The impact of mandibular advancement on articulation, resonance and voice characteristics in Flemish speaking adults: a pilot study.

Bilateral saggital split osteotomy (BSSO) of the mandible is a frequently performed mandibular orthognathic procedure, used to resolve mandibular disharmonies. Literature review showed contradictory findings regarding the effect of the orthognathic surgery on speech characteristics. The purpose of the present study was to determine a detailed analysis of the articulation, resonance and voice characteristics after BSSO with mandibular advancement for the treatment of Class II malocclusions using objective and subjective assessment techniques (perceptual evaluations, Dysphonia Severity Index, nasalance scores) in eight subjects. The findings of the present study indicate that before and after BSSO with mandibular advancement three types of articulation disorders may predominate in the Flemish language: the incorrect production of the trill sound /r/ and the /s/ sound and devoicing of the /z/. After orthognathic surgery most patients showed an identical articulation pattern (normal or disturbed pattern) as in the presurgical condition. In this study the BSSO with mandibular advancement had no significant impact on the nasality characteristics and the nasalance values probably due to the competent velopharyngeal valving in the presurgical condition. And, as expected the vocal quality revealed no significant difference. The maxillofacial surgeon and the speech language pathologist must be aware of the persistency of these preoperative articulation errors in the postsurgical condition.

Image-based planning and clinical validation of zygoma and pterygoid implant placement in patients with severe bone atrophy using customized drill guides. Preliminary results from a prospective clinical follow-up study.

The zygoma implant has been designed for those situations where there is insufficient bone in the upper jaw, which would otherwise require onlay or inlay (sinus) bonegrafts. The aim of the study was to present and validate a planning system for implant insertion based on preoperative CT imaging. It allows the surgeon to determine the desired position of different kinds of implants. Finally a customized drill guide is produced by stereolithography. In this study, zygoma, pterygoid and regular platform implants were used. The treatment protocol is validated through 12 case studies, selected at random from the total patient group (n=29 patients). From postoperative images, the exact implant location is determined and the deviation of axes between planned and inserted implants is calculated. In this in vivo study, displacements, varying according to the type of implant and the location of the implants, were observed. From a clinical standpoint, most of the inserted implants were judged to be adequately sited. A prospective clinical follow-up study was performed on all 29 patients. Although all patients presented with severe maxillary atrophy, excellent cumulative survival rates (92%) for the zygoma implants and 93% for regular platform implants have been obtained.

Cryopreservation of teeth. Organizational aspects of a tissue bank for tooth tissues.

Cryopreservation of teeth became possible since a better understanding of the cryoprotective mechanism of cosolvents such as dimethylsulfoxide (DMSO). The high number of extractions of healthy teeth performed for orthodontic reasons, i.a., gives a sufficient amount of donor teeth. The organization and quality control of a tissue bank for teeth tissues is legally regulated. This paper highlights some of the key-issues of these stipulations.

Bioactive glass particles of narrow size range: a new material for the repair of bone defects.

A clinical trial to treat dental osseous lesions with bioactive glass granules of narrow size range (300 to 360 microns) has been conducted since early spring 1990. This study followed an animal study in which the osseous tissue repair properties of bioactive glass granules of narrow size range and hydroxyapatite granules were compared for up to 2 years of implantation. The therapeutic response to the bioactive glass material exceeded the response to the hydroxyapatite as evidenced by very extensive osteoconduction, as well as the capacity to cause differentiation of osteoprogenitor cells to osteoblasts. The clinical study was started by virtue of the bioactive glass granules of narrow size range eliciting expeditious bone tissue formation throughout a defect. In this clinical study 87 patients and 106 defects were treated. The indications selected were apical resection areas, cystic defects, extraction sites, and defects of the alveolar ridge due to surgery or resorption. After insertion, the particles remained well in place and only small changes in the contours of the restored defects were seen, at the most up to 2 months postinsertion. At 3 months the application sites had fully solidified. Radiographic analysis indicated that the material integrated into the bone tissue, and at 6 months any difference between glass particles and bone tissue had nearly disappeared. The few initial cases with limited clinical results were caused by factors unrelated to the glass granules, mostly the surgical technique. By adapting the surgical technique, no unfavorable clinical results were subsequently experienced.(ABSTRACT TRUNCATED AT 250 WORDS)

Surgical treatment of exophthalmos and exorbitism: a modified technique.

A modified surgical technique for the treatment of exophthalmos caused by Graves' disease or meningioma is presented. It is based on the orbital expansion as presented by Tessier (1969). It consisted of a lateral marginotomy with extension to the superior and inferior orbital rim and removal of the great wing of the sphenoid bone. The osteotomised orbital rim segment was advanced and stabilised by interposition of a calvarial bone graft. Concomitant lipectomy was carried out when indicated. Seven patients were operated on. No major complications occurred. The advantages of the method are discussed.

[Complications of osteotomies].

The authors wish to present a review of the literature on complications in orthognathic surgery. They are classified into non-specific, general complications and into complications which are specific, according to the region the osteotomy is performed in. Though the list of possible complications is very extensive, one should realise that they are often based on single case-reports and that orthognathic surgery in general should be considered as a group of safe technical, surgical procedures.
