Autonomic nervous activity during sleep in middle cerebral artery infarction.

No data are available on the autonomic system during sleep in patients with stroke. The purpose of this study was to determine the influence of acute ischemic stroke on the autonomic cardiovascular system during sleep, and to correlate autonomic activity with the clinical status of patients. Ten patients with ischemic stroke in the middle cerebral artery were studied by means of an all-night polysomnographic recording within the 1st week of the onset of symptoms and at the 3-week follow-up examination. Power spectrum analysis of the heart rate variability was performed using an autoregressive algorithm in 180 consecutive electrocardiographic RR intervals. Spectral power was calculated in 3 main frequency bands: high frequency (HF), 0.15-0.4 Hz; low frequency (LF), 0.04-0.15 Hz; very low frequency (VLF), < 0.04 Hz. The data were compared with those of 10 age-matched controls. A significant increase in VLF (p < 0.0005) and a decrease in HF (p < 0.0002) components were found in ischemic stroke patients. The sympathetic-parasympathetic balance (VLF + LF/HF) was higher in patients than controls (p < 0.005). However, these components changed significantly during sleep, revealing a physiological pattern. These power spectral data were still present at the 3-week follow-up. The 4 patients who developed cardiac arrhythmias showed higher sympathetic-parasympathetic balance than patients without arrhythmias (p < 0.05). These data suggest a sympathetic predominance in patients with acute ischemic stroke during sleep. However, the flexible and dynamic properties of the autonomic nervous system are preserved. Cardiac arrhythmias following stroke may be related to the degree of sympathetic predominance.

[Surgical treatment of bleeding peptic lesions: our experience].

The authors report their experience on surgical treatment of peptic bleeding lesions. From January 1994 till April 1999 they observed and surgically treated 35 patients (mean age 65) suffering from bleeding gastroduodenal ulcer. Complications linked to surgical treatment had an incidence of 17.5%, while those ones linked to the patient's general conditions of 21%; mortality was 20%. Surgery has been gradually substituted by endoscopy which represent the principal examination for diagnosis of bleeding gastroduodenal ulcer with the aid of different hemostatic techniques, so that surgery has been relegated to the last place in uncontrollable bleeding treatment. Observed results, following those ones of other authors, show the unfavourable prognosis linked to patient's different conditions when surgeon operates.

[Dual mesh-plus for wall reconstruction in incisional and umbilical hernia in the aged].

The article reports the author's experience about the use of Gore-Tex Dual Mesh Biomaterial in large incisional hernia operations in patients over seventy. From January 1996 at present day we operated on 23 patients for treatment of abdominal wall defect. In all patients we used a Dual Mesh. Overall mortality was O. Morbility was 9.5%. Follow-up is too short for definitive considerations about the incidence of recurrences but our initial experience with this material encourages us to use it again for replacement of abdominal wall defects especially in the old patient.

[Retrospective study of 41 cases of pulmonary carcinoma diagnosed from 1983 to 1986].

In their retrospective study of 41 cases of lung carcinoma diagnosed in their department in the years 1983-86, the authors aimed at assessing the dynamics of their clinical, instrumental and therapeutic approach to this widespread and socially relevant pathology. Among the aspects considered are: the patients' mean age and sex, relationship to smoking, clinical and cytologic diagnosis, instrumental exploration, presenting symptoms, therapeutic approach, mean survival time. On the basis of their findings, the authors come to the conclusion that their experience coincides with that reported in national and international studies with the same dramatic picture (usually late onset of symptoms, short survival, highly invasive growth). They therefore hope that in the near future the scientific community will engage in a more decisive manner and with better support from the health authorities and more adequate financial resources in ways aimed at reducing the incidence of this pathology, at acquiring better knowledge of the relationship with the environment, and at an improvement of diagnostic and therapeutic strategies.

[Phyllodes tumor of the breast: diagnostic and therapeutic problems].

Phyllodes tumour (PT) is a rare breast tumor with mixed connective and epithelial components, which usually relapses topically after excision. Five patients with PT took part in the study (4 were positive for benign PT and one was positive for malignant PT at extemporary histological examination). The patients (3) with 3 cm or minus benign lesion had mass excision with a large resection of the surrounding mammary tissue. In the remaining patient with benign neoplasm (8 cm lesion) simple mastectomy was carried out. Finally the patient with malignant PT (5 cm lesion) underwent a simple mastectomy without following chemotherapy or radiotherapy. Follow-up (at 6 and 12 months) consisted of mammography and clinical examination. One out of the 3 patients treated with large resection had a local relapse which was then treated surgically. There was no evidence of relapse at 12 months in the remaining patients with benign tumor. The patient with malignant PT returned to our attention with multiple bone metastases in the thoracic lumbar tract two years later. She received multiple courses of combined radiotherapy and chemotherapy. Local relapses do not appear to affect survival: as a consequence, wide excision should be the primary treatment of benign phyllodes tumors. Total mastectomy has been indicated for malignant phyllodes tumors and for very large tumors.

[Early gastric cancer: our experience].

The early gastric cancer (EGC) is a very interesting pathology as it represents the first phase of gastric cancer, curable in most cases. The aim of the present study is to evaluate the outcome of surgical treatment of EGC in 19 consecutive patients. The studied patients were followed up for 5 years, during which they underwent a gastroscopy, a hepatic echography and a TC at one year intervals in case of echographic suspicion. Ten patients with limited involvement of the antrum underwent a gastric resection with Billroth II's reconstruction; five patients with circumscribed involvement of the gastric body underwent a subtotal resection; four patients (one with a multifocal involvement of the gastric body and three with multifocal involvement of the fundus) underwent a total gastrectomy with Roux's reconstruction. All patients underwent a limited lymphadenectomy of the perigastric lymph nodes. By evaluating and comparing the results of the present study with previous data on EGC, we propose gastric resection, subtotal gastrectomy and total gastrectomy, according to the location of neoplasm, with limited lymphadenectomy of perigastric lymph nodes.
