Amplification and overexpression of the c-erbB-2 protooncogene in human gastric cancer.

The c-erbB-2 protooncogene encodes a possible growth factor receptor. This gene has been studied as to whether it can be regarded as a prognostic indicator in human breast carcinoma. As amplification and overexpression of the gene have been reported in several adenocarcinomas, 24 specimens of human gastric cancers were examined by immunohistochemical staining (24 cases), by Southern blotting (23/24) and by Northern blotting (16/24). Amplification of the gene was detected in two moderately differentiated tubular adenocarcinomas (8.7%), and overexpression of c-erbB-2 mRNA was detected in three moderately differentiated tubular adenocarcinomas (18.8%). By immunohistochemical staining of paraffin-embedded tissues using a polyclonal antibody to c-erbB-2 gene products, the cell membrane was stained positively in three cases of gastric cancers which overexpressed c-erbB-2 mRNA. Peritoneal metastases were found in six gastric cancers, including two moderately differentiated tubular adenocarcinomas in which amplification of c-erbB-2 occurred. These results suggest that amplification and overexpression of c-erbB-2 may be correlated with metastases in differentiated adenocarcinoma of the stomach.

Dependence of endocochlear potential on basolateral Na+ and Cl- concentration: a study using vascular and perilymph perfusion.

Inside-positive endocochlear potential (EP) and high potassium concentration in the endocochlear duct are generated by transepithelial K+ transport in marginal cells of the stria vascularis. In order to estimate the degree of involvement of Na+ and Cl- in K+ transport in marginal cells, EP in guinea pigs was measured under artificial vascular and perilymphatic perfusion in situ. Na+ depletion due to both vascular and perilymphatic perfusion decreased EP by -10.0 +/- 4.1 mV (delta EP = -86 +/- 5.2 mV, n = 5) from the control value of 78 +/- 4.3 mV (p < 0.01). Cl- depletion due to vascular and perilymphatic perfusion also decreased EP by -10.0 +/- 4.9 mV (delta EP = 8.5 +/- 4.8 mV, n = 6) from the control value of 77 +/- 5.1 mV (p < 0.01). However, under either vascular or perilymphatic perfusion, even lowering of Na+ or Cl- concentration in the perfusate decreased EP only slightly compared to the results under both vascular and perilymphatic perfusion. Furosemide, a blocker of Na+/K+/2Cl- symport, decreased EP under vascular perfusion. This dependency of EP on basolateral Na+ and Cl- concentration strongly suggests that K+ transport by the marginal cell is dependent on the basolateral Na+ and Cl- concentration, and that Na+/K+/2Cl- symport is raised as a possible mechanism for Na+ and Cl- dependency of EP.

[Clinical assessment of urinary free L-fucose levels].

We measured urinary levels of free L-fucose in healthy subjects, patients with benign diseases, and patients with cancer using an automated analyzer and a newly isolated L-fucose dehydrogenase, and evaluated the clinical usefulness of the results. The values obtained were corrected for urinary creatinine as micromoles per gram of creatinine. The cutoff value, set at the mean + 2SD for the healthy subjects, was 250 mumol/g.Cr. Patients with gallbladder cancer, bile-duct cancer, liver cancer, pancreatic cancer, or cirrhosis of the liver had significantly higher levels of L-fucose than the healthy subjects. The diagnostic sensitivity for these five diseases, taken together, was 68% (144/213). Specificity for the detection of cancer was calculated by use of false positives for patients with cholelithiasis, hepatitis, and pancreatitis: it was 73% (76/104). Diagnostic accuracy for these seven diseases taken together was therefore 69% (220/317). We compared the positive ratio of the L-fucose level with that of the tumor markers AFD and CA19-9. The positive ratio of an L-fucose value above the cutoff was higher than the positive ratio of either marker in bile-duct cancer, gallbladder cancer, liver cancer, and pancreatic cancer. The results suggested that the urinary levels of free L-fucose reflected the metabolism of sugar chains of glycoconjugates, and may be usefully clinically as a tumor marker.

[Application of image cytometry to cytological diagnosis of breast tumors].

Computerized morphological analysis of the cell nuclei was performed on 66 cases (2222 nuclei) of aspirated materials from breast tumors: 25 benign cases (302 nuclei), 41 malignant cases (1420 nuclei). Its diagnostic significance and the correlation between nuclear analysis and clinical staging were studied. Nuclear diagram and intranuclear chromatin distribution pattern were evaluated using computer system named 6400. The malignant tumor had larger, more round-shaped nuclei and less uniform chromatin distribution. Under tnm classification, stage IV tumors had larger, more flattened nuclei and more irregular chromatin distribution pattern than the others. Computerized morphological analysis of breast tumors may give a quantitative aspect to classical diagnostic process. Furthermore, this analytical method provides information on clinical stage of breast cancer.

[Neo-adjuvant chemotherapy with tegafur suppository for rectal cancer--evaluation of the antitumor effects, tissue levels of 5-FU and inhibition of thymidylate synthase. Tochigi Colorectal Cancer Study Group].

We evaluated antitumor effect histologically and assayed the tissue levels of 5-fluorouracil (5-FU) and thymidylate synthase (TS) activity using surgical specimens obtained from the patients with rectal cancer, who were given tegafur suppositories prior to surgery. The antitumor effect was evaluated histologically according to classification of the general rules for the gastric cancer study (Japanese Research Society for Gastric Cancer). In 39 patients, 16 tumor specimens revealed no effect (grade-0), 22 tumors grade-1 effect, and one was not evaluable because of the severe inflammatory changes. In 23 of these patients, resected specimens were available for the assay. 5-FU levels in cancer tissues were significantly higher than those in normal tissues, and TS inhibition rates (TSIR) were almost identical, averaging around 20%, in both cancer and normal tissues. Comparing the 5-FU levels and TS activity according to the histological effects (i.e.: 'grade-0' vs 'grade-1'), the 5-FU levels in the tumors achieved grade-1 were significantly higher than in the tumors showed 'grade 0' (p less than 0.01), and TSIR in the former were relatively greater than in the latter (p = 0.053). It is suggested that both tissue levels of 5-FU and TSIR may be useful parameters to predict the anti-tumor effect against rectal cancer after administration of 5-FU and its derivatives.

Beta-adrenoceptor-mediated depolarization of the resting membrane in guinea-pig papillary muscles: changes in intracellular Na+, K+ and Cl- activities.

Effects of beta-adrenergic stimulation on the membrane potential and intracellular Na+, K+ and Cl- activities were examined in isolated guinea-pig ventricular muscles using conventional and ion-selective electrodes. Isoproterenol in concentrations of 30 nM - 1 microM produced a transient depolarization followed by a slight hyperpolarization in electrically stimulated or quiescent papillary muscles. The negative logarithm of the concentration producing 50% maximum effect (pD2) for the membrane-depolarizing effect of isoproterenol was smaller than that for the positive inotropic effect, suggesting that a higher level of cAMP accumulation is required to produce the transient depolarization. Whereas the isoproterenol(1 microM)-induced depolarization was not blocked by tetrodotoxin (10 microM), nifedipine (10 microM), Cs+ (5 mM), Ba2+ (0.3 mM), amiloride (1 mM) or ouabain (10 microM), it was significantly attenuated by anthracene-9-carboxylic acid (1 mM), a Cl(-)-channel blocker. Intracellular K+ activity increased, whereas intracellular Na+ activity slightly decreased during the transient depolarization. Intracellular Cl- activity significantly decreased during the isoproterenol-induced depolarization of the resting membrane. These results suggest that an inward current resulting from outward Cl- movement, rather than inward Na+ movement, may be involved in the beta-adrenoceptor-mediated membrane depolarization.

[Clinical observation on the dip location of industrial deafness and physiological consideration on the dip origin].

It has generally been agreed that the industrial deafness is characterized by the hearing loss of dip type at 4 kHz, so-called c5 dip, in the early stage of the deafness. When the hearing tests were to be made at each 1 kHz on either side of 4 kHz, however, the hearing pattern of so-called c5 dip which was shown clearly on the octave audiogram disappears so often and the accurate locations of the dip could occur at 5 kHz or 6 kHz. In short, it was reconfirmed through this observation that the early dip was found not only at 4 kHz but also frequently at 5 kHz or 6 kHz, as already reported by some investigators. Then, it is indispensable to measure the hearing thresholds at 5kHz and 6kHz for the early diagnosis and control of the industrial deafness. Furthermore, it should be emphasized that the term "c5 dip" is replaced by the term "high frequency dip". As the origin of inducing high frequency dip, the vaso-constriction of cochlear blood vessels which present the sympatheticotonic condition by chronic exposure to the intense sounds seems to be the most possible factor. At present, the vascular hypothesis has been put forward as the most reliable one, attributing the origin of the dip to the chronic blood flow disturbance at the operating area of basilar membrane which corresponds with the dip frequency. From the anatomical viewpoint, it would possibly be suggested that the blood flow disturbance is liable to occur at the anastomosing region between the cochlear proper artery and the cochlear branch of vestibulo-cochlear artery.(ABSTRACT TRUNCATED AT 250 WORDS)

[Plasma and tissue concentration of tegafur by a new soft capsule type suppository in colorectal carcinoma].

Plasma, tissue, and Lymph-node concentrations of tegafur and 5-FU were examined in 19 patients with colonic and rectal carcinoma after administration of tegafur by a new soft capsule type suppository: Plasma levels of tegafur and 5-FU after administration by soft capsule were much higher than those seen using supp. type suppositories. Tissue levels of 5-FU were high in tumor tissue compared to levels found in adjacent normal tissue. Lymph node levels of 5-FU were higher than plasma levels. Lymph node levels of 5-FU in the first-group lymph nodes were higher than in the second-group. These results suggest the clinical usefulness of administration of tegafur by soft-capsule suppository.

[Therapy of advanced neuroblastoma by the protocol of the Welfare Ministry used by the Sawaguchi Group].

Five patients with advanced neuroblastoma were treated with the protocol of the Welfare Ministry used by the Sawaguchi group. This protocol consists of cyclophosphamide (CPM), vincristine (VCR), 4'-O-tetrahydropyranyladriamycin (THP-ADM) and cisplatinum (CDDP). Two patients treated with this protocol as the first therapy obtained partial remission. Two patients who had obtained complete remission before the treatment with this protocol remained in complete remission after the treatment. One patient who had shown poor response to the therapy given before treatment with this protocol also showed poor response to the protocol. The most significant side effect with this protocol was myelosuppression, the severity of which was closely related to the dose of THP-ADM. Damage to the liver, kidney and heart was not very significant. We advocate that patients with advanced neuroblastoma can obtain complete remission with the aid of surgery and radiotherapy, etc., if they are treated with the present protocol as the first therapy.

Interaction of polyanion against kanamycin ototoxicity.

We studied whether interference of the electron bindings between kanamycin (KM) and the outer plasma membranes of the hair cells with polyanion such as heparin can reduce ototoxicity. In a short course experiment, KM 200 mg/kg/day was injected intramuscularly 23 times with or without 1 U or 0.5 U of heparin/g/day differently to 22 guinea pigs. The reducing effect of heparin against KM ototoxicity was significant from a comparison of the N1 threshold obtained by the cochleogram. The number of surviving outer hair cells in the heparin groups was significantly greater in the third turn than in the group given KM alone. A long course experiment in which 50 injections of KM with or without 0.5 U of heparin were given intramuscularly to 16 guinea pigs revealed from the pinna reflex, cochlear microphonics and cell counting that heparin could reduce ototoxicity slightly at an early stage or before reaching crucial accumulation.

[Multiple submucosal cysts of the stomach associated with IIc + III type early gastric cancer--report of a case].

A 59-year-old man was admitted with a complaint of epigastric discomfort, under the diagnosis of gastric cancer, subtotal gastrectomy was performed. In the resected specimen IIc + III type early gastric cancer were observed at the lesser curvature of the corpus and multiple submucosal cysts in almost entire area of the stomach. Histologic examination confirmed early adenocarcinoma and an atypical epithelium, which was present separately. Submucosal cysts consisted of a pseudo-pyloric-gland-type of epithelium, containing some fundic glandular and foveolar epithelium. Frequent erosion and regenerated epithelium and extensive intestinal metaplasia were observed in the gastric mucosa. There was no carcinomatous change in the submucosal cysts.

[Pulmonary resection by microwave tissue coagulator (MTC)--clinical application].

A 55 year old male with metastatic lung and liver cancer was subjected to pulmonary and hepatic resections using Microwave Tissue Coagulator (MTC). No adverse effects were observed in this patient after operation. The resected specimen of the lung had the bronchioli partially accompanied with cartilages. Coagulation for the bronchioli of at least lmm in size, by MTC, could be carried out resulting in no air leakage after operation.
