Esophageal scintigraphy: a comparison with esophagoscopy.

Fifty consecutive patients with different esophageal symptoms were investigated with esophageal endoscopy, transit scintigraphy, and gastroesophageal (GE) scintigraphy with extra-abdominal compression. Scintigraphic findings were abnormal in 27 of those 31 patients (87%) who were classified as abnormal at endoscopy. A prolonged transit time was the commonest finding, but hiatal hernia and GE reflux were also found. However, the scintigraphic procedure showed abnormalities in 6 of 19 (31%) who were classified as normal at endoscopy. Esophageal scintigraphy is recommended as a screening test before endoscopy is decided on.

Transluminal balloon dilation of ureteral stricture in a child. A case report.

A 12-year-old girl complained of left flank pain 4 months after the onset of symptoms of polyarthritis nodosa with hypertension, multiple neuritis and impaired renal function. Intravenous pyelography (IVP) revealed no stone. Several months later the flank pain recurred, and IVP now showed a calcified stone midway in the left ureter. Ureterolithotomy was performed, and a 3 cm long stone embedded in the mucosa was removed. Two months postoperatively IVP showed severe ureteric obstruction due to stenosis at the level of the incision. Dilation of the stricture was accomplished with a 4 mm balloon, and further dilation 2 weeks later with a 6 mm balloon. A third dilation was performed with a 4 mm balloon. At examination 3 months later the girl was free from symptoms and IVP with forced diuresis was normal, as were the findings after 6 more months. Transluminal balloon dilation of postoperative ureteral strictures in children should be considered as an alternative to surgery.

Slipped capital femoral epiphysis in southern Sweden. Long-term result with closed reduction and hip plaster spica.

Seventy-two patients treated with closed reduction and hip spica for slipped capital femoral epiphysis (physiolysis colli femoris; PCF) were evaluated by questionnaire 25 to 66 years after primary treatment. Fifty-seven cases were examined with the use of clinical and radiographic methods. Pain and restricted hip function were obvious in most patients. In 25 of 72, an operation had been done in adult life for coxarthrosis. In a previously investigated group of PCF patients not actively treated, only two of 49 had had surgical treatment of coxarthrosis. Thus the results of closed reduction and hip spica are unfavorable, compared with those of nontreated cases. The results suggest a harmful influence of both reduction and hip spica.

Slipped capital femoral epiphysis in southern Sweden. Long-term results after nailing/pinning.

One hundred seventy-two patients with 204 slipped capital femoral epiphyses treated with nailing or pinning were evaluated an average of 28 years after surgery. Follow-up evaluation was obtained by questionnaire in 153 cases (181 hips) and by clinical and radiographic reexamination in 132 cases (157 hips). Segmental collapse was seen in four of 179 hips nailed/pinned in situ and in four of 25 hips operated after reduction. This was the only early complication associated with problems at reexamination. Arthrosis was twice as frequent after reduction (53%) than after fixation in situ (24%). The clinical and radiographic results were better than those from the same region after other methods of treatment. Nailing or pinning in situ is the method of choice when possible, regardless of the degree of slipping. Bilateral slipping was found in 67% of the hips; therefore, prophylactic pinning of the contralateral hip is indicated in cases with unilateral slipping.

Familial slipped capital femoral epiphysis.

In 50 consecutive cases of slipped capital femoral epiphysis (SCFE) from 49 families, the heredity was analyzed by radiographic examination of the first-degree relatives and by interview regarding the second-degree relatives. In four of the 49 families, SCFE was obvious in one or more first-degree relatives; and in another 13 families (14 relatives), radiographic signs of SCFE were found besides the primary case. The familial accumulation was much higher than expected from incidence studies, indicating a hereditary factor in the etiology.

Slipped capital femoral epiphysis in southern Sweden. Long-term results after femoral neck osteotomy.

Opinions differ concerning the treatment of choice for severe slipped capital femoral epiphysis, probably due to the lack of long-term follow-up evaluations on the different methods of treatment. A series of 33 patients with severe slipped capital femoral epiphysis, treated per primam with wedge osteotomy of the femoral neck, were radiographically and clinically reexamined an average of 28 years (range, 16-32 years) after the operation. Segmental collapse and/or chondrolysis developed in ten patients. Nine of these patients were available for reexamination and all had severe arthrosis with poor function. Arthrosis developed in nine of the 19 patients without signs of segmental collapse or chondrolysis; these patients had a satisfactory joint function. This series was compared with another series (from the same orthopedic departments) of patients with severe slip without any primary treatment. The long-term results in these two groups were similar; consequently, the value of realignment by wedge osteotomy of the femoral neck is questionable.

The urine smell around patients with urinary incontinence. The production of ammonia in ordinary diapers and in diapers impregnated with copper acetate.

The foul smell of urine around patients with urine incontinence is thought to be due mainly to the production of ammonia from urea by bacterial ureases. The production of ammonia was thus measured by infrared spectrophotometry in control diapers and in test diapers impregnated with copper acetate (CA), after use by urine-incontinent patients with persistent bacteriuria. The CA-impregnated diapers produced significantly less ammonia than the control diapers. The CA-impregnated diapers were not bactericidal, however. The results are consistent with the hypothesis that the smell reduction of the CA-impregnated diapers is due to a bacteriostatic action or an inhibition of bacterial ureases by CA.

Slipped capital femoral epiphysis in southern Sweden. Long-term result with no treatment or symptomatic primary treatment.

Forty-nine cases of slipped capital femoral epiphysis or physiolysis colli femoris (PCF) without primary treatment were evaluated 20-60 years after diagnosis by questionnaire and in 44 cases by clinical and radiographic reexamination. The cases represent PCF cases without primary treatment from three different orthopedic centers from 1910 to 1960. Only a few had restrictions in working capacity or social life. Two of these 49 cases have required surgery because of secondary arthrosis, far fewer than were found in a comparable group of cases treated with closed reduction and hip spica.

Multifocal sclerotic BCG spondylitis in a 13-year-old girl.

A case is reported of multifocal BCG osteomyelitis in a 13-year-old girl. The lesions in the skull, ribs and in several vertebrae were mainly sclerotic and healed with antituberculotic therapy. The case is unusual because of the late onset, sclerotic changes, and involvement of the spine.

Emergency pacing and subsequent permanent pacemaker implantation in a premature infant of 1770 g with a follow-up of 6 years.

We report successful emergency pacing followed by permanent pacemaker implantation due to complete block in an otherwise healthy premature infant of 1,770 g. Via the umbilical vein a temporary bipolar pacing lead was placed in the right ventricle. The lack of spontaneous improvement warranted implantation of a permanent pacemaker system at the age of 2 weeks. Via a transatrial approach an endocardial unipolar screw-in lead was placed in the right ventricle and connected to a pulse generator implanted subcutaneously. During the follow-up period of 6 years the child has been doing well with his VVI-R pacemaker operating at rates between 80 and 160 pulses/minute at a chronic stimulation threshold below 0.9 V at 0.37 msec.

Computed tomography in the diagnosis and treatment of mediastinal abnormalities in children.

Thirty-one children with mediastinal abnormalities--14 malignant lymphomas, 4 other primary malignancies, one metastatic and 12 benign lesions--were examined one or several times using CT, which proved to be effective especially for cysts (5 patients), ductus arteriosus aneurysm (2 patients), and intrathoracic liver (one patient). It also supplied important diagnostic information regarding the extent of disease in malignant thymoma (one patient), in neurinoma (one patient), and in Hodgkin's lymphoma (5 patients). It was found to be useful in the monitoring of treatment of patients with lymphomas, in which a small residue, probably a fibrotic remnant, was invariably seen after completion of chemotherapy and irradiation. It was concluded that when the residue was enlarged, the possibility of relapse and even thymic hyperplasia should be considered. However, if CT was performed under general anaesthesia pseudo-widening of the anterior mediastinum could simulate recurrence. Surgical biopsy was found to be necessary in these cases because fine-needle aspiration biopsy was unsuccessful.
