case of histiocytosis X of the petrous bone presenting as petrous apex syndrome. The clinical, radiological, and histopathological features are discussed, and the pertinent literature is reviewed. To our knowledge, histiocytosis X of the petrous apex has previously been reported to cause this syndrome in only two cases.

Spinal meningiomas.

Between the years of 1970-1982 29 spinal meningioma cases were operated on; 22 cases (76%) were female and 7 (24%) were male. The youngest patient was 16, and the oldest was 68 years old. On neurological examination, patients had neurological deficits in varying degrees. Diagnosis were done by myelography in all cases. Laminectomy was performed in all cases. 34.4% of cases were situated in cervical region and 65.6% were in thoracic region. 14% of the tumours were located extradurally and 68% were intradural. Multiple spinal meningioma was seen in only one case. One case died after operation; the mortality rate is 4%. 22 cases came for follow-up examination, 86.5% of them were in excellent or good condition. Only three cases showed no improvement.

Indications for hypophysectomy in diabetic retinopathy and cancer of the breast and prostate.

Hypophysectomy can be advised for diabetic patients whose vision is threatened by neovascular changes characterizing proliferative retinopathy. In the management of advancing breast cancer, hypophysectomy is recommended in postmenopausal (natural or induced) patients who have received two trials of hormonal therapy. We advocate wider application of hypophysectomy for advancing prostate cancer, where, when used in sequence after castration and administration of hormonal therapy, it will relieve pain and produce tumor regression in some patients unresponsive to other forms of treatment. Transsphenoidal cryohypophysectomy is a safe and effective means of pituitary ablation.

Placement of mechanically detachable spiral coils in the endovascular treatment of intracranial aneurysms. Work in progress.

PURPOSE: To determine the usefulness of mechanically detachable spiral tungsten coils (MDSs) in the endovascular, endosaccular occlusion of intracranial aneurysms. MATERIALS AND METHODS: Anterior communicating artery aneurysms shown at angiography in two patients and a basilar tip aneurysm shown in one patient were treated with MDSs. RESULTS: In the basilar artery aneurysm, eight coils were delivered. Two additional coils were placed at 3 months because of filling of the residual aneurysm neck. Angiography at 1 year showed no recanalization. The smaller aneurysm in the anterior communicating artery was totally occluded by a single coil. Angiography at 6 months showed no recanalization. The other aneurysm was occluded by two coils, with a small amount of residual filling. A third coil was withdrawn before detachment. The patient had aspiration pneumonia and electrolyte imbalance, but he was in stable condition 3 weeks later and was discharged. CONCLUSION: The pliable, soft, retrievable MDS system provides instantaneous release of a spiral coil.
