[Myopericytoma-type perivascular myoma located in the soft tissue of the foot: a case report and review of the literature].

We describe a rare case of myopericytoma-type perivascular myoma (MTPM) which arose in acral location and the literature on this field is briefly reviewed. The patient, a 68-year-old man, presented with an enlarging painful nodule, 1.4 cm across, located in the subcutaneous tissue of the right foot. The patient is alive and well 17 months after surgical excision of the nodule. Microscopically, the lesion was well-circumscribed and characterized by a biphasic pattern with a concentric perivascular spindle and ovoid cell proliferation and an extensive hemangiopericytomatous growth component. Immunohistochemically, the neoplastic cells were positive for vimentin, smooth muscle actin, desmin and calponin, negative for S-100 protein, CD34, CD31 and cytokeratins (AE1/AE3, Cam 5.2). Electron microscopy showed electrondense cell bodies suggestive for myopericytic differentiation. The clinico-pathological features of the present case are similar to those previously reported in the literature and we also discuss herein the main histological criteria for the differential diagnosis with other spindle cell and vascular lesions of soft tissues.

[Galanin immunoreactivity in a laryngeal paraganglioma: case report and literature review].

The authors describe a case of laryngeal paraganglioma (LP) occurring in a 57-year-old-woman. To date, 70 cases have been described in the literature. It is benign and recurrences are infrequent. The differential diagnosis with typical and atypical carcinoids, hemangiopericytomas, alveolar soft-part sarcomas, medullary thyroid carcinomas, malignant melanomas and metastatic renal cell carcinomas is supported by immunohistochemistry. Moreover, this tumor shows the immunohistochemical expression of galanin, a variably expressed marker of paragangliomas that it is not expressed in carcinoid tumors. Nevertheless, our observations militate against its role as a solitary marker but advocate its use in conjunction with other antibodies for the differential diagnosis of neuroendocrine neoplasms of larynx.

[Ganglioneuroma of parapharyngeal space: case report and review of the literature].

A case of ganglioneuroma of parapharyngeal space is described and the literature on this field is briefly reviewed. The patient, a 7-year-old female, presented with a left parapharyngeal space mass causing medial displacement of the left lateral pharyngeal wall. The lesion sized 4.4 cm in the largest diameter. Histologically, the tumour showed the coexistence of two distinct cells types--ganglion cells and Schwann cells--within a loose stroma. Immunohistochemically, both cell types were immunoreactive for S-100 protein and negative for cytokeratins.

[Lymphoepithelioma-like carcinoma of the esophagus: description of a case].

The authors describe a case of lymphoepithelioma-like carcinoma (LELC) of the esophagus occurring in a 77-year-old man. To date, 13 cases have been described in the literature, all in natives of Japan. Histological features, immunohistochemical findings and differential diagnosis are discussed. In our case epithelial neoplastic cells were completely negative for the EBV genome.

[Lympho-epithelial carcinoma (malignant lympho-epithelial lesions) of the parotid gland. Description of a case with detection of EBV by in situ hybridization].

The authors describe a case of malignant lymphoepithelial lesion (MLEL), commonly referred to as lymphoepithelial carcinoma of parotid gland, that is a very rare tumour. There is a relatively high incidence in Eskimos of Alaska and Greenland, but some cases are described in natives of south China. The immunophenotypic profile and histopathological aspect of this neoplasm are discussed, and the differential diagnosis in regard to other primitive or metastatic tumours of parotid is also considered. In our case a diffuse positivity of epithelial neoplastic cells for EBV genome was found using in situ hybridization. The possible role of EBV in the ethiopathogenesis of this rare lesion is herein discussed.

[Hemangiopericytoma-like tumor (HPCLT) of the nasal cavity: morphological, immunohistochemical, and ultrastructural features].

A case of hemangiopericytoma-like tumour (HPCLT) of the nasal cavity is described in a 81-year-old woman. The tumour, accompanied by symptoms of nasal obstruction and epistaxis, presented as a polypoid lesion filling the left middle meatus region. Histological, immunohistochemical and electron microscope studies revealed a uniform sheet-like proliferation of spindle to oval vimentin-positive cells growing around a rich vascular network. The favourable outcome sixteen months after endoscopic sphenoethmoidectomy would seem to confirm the diagnosis of HPCLT. The differential diagnosis of this rare lesion is discussed.

[Eradicating therapy of Helicobacter pylori in the duodenal ulcer].

Helicobacter pylori (HP) is the conclusive etiopathogenetic element of peptic lesion. The disappearance of the bacteria from the antral mucosa involves a rapid cicatrization of the ulcer with correlated recidivist, in direct manner, to reinfections or insufficient treatments. The leading problem lies in definitive bacterial eradication, made difficult by the notable resistance of the HP to the antimicrobic agent. The therapeutic solutions require the contemporary use of medicine with a different mechanism of action. A triple medicine scheme has been experimented, with non-contemporary administration of Azithromycin and subsequently of metronidazol for 3 days and a tablet/day of omeprazolo for 30 days. The research was carried out on a sample of 60 units, affected by duodenal ulcer in active phase, with endoscopic and hystologic controls at the beginning and follow up at 60 days. The results show a bacterial eradication which exceed 90%, with regression of the symptomatology in 48-72 hours and side effects of modest entity. The collaboration of the subjects, after informed consent, was total, also favoured by the brevity of the cycle, uniformity of the dosing and easy distribution/day. The data confirm the etiopathogenetic value of the HP in the gastric and duodenal ulcer.

[Bi- and tridimensional reconstruction with spiral computerized tomography in esophageal neoplasms. Preliminary clinical applications].

Spiral CT allows rapid data acquisition in an entire anatomical region during a single breath-hold and permits to obtain 2D and 3D reconstructions with good diagnostic accuracy. We investigated the capabilities of spiral CT reconstructions in demonstrating esophageal cancers. Spiral CT was performed in 10 patients with esophageal cancers and in 3 patients who underwent endoprostheses placement with surgical or interventional procedures. Bidimensional (MPR, CPR, MIP) and three-dimensional reconstructions with Shaded Surface Display (SSD) were obtained at the end of each exam. In the analysis of the results, CT reconstructions were compared with previous barium esophagography. Some of the main semiologic patterns were considered: tumor visualization, longitudinal extent of the neoplasm and its relationships with adjacent structures. MPRs were obtained in 9 patients and allowed to visualize the esophageal lumen with a filling defect, the neoplastic mass and its longitudinal and extraluminal extent. SSD reconstructions was obtained in 7 patients and allowed a good assessment of longitudinal neoplasm extent in 6 patients. Neoplastic mass visualization and extraluminal tumor extent were difficult to assess in all cases. MIP reconstructions were obtained in all patients and demonstrated longitudinal neoplasm extent with good accuracy. Neo-plastic tissue was visualized only indirectly through the filling defect of the esophageal lumen. MIP allowed to define, in 5 patients, the relationships of the neoplasms with adjacent vascular structures after the intravenous administration of contrast agent. In conclusion, we consider that Spiral CT reconstructions can be useful for a more accurate staging of esophageal cancers.

Venous angiomas: plain and contrast-enhanced MRI and MR angiography.

We studied 17 patients with venous angiomas, 4 of whom had associated cavernous angiomas. All underwent MRI with spin-echo T1- and T2-weighted images and T1-weighted images after Gd-DTPA; MR angiography (MRA) was also performed with 3D and 2D time-of-flight technique; 5 patients underwent conventional angiography. Contrast-enhanced MRI demonstrated all the lesions, showing the peripheral medullary veins, the collector and the type of drainage. Both 3D and 2D MRA provided diagnostic information identical to that obtained after infusion of Gd-DTPA. Contrast-enhanced were T1-weighted images and MRA superior in all the cases to images without gadolinium. The possible association with cavernous angiomas (24% in our study) indicates T2-weighted imaging.

[Angiography with spiral computed tomography. The basic principles and clinical potentials].

Technical characteristics, acquisition modality and data representations as well as examination protocols and main clinical applications of spiral CT angiography (CTA) are described. The different methods of 2D (MPR, CPR) and 3D (SSD, MIP) data display and their advantages and limitations were investigated. We describe the examination technique representing an important tool to achieve good image quality and to make the correct diagnosis. The clinical effectiveness and the main applications in the body were studied. CTA already has a clinical role for arterial system study, while it is less indicated in the venous system because good and continuous contrast agent opacification is difficult to obtain. In our preliminary experience, CTA can be considered an effective alternative to DSA in the assessment of vascular conditions because it is easier to perform, more rapid and less invasive.

[Aneurysmal disease. Initial clinical experience with spiral computerized tomography angiography].

This study was aimed to assess the comparative capabilities of CT angiography (CTA) and DSA in diagnosing aneurysms in different sites in a series of 20 patients. Volumes were measured during intravenous contrast agent infusion and images were then edited and reformatted as 2D and 3D displays (MPR, SSD, MIP techniques). Some of the main semiologic patterns of aneurysms were considered and the diagnostic accuracy of the 3 reconstruction techniques was assessed and compared with that of axial CT and DSA. Abdominal aortic aneurysms were analyzed separately to study the involvement of main collateral vessels. The best results were obtained with MIP and MPR in the assessment of aneurysm extent and neck. MPR is the only technique which could depict the thrombus and the best technique to demonstrate the relationships between aneurysm and adjacent structures. The MIP technique depicts calcifications and is the method of choice to demonstrate the involvement of collateral vessels by abdominal aortic aneurysms. SSD permits good assessment of the neck and of the longitudinal extent of the aneurysms but is severely limited in the analysis of the other variables.

Noninvasive monitoring of ovarian cancer: improved results using CT with intraperitoneal contrast combined with immunocytology.

The purpose of this study was to evaluate whether CT with intraperitoneal contrast (ipc/CT) in combination with immunocytochemical (ICC) tests could improve the diagnostic accuracy over conventional methods in the follow-up of ovarian carcinoma. Forty-five clinically disease-free ovarian cancer patients eligible for a second-look laparotomy were given an intraperitoneal infusion of positive contrast material followed by pelvic and abdominal CT. After the radiographic examination, the contrast fluid was collected by paracentesis and processed for morphological as well as for immunocytochemical analysis employing a panel of monoclonal antibodies identifying distinct ovarian tumor-associated antigens. While ipc/CT correctly detected the presence of peritoneal recurrences in 22 of 45 (49%) patients, the immunocytochemical tests demonstrated the presence of otherwise undiagnosed microscopic disease in an additional 8 patients (18%). Our results demonstrate that the combination of radiological and immunological methods may represent a more accurate, noninvasive means of monitoring ovarian cancer, thus reducing the need for a second-look laparotomy.

T stages of tumors of the tongue and floor of the mouth: correlation between MR with gadopentetate dimeglumine and pathologic data.

PURPOSE: To define MR accuracy in the evaluation of the T stages of tumors of the tongue and floor of the mouth. METHODS: Fifty-two patients affected by squamous cell carcinoma were studied with a superconductive system at 1.5 T. The study was performed with spin-echo T1- and T2-weighted images before contrast and short spin-echo T1-weighted and gradient-echo sequences after gadopentetate dimeglumine administration. The results obtained with MR were compared with pathologic data. RESULTS: Good correlation of T stages (TNM system) was obtained in 45 of 52 cases. MR did not show four superficial lesions. In one case, infiltration of the cortical bone of the mandible was not demonstrated (MR, T2; pathologic, T4), and in another the lesion was classified as T2 instead of T3, as it was pathologically. One lesion was classified as T4 on MR because of infiltration of the alveolar ridge but was classified as T2 at surgery. In 46 patients who underwent surgery, the accuracy of MR was excellent for predicting the relationship of tumor to midline and oral floor musculature. The results obtained with gadopentetate dimeglumine were better than those obtained in noncontrast studies in 32 (62%) of 53 cases. CONCLUSION: MR showed high accuracy in the study of tumors of the tongue and floor of the mouth.

[Percutaneous biliary drainage in obstructions due to malignant pathology].

Experience with percutaneous transhepatic biliary drainage (PBD) in 68 cases of malignant biliary obstructions, is described. 78 procedures (11 external drains, 36 internal-external drains, 17 soft endoprosthesis and 14 self-expandable metallic stents), were performed between December 1989 and December 1991. No significant differences in terms of results were found between internal endoprosthesis and metallic stents. The mortality rate of the procedure was 1.47% (1 case). Complications for the internal prostheses were: obstructions 8 (25.8%), dislodgement 2 (6.5%), cholangitis 1 (3.2%) and metastases along the catheter tract 1 (3.2%). Considering this, the Authors prefer the endoprosthesis instead of external or internal-external drainages.

[Primary embryonal rhabdomyosarcoma of the breast. Description of a case].

Authors report a case of embryonal rhabdomyosarcoma of the breast occurring in a 16-year-old girl. To date, 26 cases have been described in the literature of which only 4 in girls younger than 16 years. Histological features, immunohistochemical findings and differential diagnosis are discussed.

[The magnetic resonance estimation of the T parameter in the staging of tumors of the oral cavity and tongue. A correlation with postoperative data and preliminary echotomographic experience].

Fifty-three patients with oral cavity and tongue tumors were studied with MRI, 18 with CT and 10 with US. MR results were compared with postoperative and clinical results relative to the T variable of the TNM classification of oral cavity tumors. MRI demonstrated 51 of 53 tumors; it was in agreement with surgery, relative to T, in 43 of 48 cases and with clinics in 5 of 5 cases. In 5 patients MRI understaged: two lesions were classified as T0 because the tumors developed on the surface (p T4, for alveolar ridge infiltration). Moreover, MRI staged another lesion as T2 instead of T3 because of its superficial component, while in 2 cases the infiltration of the cortical bone of the jaw was not demonstrated (T1 at MRI, T4 at p; T2 at MRI, T4 at p). MRI was reliable enough to evaluate the relationships between the lesions and the surrounding structures but it was limited in demonstrating superficial lesions and in evaluating the infiltration of the alveolar ridge and of the cortical bone of the jaw in the early stage. The use of gadolinium proved to be especially useful for the exact definition of the size of the lesions and of their relationships with the surrounding structures. CT proved better than MRI to evaluate bone involvement in one case, even though neither technique could identify bone involvement in another patient. US yielded accurate information on the presence of lesions, their size and relationships with midline and floor of the mouth muscles, while its demonstration of the relationships between tumors and bone structures was poor, especially relative to tumor spread outside the oral cavity.

The basement membranes in sarcomatoid carcinomas. An immunohistochemical study.

AIMS: Eight sarcomatoid carcinomas from various anatomical locations were investigated by immunohistochemical staining to laminin, type IV collagen and heparan sulfate proteoglycan, to study the characteristics of basement membranes at the interface between carcinomatous and sarcomatous tissues. METHODS: Paraffin wax embedded tissue sections from representative tumor samples have been stained with specific antibodies, using the peroxidase-antiperoxidase technique. RESULTS: In all cases several interruptions or discontinuities of the basement membrane staining pattern were seen. In 4 cases, larger defects or complete loss of staining was also noted. At these places, the boundaries between carcinomatous and sarcomatous tissue were often blurred. CONCLUSIONS: Disruption and loss of basement membranes at interface between carcinomatous and sarcomatous tissues is a frequent finding in sarcomatoid carcinomas. These changes could be consistent with an epithelial origin of the sarcomatous component in these tumors by means of an epithelial-mesenchymal conversion mechanism.

Ductal adenoma of the breast. An immunohistochemical study of five cases.

The immunohistochemical features of 5 ductal adenomas of the breast were investigated. The tumours were composed by tubular glands lined by cells with both luminal and basal/myoepithelial immunophenotypes. Tubules were encircled by an intact basement membrane as demonstrated by staining for laminin and type IV collagen. Areas of dense fibrosis, a usual finding in these lesions, stained for fibronectin, type III collagen and contained vimentin and actin positive spindle cells, consistent with myofibroblasts. These observations confirm a double epithelial and myoepithelial differentiation in these lesions and seem to support the suggestion that ductal adenomas result from sclerosis of ductal papillomas.
