Influence of nicorandil on the pharmacodynamics and pharmacokinetics of gliclazide in rats and rabbits.

Chronic diabetes precipitates ischaemic heart disease (IHD) and many other disorders. IHD inturn is shown in the form of angina initially. According to EUROPA study, the incidence of angina is high in type II diabetics. Gliclazide, a second generation sulphonylurea derivative is widely used in the treatment of type-II diabetes and is known to release insulin by K(+) channel inhibition. Nicorandil, a newer antianginal drug widely used now a days acts by opening potassium channels in the cardiac muscle cell and also by releasing nitric oxide. However its action on pancreatic cell K(+) channel is not known. Since there is possibility for drug interaction leading to decreased activity of gliclazide the present study was conducted to evaluate the effect of the combination. Studies in normal and alloxan induced diabetic rats were conducted with oral doses of 2 mg/kg bd. wt. of gliclazide, 1.8 mg/kg bd. wt. of nicorandil and their combination with adequate washout periods in between treatments. Studies in normal rabbits were conducted with 5.6 mg/1.5 kg bd. wt. of gliclazide, 1.4 mg/1.5 kg bd. wt. of nicorandil and their combination given orally. Blood samples were collected in rats from retro orbital puncture at 0, 1, 2, 3, 4, 6, 8, 10 and 12 h and by marginal ear vein puncture in rabbits at 0, 1, 2, 3, 4, 6, 8, 12, 16, 20 and 24 h. All the blood samples were analysed for glucose by GOD/POD method. The blood samples of rabbits were analysed by HPLC for gliclazide. Gliclazide produced hypoglycaemic/antidiabetic activity in normal and diabetic rats with peak activity at 1 h and 8 h and hypoglycaemic activity in normal rabbits at 3 h, while nicorandil alone produced significant hyperglycaemia at 4 h and reduced the effect of gliclazide with no significant change in pharmacokinetics when administered in combination. The interaction observed appears to be pharmacodynamic at the receptor level as expected.

Influence of selenium (antioxidant) on gliclazide induced hypoglycaemia/anti hyperglycaemia in normal/alloxan-induced diabetic rats.

Oxidative stress is involved in diabetes mellitus and its complications. Since diabetes is a stress-related disorder, supplementation with antioxidants may improve the condition. The purpose of this study is to know the effect of oral administration of selenium on blood glucose and its influence on gliclazide induced hypoglycaemia/antihyperglycaemia in normal and alloxan-induced diabetic rats. Albino rats of either sex were divided into three groups of six each. Group-I/II/III were treated with selenium 1/2 TD (0.9 microg/200 g rat)/TD (1.8 microg/200 g rat)/2TD (3.6 microg/200 g rat), respectively. Later group II was treated with gliclazide TD (1.44 mg/200 g rat)/selenium TD + gliclazide TD with a washout period of 1 week between the treatments. Diabetes was induced by alloxan monohydrate 100 mg/kg body weight i.p. A group of six rats showing fasting blood glucose levels ranging from 175-250 mg/dl were selected for the study. Rats were treated with selenium TD, gliclazide TD and selenium TD + gliclazide TD with a washout period of 1 week between the treatments. Selenium 1/2 TD and TD produced hypoglycaemia while 2TD produced hyperglycaemia. The combination of selenium TD with gliclazide TD, significantly enhanced the glucose lowering effect of gliclazide in normal and diabetic rats.

Efficacy of a novel calcium/potassium salt of (-)-hydroxycitric acid in weight control.

The weight-loss efficacy of a novel, water-soluble, calcium-potassium salt of (-)-hydroxycitric acid (HCA-SX) was re-examined in 90 obese subjects (BMI: 30-50.8 kg/m2). We combined data from two previously reported randomized, double-blind, placebo-controlled clinical studies in order to achieve a better statistical evaluation based on a larger population. This re-examination of data also allowed us to reflect more intensely on various aspects of weight loss studies. Subjects were randomly divided into three groups: group A received a daily dose of HCA-SX 4, 667 mg (providing 2,800 mg HCA per day); group B was given a daily dose of a combination of HCA-SX 4,667 mg, niacin-bound chromium (NBC) 4 mg (providing 400 microg elemental chromium), and Gymnema sylvestre extract (GSE) 400 mg (providing 100 mg gymnemic acid); and group C received a placebo in three equally divided doses 30-60 min before each meal. All subjects were provided a 2,000 kcal diet/day and participated in a supervised walking program for 30 min/day, 5 days/week. Eighty-two subjects completed the study. At the end of 8 weeks, in group A, both body weight and BMI decreased by 5.4%, low-density lipoprotein and triglycerides levels were reduced by 12.9% and 6.9%, respectively, while high-density lipoprotein levels increased by 8.9%, serum leptin levels decreased by 38%, serotonin levels increased by 44.5% and urinary excretion of fat metabolites increased by 32-109%. Group B demonstrated similar beneficial changes, but generally to a greater extent. No significant adverse effects were observed. The combined results confirm that HCA-SX and, to a greater degree, the combination of HCA-SX plus NBC and GSE reduce body weight and BMI, suppress appetite, improve blood lipid profiles, increase serum leptin and serotonin levels and increase fat oxidation more than placebo. We conclude that dosage levels, timing of administration, subject compliance and bioavailability of HCA-SX significantly affect results and that when taken as directed, HCA-SX is a highly effective adjunct to healthy weight control.

Effects of a natural extract of (-)-hydroxycitric acid (HCA-SX) and a combination of HCA-SX plus niacin-bound chromium and Gymnema sylvestre extract on weight loss.

AIM: The efficacy of optimal doses of highly bioavailable (-)-hydroxycitric acid (HCA-SX) alone and in combination with niacin-bound chromium (NBC) and a standardized Gymnema sylvestre extract (GSE) on weight loss in moderately obese subjects was evaluated by monitoring changes in body weight, body mass index (BMI), appetite, lipid profiles, serum leptin and excretion of urinary fat metabolites. HCA-SX has been shown to reduce appetite, inhibit fat synthesis and decrease body weight without stimulating the central nervous system. NBC has demonstrated its ability to maintain healthy insulin levels, while GSE has been shown to regulate weight loss and blood sugar levels. METHODS: A randomized, double-blind, placebo-controlled human study was conducted in Elluru, India for 8 weeks in 60 moderately obese subjects (ages 21-50, BMI >26 kg/m(2)). Subjects were randomly divided into three groups. Group A was administered HCA-SX 4667 mg, group B was administered a combination of HCA-SX 4667 mg, NBC 4 mg and GSE 400 mg, while group C was given placebo daily in three equally divided doses 30-60 min before meals. All subjects received a 2000 kcal diet/day and participated in supervised walking. RESULTS: At the end of 8 weeks, body weight and BMI decreased by 5-6% in both groups A and B. Food intake, total cholesterol, low-density lipoproteins, triglycerides and serum leptin levels were significantly reduced in both groups, while high-density lipoprotein levels and excretion of urinary fat metabolites increased in both groups. A marginal or non-significant effect was observed in all parameters in group C. CONCLUSION: The present study shows that optimal doses of HCA-SX and, to a greater degree, the combination of HCA-SX, NBC and GSE can serve as an effective and safe weight-loss formula that can facilitate a reduction in excess body weight and BMI, while promoting healthy blood lipid levels.

Antioxidant activity of the aqueous extracts of spicy food additives--evaluation and comparison with ascorbic acid in in-vitro systems.

The antioxidant activity of the aqueous extracts of five umbelliferous fruits--caraway (Carum carvi), coriander (Coriandrum sativum), cumin (Cuminum cyminum), dill (Anethum graveolens) and fennel (Foeniculum vulgare)--were investigated in comparison with the known antioxidant ascorbic acid in in vitro studies. The amount of aqueous extract of these five umbelliferous fruits and ascorbic acid needed for 50% scavenging of superoxide radicals was found to be 105 microg (caraway), 370 microg (coriander), 220 microg (cumin), 190 microg (dill), 205 microg (fennel) and 260 microg (ascorbic acid). The amount needed for 50% inhibition of lipid peroxide was 2100 microg (caraway), 4500 microg (coriander), 4300 microg (cumin), 3100 microg (dill), 4600 microg (fennel) and 5000 microg (ascorbic acid). The quantity needed for 50% inhibition of hydroxyl radicals was 1150 microg (caraway), 1250 microg (coriander), 470 microg (cumin), 575 microg (dill), 700 microg (fennel) and 4500 microg (ascorbic acid). The daily use of the above fruits in various forms is very common in India and the present study revealed strong antioxidant activity of their extracts that was superior to known antioxidant ascorbic acid and indicate their intake may be beneficial as food additives.

Antithrombin III assay using thrombin in disseminated intravascular coagulation (DIC), other thromboembolic disorders and hepatic diseases.

Fifty cases comprising 11 cases of disseminated intravascular coagulation (DIC), 16 cases of venous thromboses and 23 cases of hepatic diseases were studied for AT III levels using clotting assay. Twelve samples were subjected to ATIII estimation by the commercially available synthetic chromogenic assay. Twenty age and sex matched controls were also analysed to find out the reference value for the techniques. Low AT III levels, if present, were correlated with other markers of DIC, viz FDP and D-dimer assays. There was a decrease in the AT III levels in all the three disease categories with a significant difference between the AT III levels of the three disease categories. In DIC, lowest levels were observed which correlated well with FDP and D-dimer levels. There was no significant difference between the average AT III levels measured by both the clotting and synthetic chromogenic assay with the former procedure being relatively inexpensive.

Oropharyngeal carriage of Candida species in HIV-infected patients in India.

The present investigation represents the first study of oropharyngeal carriage of Candida and other yeasts in HIV-infected patients in India. One hundred and fifty HIV-positive patients were investigated by culturing their swish samples on plates of CHROMagar Candida. Ninety-eight patients (65.3%) were positive for Candida and four (2.7%) were positive for other yeasts. Among them, the first Indian C. dubliniensis isolate has been recovered. Molecular typing of selected C. albicans isolates by AP-PCR revealed two major genotypes based on the banding patterns. The susceptibilities of 30 Candida isolates to five antifungal agents including the new triazole voriconazole were determined in a micro-dilution test, according to the NCCLS protocol M 27. All the 22 C. albicans isolates were susceptible to five antimycotic agents (flucytosine, amphotericin B, fluconazole, voriconazole and itraconazole) except one isolate (VPCI-122), which was resistant to flucytosine (MIC > or = 64 mg l-1). The azole-resistant isolates reported here endorse the role of antifungal susceptibility testing whenever antifungal treatment with azoles is planned.

Atypical mycobcterial injection abscess.

Other than suppurative organisms, atypical mycobacteria are also known to cause injection abscesses following vaccinations, injections, tattooing and even after implants. Though the usage of disposable needles is practised universally, sporadic cases do occur. The disease entity should be considered, while dealing with injection abscesses, to institute specific therapy. Acid-fast bacilli should be looked for in the pus and mycobacterial culture of the material from injection abscesses should be done for a definitive diagnosis.

Needle necropsy in AIDS.

The value of autopsy in understanding the natural course of any disease is beyond any argument. The reluctance of pathologists to perform autopsy in HIV infected cadavers is justified due to the risks involved to the prosector and the morgue attendants. A relative low risk needle necropsy protocol is proposed using fine needle aspiration cytology, tru-cut biopsies and microbiological examination. Diagnosis could be offered in all the forty-four needle necropsies performed. Disseminated tuberculosis in 18/44 (40.9%) cases, disseminated cryptococcosis in 12/44 (27.2%) cases, poly-microbial infections in 27.2% cases and non-Hodgkin's lymphoma in 9% cases were detected in the study. Infectious agents like Histoplasma capsulatum, Isospora belli, tachyzoites of Toxoplasma gondii, Candida sp and Cryptococcus sp could be demonstrated in the samples obtained in the study. Lack of material for study of gross pathology, inaccessibility of deep-seated lesions and risk of needle stick injury to the prosector though low are the limitations of this procedure.

Determination of diamino acids in peptidoglycans from anaerobic bacteria.

Peptidoglycans isolated from two Fusobacterium species of anaerobic bacteria were analyzed for constituent amino acids. Hydrolysis conditions were varied to optimize the yield of diamino acids from peptidoglycan. The o-phthalaldehyde derivatives of the diamino acid stereoisomers were separated by high-performance liquid chromatography (OPA-HPLC), and variations in the relative areas of the two peaks noticed during analysis of solid samples were attributed to sampling errors. Co-derivatization/injection experiments using standards of the meso and rac forms separated from commercial mixtures demonstrated that meso-2,6-diaminopimelic acid and meso-lanthionine were peptidoglycan components in Fusobacterium varium and Fusobacterium nucleatum, respectively. The protonated molecules of 2,6-diaminopimelic acid and lanthionine were detected in peptidoglycan hydrolyzates by off-line, flow-injection electrospray mass spectrometry (ESI-MS). In ESI-MS-MS experiments under identical collision-induced dissociation (CID) conditions, peptidoglycan-derived and standard diamino acids exhibited similar fragmentations. Fragmentation pathways are proposed for each diamino acid. The results confirm that the meso forms of two different diamino acids are utilized in the peptidoglycans of Fusobacterium species.

Extreme lateral approach to ventral and ventrolaterally situated lesions of the lower brainstem and upper cervical cord.

Lesions situated ventrally and ventrolaterally to the lower brainstem and upper cervical spinal cord test the skills of neurosurgeons. We present our experience with eight such patients who underwent the extreme lateral craniocervical approach. The pathologies encountered include three distal vertebral aneurysms, one prepontine epidermoid, one anterior foramen magnum meningioma, and three high cervical dumbbell neurofibromas. All lesions were treated effectively. Postoperatively, the patients improved significantly. Complications included transient lower cranial nerve paresis in three patients, meningitis in one patient, and a pseudomeningocele in two patients. All complications improved with therapy. We conclude that the extreme lateral approach offers excellent visualization and access with minimal neural retraction for treating these difficult lesions.

Ovarian granulosa cell 'tumorlet' and mature follicles with ectopic decidua in pregnancy--a case report.

Non-neoplastic, tumor like lesions are encountered in the ovaries during pregnancy. Of these pregnancy luteoma is the most common lesion mimicking an ovarian neoplasm. Ovarian granulosa cell proliferations are also reported in the ovaries removed during pregnancy as an incidental finding. The granulosa cell proliferation occurs due to the follicular stimulating hormone (FSH) like activity of human chorionic gonadotropin (hCG). We report a case of second gravida who developed uterine atonia, necessitating emergency hysterectomy. Right ovary showed extensive ectopic decidua, numerous well-formed follicles and granulosa cell tumor-like proliferation. Usually follicles are not formed in the ovaries during pregnancy. The same FSH like activity of hCG might have been responsible for the formation and maturation of follicles in this case.

Synovial cyst of lumbar spine presenting as disc disease: a case report and review of literature.

Synovial cysts most commonly involve the joints of the extremities. These cysts are rarely found in the spinal canal or the vertebral facet joints. However, if manifested as such, they can pose serious diagnostic and therapeutic problems due to the presentation, which most often resembles nerve root or spinal cord compression. Acute low back pain and radiculopathy are often attributed to a herniated nucleus pulposus. This paper presents a case of synovial cyst in a 62-year-old woman with a 2-year history of refractory low back pain with distal radiation. A facet joint cyst was encountered upon neuroimaging, resulting in excision of the cyst. In this report, we discuss the differential diagnosis of synovial cysts, the role of computed tomography and magnetic resonance imaging in the diagnosis, and treatment options for this uncommon entity.

Hypoglycaemic activity of extracts from soft corals of Andaman and Nicobar coasts in rats.

The ethylacetate extract of soft corals collected from Andaman and Nicobar Coasts were screened for hypoglycaemic activity in fasting rats. Rats were divided into 5 groups. Group I received 0.5 ml of 5% gum acacia suspension (control). Group II received the extract of Cladiella australis (CAS), at a dose of 250 mg/kg. Group III received the extract of Sinularia new species (SNS), at a dose of 75 mg/kg. Group IV received the extract of Lamnalia new species (LNS), at a dose of 400 mg/kg and Group V received the extract of 250MF-CBR-13 at a dose of 250 mg/kg. All extracts were administered orally. Blood samples, collected before the administration of test extracts and also at 2, 4, 6, and 8 hr after treatment, were analysed for glucose content. The percentage blood glucose reduction from that of control was also calculated. A very promising hypoglycaemic activity was observed in rats with CAS at 8 hr (42.3%), with SNS at 4 hr (28.34%) and 6 hr (40.6%), with LNS at 6 hr (32.38%) and with MF-CBR-13 at 6 hr (20.25%).

Studies of guar gum compression-coated 5-aminosalicylic acid tablets for colon-specific drug delivery.

The aim of this study was to develop colon-specific delivery systems for 5-aminosalicylic acid (5-ASA) using guar gum as a carrier. Core tablets containing 5-ASA were prepared by wet granulation with starch paste and were compression coated with coating formulations containing different quantities of guar gum (300, 200, 150, and 125 mg). In vitro drug release studies were carried out in simulated gastric and intestinal fluids and in pH 6.8 buffer containing rat cecal contents. The application of 175 mg of coating formulation containing 150 mg of guar gum over 5-ASA core tablets resulted in the release of less than 2% drug in simulated gastric and intestinal fluids and about 93% of 5-ASA in pH 6.8 buffer containing rat cecal contents. Differential scanning calorimetric (DSC) studies showed the absence of any interaction between 5-ASA and the excipients on storage at 45 degrees C for 12 weeks. The study confirmed that selective delivery of 5-ASA to the colon can be achieved using guar gum as a carrier in the form of a compression coating over the drug core.
