[Evaluation of qualitative values of blood flow indices in uterine and arcuate arteries during the third trimester of normal pregnancy].

We presented in our study obtained values of qualitative indices of blood flow in uteroplacental circulation in the third trimester of normal pregnancy. We performed examinations in 154 pregnant women with pregnancy age of 32-42 wk. We evaluated blood flow velocity in uterine and arcuate arteries by means of qualitative analysis of blood flow waveforms. We proved that in the third trimester of normal pregnancy qualitative parameters of blood flow decrease slightly what may suggest that there is systematic placental perfusion increase.

[Evaluation of the influence of placental localization on qualitative estimation of waveforms of blood flow velocity in uterine and arcuate arteries in third trimester physiologic pregnancy].

We presented in our study methods and results of blood flow measurements with Doppler ultrasound in uterine and arcuate arteries in relation to localization of placenta in physiological pregnancy. We performed examinations in 154 pregnant women with pregnancy age of 32-42 wk. On the basis of qualitative analysis of blood flow waveforms we discovered the difference in the blood flow in utero-placental compartment in relation to placental localization and we proved that this change takes place in uterine as well an in arcuate arteries.

[Evaluation of the clinical value in doing complex analysis of blood flow velocity parameters of the umbilical artery for determining prognosis of fetal status].

Complex analysis of the clinical value of the doppler flow indices was undertaken. A/B, RI and PI were compared separately and in different combinations with fetal status and neonate's condition. Statistical analysis revealed that complex and detailed analysis of few quality indices is of greater clinical value and increases sensitivity, specificity and positive predictive value of the doppler technique in the sense of prognosis of fetal distress and neonate's condition.

[Hysteroscopic evaluation of the uterine tubal opening].

The hysteroscopic evaluation of uterine openings in 23 sterile women was described. The attention was paid to morphological features of obliterated uterine openings. The hysteroscopic examination of structural changes located in uterine openings seems to be a reliable method for evaluation of potency of oviducts. This procedure may help in choosing the proper way of therapy--microsurgery or IVF.

[Evaluation of valvular mechanisms in uterine openings using endoscopic methods].

Endoscopic examination was made in 20 sterile women. Particular attention was drawn to the mechanism of uterine openings of the oviductus, viewed by hysteroscopy. Clinical condition end the flux of contrast via abdominal part of oviductus evaluated using laparoscopic technique. The hysteroscopic picture of valvular mechanisms is reliable element for evaluation of potency of oviductus. Combined laparoscopy and hysteroscopy brings a lot of information concerning the structure and function of oviductus.

[Usefulness of transvaginal ultrasound and hysteroscopy in diagnosing endometrial hyperplasia and endometrial carcinoma].

Modern methods enabling evaluation of endometrium in all phases of the menstrual cycle were presented. Transvaginal ultrasound does not give characteristic pictures. The most frequently observed sonographic features in endometrial hyperplasia and endometrial carcinoma were compared. Most frequently, in 29% we observed the thickening of the endometrium. The enlargement of the uterine body was detected in 27%. The dominant feature in endometrial carcinoma was distortion or lack of medium-focus echo-90%. Different echogenicity was observed in 69% of all cases. Application of hysteroscopy enables us to visualize changed endometrium and also to sample focal lesions for histopathological examination. Endometrial carcinoma was detected in all analyzed cases with application of hysteroscopy and ultrasound. Pathological endometrial hyperplasia was diagnosed by ultrasound only in 44% and with application of hysteroscopy in 84% of all material.

[Uterine endoscopy. I. Role of hysteroscopy in gynecologic oncology].

The historical sketch of development of hysteroscopy is presented. At the present time instrumentation consists of the set of precision lenses system, with "cold light" source and the light conveying glass fibre. In order to obtain good visibility and ability to perform surgeries one uses various media for distention of the uterine walls. For diagnostic hysteroscopy the most commonly used medium is carbon dioxide. The most advantageous substances when performing intrauterine procedures are liquid media such as: HYSKON, MANNITOL and GLUCOSE SOLUTION. Indications for hysteroscopy are numerous and they divide into diagnostic and surgical. Hysteroscopy is a very useful procedure in diagnosis and therapy of sterility and diagnosis and therapy of hyperplastic changes of the endometrium. Introduction of resection of the endometrium to hysteroscopy allows us, in certain situations to limit the number of laparatomies. Complications after hysteroscopy do not differ from complications following other gynecological surgeries.

Microheterogeneity forms of alpha-fetoprotein present in amniotic fluid.

The microheterogeneity forms of human alpha-fetoprotein present in amniotic fluid were studied in affinity immunoelectrophoresis using two lectins: concanavalin A and Lens culinaris agglutinin. Ninety-seven samples of amniotic fluid collected throughout gestation were obtained from pregnancies in which there was no fetal malformation. Two samples of amniotic fluid from pregnancies complicated by anencephaly and one sample obtained from an empty gestational sac were also studied. The changes in relative concentrations of AFP microheterogeneity forms during gestation are described. It is suggested that concanavalin A is suitable for the demonstration of malformations of the central nervous system in the first and second trimester of pregnancy while in the third trimester it would be of no value. The results of this study suggest that Lens culinaris agglutinin might be suitable for this purpose in the third trimester of gestation, but this needs to be confirmed in a larger study. The optimal concentrations of both lectins were investigated. In addition, a simple method of rocket affinity electrophoresis is also described. The possible mechanisms responsible for the changes in relative concentrations of microheterogeneity forms of alpha-fetoprotein during gestation and in pathological conditions are discussed.

Using logistic regression analysis in preliminary differential diagnosis of adnexal masses.

The aim of our study was to generate predictive model, which would allow to estimate the influence of analyzed parameters on predictive accuracy of differential diagnosis of adnexal masses and to evaluate prospectively diagnostic efficacy of the statistic model in the new set of patients. A total of 686 women diagnosed and surgically treated in the Gynecological and Obstetrical Teaching Hospital of University of Medical Sciences in Poznan, Poland, were recruited into the study. Preoperative diagnostics included gynecological examination, ultrasonographic evaluation, tumor Doppler blood flow analysis, and serum levels of CA125 and TPS. In order to find the best combination of features and to calculate the individual probability of the malignancy, stepwise logistic regression analysis with quasi-Newton estimation was applied. The essential part of the best prognostic model, described by foregoing variables, is as follows: [z = -6.005 + 0.058 x age + 1.174 x septa + 1.317 x tumor localization + 1.185 x ascites + 2.28 x solid element + 2.429 x vessels localization -2.386 x PI + 0.084 x MEDV]. The highest sensitivity and specificity for the obtained model were 87.84% and 93.74%, respectively. Prognostic model, constructed with the use of logistic regression analysis, is characterized by higher sensitivity and specificity than individually applied diagnostic tests. Prospective evaluation of this model application in a larger group of patients with adnexal masses will enable precise assessment of its objective clinical usefulness.

Adnexal Diseases↗

Comparison of diagnostic usefulness of predictive models in preliminary differentiation of adnexal masses.

The purpose of this study was to compare prognostic models evaluating the probability of an ovarian cancer occurrence based on a number of clinical and ultrasonographic data in women with adnexal masses. A total of 686 women with adnexal masses underwent the examinations between 1994 and 2002. The recorded parameters included: age, menopausal status, body mass index, the grayscale and Doppler ultrasonographic examination, and selected markers concentration levels. In order to find the best combination of features, which significantly influences the probability of malignancy, stepwise logistic regression analysis, as well as artificial neural network, was used. The diagnostic efficiency of received models was estimated and compared using receiver-operating characteristics (ROC) curve. The results indicate that 431 and 255 patients had a benign and malignant ovarian tumor, respectively. Application of stepwise logistic regression analysis revealed statistically significant importance of eight features. The sensitivity and specificity for the received model were 65.71% and 77.59%, respectively. Three-layer perceptron network shows 13 features as significant predictors of malignancy. The network gave a sensitivity of 85.7% and specificity of 93.1%. Comparison of area under ROC curve for received models was 0.9679 vs 0.9716. Prognostic values of the analyzed neural model are not optimal but seem to surpass logistic regression model in terms of the predictive possibilities.
