A double-blind, randomized study of naproxen and acetylsalicylic acid after surgical removal of impacted lower third molars.

100 patients had an impacted lower 3rd molar surgically removed in a double-blind study. Naproxen (500 mg b.i.d.) or acetylsalicylic acid (ASA) (1 g t.i.d.) were administered to the patients. Paracetamol was allowed as escape medication. 49 patients received naproxen and 51 ASA. 4 patients from each group were excluded because they took other analgesics, took too few tablets, were lost to follow-up or had misunderstood the instructions. There was a significantly better over all analgesic effect of naproxen than ASA (p = 0.004). More patients in the naproxen group than in the ASA group (p less than 0.01) would accept treatment with the same drug again. 4 patients, all from the ASA group, complained spontaneously about side effects.

Comparative study of secondary and late secondary bone-grafting in patients with residual cleft defects. Short-term evaluation.

This report is a comparative study of the treatment results after secondary and late secondary bone-grafting in cleft-palate patients with residual cleft in the alveolar process, operated with surgical technique as described by Krantz Simonsen. 293 bone-grafted cleft-palate patients are included in the study. Marginal bone-level, periodontal state of teeth adjacent to the cleft-region, recurrence of oro-nasal fistula together with possibility of total dental rehabilitation (i.e., morphology of alveolar process in cleft region and amount of attached gingiva) have been used for evaluation of the treatment results. The best results are seen in the youngest group of patients, and based on the results of the present study, bone-grafting is recommended immediately before eruption of the canine in the cleft-region.

Incidence of pain conditions in dental practice in a Danish county.

From September 1981 to February 1982, an epidemiologic study was carried out on the incidence of pain conditions among patients between 18 and 70 yr of age in 30 dental practices in Ribe County, Denmark. A total of 35 464 patients consulted the 50 participating dentists during the study period. 823 patients consulted the dentist due to pain. The most frequent cause of pain was caries sequelae (pulpitis (35%) and acute apical periodontitis (31%)). There was a significant difference in the distribution of the diagnoses between patients with regular and non-regular dental care. Although the data in the present study may not be representative for the entire Danish population, it seems reasonable to assume that in Denmark there are at least 70 000 annual consultations to a dentist due to pain. Furthermore, a minimum of 4000 patients can be expected to be absent from work annually, due to illness caused by dental pain.

A study of operative treatment and bacteriological examination of persistent oro-antral fistulas.

A retrospective study of 35 patients with oro-antral fistulas of more than 2 weeks duration is presented. Treatment results by using the Rehrmann plastic procedure, as well as complications and sequelae are described. The results of bacteriological examination in 15 patients with maxillary sinusitis in association with oro-antral fistulas are presented. The bacteriological findings suggest that broad spectrum antibiotics in certain conditions should be administered in cases of persisting oro-antral fistulas associated with maxillary sinusitis. The results of the present study show that the Rehrmann-operation proves satisfactory even in treatment-resistent cases.

Outcome of treatment with implant-retained dental prostheses in patients with Sjögren syndrome.

The purpose of this investigation was to evaluate the outcome of treatment with implant-retained prostheses in patients suffering from Sjögren syndrome. Eight women were included in the study; all had suffered oral symptoms of Sjögren syndrome for many years. Seven patients were edentulous in both arches, and 1 patient was edentulous in the maxilla only. All patients reported poor or very poor comfort levels with their conventional dentures. It was the intention to treat each arch that showed subjective and objective denture problems with a complete fixed prosthesis after placement of 6 implants. In all, 54 Brånemark dental implants were placed in these patients. No implants were lost, but 7 implants in 4 patients were clinically not osseointegrated at the time of the abutment connection procedure. Because of nonosseointegrated implants and lack of jawbone, 3 arches were treated with an implant-retained overdenture. Fixed prostheses were made with a titanium framework of premachined components welded together (Procera) and acrylic resin teeth and flanges. Patients answered a questionnaire regarding their oral function before the onset of treatment and 1 month and 2 years after treatment. An average radiographic bone loss of 0.7 mm from the time of implant placement to 1 year after treatment was observed; additional bone loss of less than 0.6 mm was recorded 4 years after treatment. During the first year of function 2 implants lost osseointegration. No prostheses were lost or remade. Treatment with implant-retained prostheses considerably increased the prosthetic comfort and function of the patients. Two years after prosthetic treatment, only 1 patient indicated poor comfort of the prostheses, while the remaining patients reported good or very good comfort levels.
