Telemedicine and robotics: paving the way to the globalization of surgery.

The concept of delivering health services at a distance, or telemedicine is becoming an emerging tool for the field of surgery. For the surgical services, telepresence surgery through robotics is gradually being incorporated into health care practices. This article will provide a brief overview of the principles surrounding telemedicine and telepresence surgery as they specifically relate to robotics. Where limitations have been reached in laparoscopy, robotics has allowed further steps forward. The development of robotics in medicine has been a progression from passive to immersive technology. In gynecology, the utilization of robotics has evolved from the use of Aesop, a robotic arm for camera manipulation, to full robotic systems such as Zeus, and the daVinci surgical system. These systems have not only been used directly for a variety of procedures but have also become a useful tool for conferencing and the mentoring of surgeons from afar. As this mode of technology becomes assimilated into the culture of surgery and medicine globally, caution must be taken to carefully navigate the economic, legal and ethical implications of telemedicine. Despite the challenges faced, telepresence surgery holds promise for more widespread applications.

Cardiomyopathic features associated with muscular dystrophy are independent of dystrophin absence in cardiovasculature.

The loss of dystrophin results in skeletal muscle degeneration and cardiomyopathy in patients with Duchenne muscular dystrophy. In skeletal muscle, dystrophin strengthens the myofiber membrane by linking the submembranous cytoskeleton and extracellular matrix through its direct interaction with the dystroglycan/sarcoglycan complex. In limb-girdle muscular dystrophy, the loss of the sarcoglycans in cardiovasculature leads to cardiomyopathy. It is unknown whether the absence of dystrophin in cardiomyocytes or cardiovasculature leads to the cardiomyopathy associated with primary dystrophin deficiency. We show here that the cardiomyopathic features of the utrophin/dystrophin-deficient mouse can be prevented by the presence of dystrophin in cardiomyocytes but not in cardiovasculature. Furthermore, restoration of the dystroglycans and sarcoglycans to the cardiomyocyte membrane is not sufficient to prevent cardiomyopathy. These data provide the first evidence that dystrophin plays a mechanical role in cardiomyocytes similar to its role in skeletal muscle. These results indicate that treatment of cardiomyocytes but not cardiovasculature is essential in dystrophinopathies.

Computer simulation study of the interface width of the liquid/liquid interface.

Four molecular dynamics computer simulations have been performed to study the intrinsic width and the width due to thermal fluctuations of the water/carbon tetrachloride interface. We observed that thermal fluctuations have a capillary wave character. The surface tension calculated by using capillary wave formalism shows a very good agreement with the value obtained from the components of the pressure tensor.

Structure of a mixed dipolar liquid near a metal surface: A combined approach of weighted density and perturbative approximations

We study the interfacial structure of a mixed dipolar liquid in contact with a metal surface by using a combined approach of the weighted density and the perturbative approximations. Both the molecular size and the dipole moment of various species can be unequal. The metal surface is treated by using the jellium model. Explicit numerical results are obtained for the interfacial structure of a binary dipolar liquid in contact with a metal surface of varying electron density. The theoretical predictions are compared with the results of Monte Carlo simulations and a good agreement is found for the inhomogeneous density, mole fraction, and polarization profiles of both the species in the interfacial region.

Management of carcinoma anal canal with combination chemotherapy and concurrent radiotherapy.

Results of 21 cases of squamous cell carcinoma of anal canal treated by continuous infusion of 5-fluorouracil 1000 mg/sqm for 4 days and mitomycin-C 15 mg/sqm on 1st day by i.v. route with concurrent external beam radiotherapy of 42 Gy from 1st day of chemotherapy were analysed. Out of 21 patients, 16(76.19%) achieved complete remission, 3 (14.28%) partial remission and 2 (9.52%) cases did not respond at all. On follow-up, 7 (33.33%) histopathological positive cases needed abdominoperineal resection with good recovery. After 2 years of follow-up 14 (66.67%) cases were free from the disease and 3 (14.28%) were alive with the disease. The median duration of follow-up was 36 months.

Strategies educated mothers use to ensure the health of their children.

Mothers attending the out-patients in a programme of primary health care in rural west Bengal were interviewed for obtaining personal, socio-economic, and health data. Anthropometric measurements were made on them and their accompanying children. Sixty-five of the mothers were educated (defined as primary level and above of education) and 136 were not. The uneducated group had experienced a greater rate of child loss at 130 per 1000 births compared to 58 per 1000 births in the educated group. They were shorter (mean height 1.487 +/- 0.0475 m) compared to the educated group (mean height 1.507 +/- 0.051 m; P less than 0.01). Their children had a higher proportion of growth deficiencies, significant for height-for-age (P less than 0.001). These differences persisted after controlling for socio-economic status. The strategies used by the educated mothers were significantly more appropriate than those of their non-educated counterparts with regard to pregnancy and childbirth, diarrhoea, immunization, family planning, and source of treatment in illness. The educated women also benefited more from the primary health care programme.
