[Macroscopic study of cholangiocarcinoma].

Fourty three autopsy cases of cholangiocarcinoma and 18 cases of Thorotrast-related cholangiocarcinoma were macroscopically studied. They were classified into two types, according to the intrahepatic location of the tumors; middle-peripheral type and hilar type. Most Thorotrast-related cases (83%) were middle-peripheral type, while the non-Thorotrast-related cases were hilar type (68%). According to the gross characteristics of the tumor. They were classified into four gross types: infiltrative type (54%), nodular type (21%), periductal type (18%), and diffuse type (7%). Non-specific liver cirrhosis (mixed macro and micronodular type) was associated in 4 of non-Thorotrast related cases, and the association of biliary cirrhosis was found in 3 of the non-Thorotrast cases of the hilar type.

[An autopsy case with a combination of hepatocellular carcinoma and cholangiocellular carcinoma].

A 68-year-old male was admitted to the Kurume University Hospital with a diagnosis of hepatocellular carcinoma (HCC). Serum CEA levels ranged from 3.7 ng/ml and 6.8 ng/ml during the course. Serum AFP levels were at normal range at admission and elevated to 13,250 ng/ml 3 months before death. He died of liver failure 17 months after admission. An autopsy disclosed diffuse infiltrative tumors distributed throughout the liver with metastasis to the bilateral lungs. Histologically, tumors of the liver consisted of elements of both HCC and cholangiolocellular carcinoma. Cholangiolocellular carcinoma is believed to occur from the canals of Hering or cholangiole, and this case may provide further information as to the histogenesis of combined HCC and cholangiocarcinoma.

Emergence of malignant lesions within an adenomatous hyperplastic nodule in a cirrhotic liver. Observations in five cases.

Five cases of an adenomatous hyperplastic nodule or a similar lesion resected from a cirrhotic liver in which early malignant foci were seen as small nodule-in-nodule lesions are described. These hyperplastic lesions were detected by imaging diagnosis in patients with nonalcoholic cirrhosis, mostly during routine clinical follow-up. In 2 patients, recurrence of hepatocellular carcinoma occurred 11 mo and 15 mo postresection. Thus, these nodule-in-nodule lesions in an adenomatous hyperplastic nodule seem to represent an early stage of hepatocarcinogenesis in humans. In nonalcoholic cirrhotic patients from Japan and Southeast Asia, in whom hepatocellular carcinoma is endemic, an adenomatous hyperplastic nodule or a similar hyperplastic lesion that occurs in cirrhotic livers may be preneoplastic and already committed to malignant transformation.

Activated lymphocytes in patients with newly diagnosed type 1 (insulin-dependent) diabetes mellitus.

The expression of activation antigens (transferrin receptor, IL-2 receptor and Ia antigen) on circulating T lymphocytes from Japanese children with Type 1 diabetes was studied using five monoclonal antibodies (Ab), OKT9, anti-Tac Ab, OKIa1, anti-human HLA-DR Ab and OKT3. For detecting Ia positive T cells, the dual staining technique using OKT3 and anti-Ia antibody was employed. Four out of six patients (67%) with newly diagnosed Type 1 diabetes showed a raised level of either OKT9 or Tac positive cells when examined at diagnosis. These patients, however, rapidly lost these activation antigens after the insulin therapy was started. In contrast, in 32 long-standing patients, only 2 (6%) had a high percentage of OKT9 positive cells and none of them demonstrated Tac positive cells. One out of six newly diagnosed patients or three out of 21 long-standing patients had a significantly high percentage of Ia-positive T cells compared with normal subjects. In poorly controlled long-standing patients whose HbA1 value was higher than 14%, none of them had an increased number of activated lymphocytes. Therefore, it is unlikely that insulin deficiency and hyperglycemia were responsible for the changes observed in these studies. Activated lymphocytes might be related to activation of the immune system involved in pathogenesis of Type 1 diabetes.

[Pathomorphological study of thorotrast-related intrahepatic cholangiocarcinoma--a comparison with non-thorotrast cases].

Thirty-five autopsy cases of Thorotrast (TH)-related intrahepatic cholangiocarcinoma were morphologically studied with a comparison to 45 non-TH cases. Latent periods ranged from 25 to 48 years, with a mean of 34.1 +/- 6.6 years. As to tumor location, the peripheral-middle type, in which the main tumor was located in the periphery to middle portion of the liver, was the most common (89.2%) in the TH-related cases, and the hilar type, in which the main tumor was located in the hepatic hilum, was the most common (78.8%) in the non-TH cases. However, there was no close relationship between the distribution of TH deposits and tumor location by soft-X ray examination of the liver slices. Grossly, the massive type with an infiltrative growth was the most common both in the TH- and non-TH cases. Histologically, there were no remarkable differences between the two groups, and tubular adenocarcinoma with varying degrees of fibrous stroma was the most common. In noncancerous areas, proliferation of the bile ducts with slight to moderate atypism and ductular proliferation around Glisson's capsule were found in 30%, and 10%, respectively, of the TH-related cases. However, such changes were also found in the non-TH cases at almost the same incidence.

[The concentration in the tissue and antitumor effect of anticancer drug--assessment of breast cancer with UFT or FT-207].

The correlation between intralesional 5-FU concentrations and the extent of antitumor effect with particular attention to histopathology was assessed in 30 patients with breast cancer. Therapy consisted of either UFT or FT-207 given preoperatively. Although the UFT-treated group showed a significantly higher intratumoral concentration of 5-FU, there was no intergroup difference in 5-FU levels in normal tissue. In addition, this greater concentration of intratumoral drug had no significant effect on therapeutic response; for both UFT and FT-207 treated groups, the histologic interpretation was rated as grade IIa. Regardless of which drug was used, when tumors were stratified according to histologic type, papillotubular carcinomas revealed the greatest tissue 5-FU levels. However the greatest histologic antitumoral responses were seen in scirrhous carcinomas; hence no correlation was seen here either. The results suggest that attainment of a high intratumoral 5-FU concentration is not reflected in a better histologic tumor response in patients receiving the above drug regimens.

[Chemotherapy of renal cell carcinoma. 2. Combined chemotherapy using ACNU and VBL].

Nineteen patients with renal cell carcinoma who had undergone radical nephrectomy have been treated with a combination of chemotherapy and partly hormonal therapy using ACNU, vinblastine and chloromadinone acetate. The efficacy of the combination therapy in advanced renal cell carcinoma was studied and only one out of nineteen cases having lung and pleural metastasis shows no change lasting from one to eighteen months. The prophylactic efficacy was studied in eighteen patients with stage I to IVA renal cell carcinoma (contained one patient with stage IVA renal cell carcinoma who had undergone radical nephrectomy and coecum resection for coecum invasion). Tumor recurrence occurred in 5 cases, that is 5 cases within 1 year and 1 case within 2 years. The combination of chemotherapy using ACNU & vinblastine must be used carefully for severe side effects.

Ultrastructural study of hepatocellular carcinoma with replacing growth pattern.

The replacing growth of hepatocellular carcinoma (HCC) has been considered as a basic growth pattern at the tumor-nontumor boundary. In order to clarify the ultrastructural characteristics of HCC with the replacing growth, we have studied 59 surgical cases of HCC. At the tumor-nontumor boundary of HCC with the replacing growth pattern, the tumor cells grow to replace the hepatocytes along the liver cell cord, but oppress and distort the adjacent liver cell cord with varying degrees of aggregation of reticulin fiber. Hepatocytes and cancer cells are in direct contact, but some of the hepatocytes are compressed by cancer cells. The sinusoid in the non-cancerous area is continuous with the blood space in the cancerous tissue, and several layers of endothelial cells are often observed. Accordingly, it is predicted that sinusoidal blood through the portal vein and arterial blood through arterial tumor vessels may mix at the tumor-nontumor boundary.

[Adenocarcinoma arising in a bronchial cyst of the mediastinum].

A 39 year-old man with a mediastinal tumor was operated on. After the median sternotomy incision, a mediastinal cyst, which was connected to the right pulmonary hilus with a string, was found. The microscopic examination of the cyst revealed a bronchial cyst with ciliated columunar epithelium and muscle fibers. Adenocarcinoma was found in the mucosa of the cyst. This patient seemed to be the first reported instance of carcinoma in a mediastinal bronchial cyst.

Antitumor agent poly (amino acid) conjugates as a drug carrier in cancer chemotherapy.

Antitumor agent melphalan was conjugated through a carbodiimide-catalyzed reaction to poly (L-lysine) (71.3K and 2K) and poly (L-glutamic acid) (60K and 14K) at a ratio of approximately one molecule per 7-lysyl and 23-glutamate residues, respectively. These conjugates had 40-70% of alkylating activities by themselves in vitro as compared with free melphalan. Poly (glutamic acid) conjugates showed the antitumor activity in vivo against Yoshida sarcoma in rats. In addition, poly (glutamic acid) conjugates containing 3H-phenylalanine which was used as a model compound instead of melphalan had a sustained release property of radioactivity and the release rates could be regulated by exopeptidases. After subcutaneous injection as the first choice of routes of administration, moreover, the conjugates were found to be absorbed through the lymphatic transport system, probably due to the macromolecularity. These results suggest that the melphalan-poly (amino acid) conjugates are one of good candidates to attain the sustained release and targeting of drugs in cancer chemotherapy.

Hepatocellular carcinoma with intra-atrial tumor growth. A clinicopathologic study of 18 autopsy cases.

We studied the features of 18 cases of hepatocellular carcinoma (HCC) with prominent intra-atrial tumor growth that were seen among 439 autopsy cases of HCC. Diuretic-resistant, marked edema in the lower extremities and marked venous dilatation in the abdominal wall were observed in 14 cases (77.7%) and five cases (27.7%), respectively. Consistent tachycardia was seen in three cases. Macroscopically, most of the cases showed an infiltrative or mixed (infiltrative and nodular) growth pattern. A continuous tumor thrombus involving the right atrium, the IVC, and the hepatic vein was seen in 15 cases; a tumor that involved the right atrium alone was present in three cases. In five cases, the tumor bolus crossed the tricuspid valve and entered the ventricle.

Influence of airway geometry on expiratory flow limitation and density dependence.

In a study of individuals of the same sex and similar age and size, between subject variability was noted in lung recoil, lung size, density dependence of maximum expiratory flow, tracheal cross-sectional area, and anatomic dead space. No significance was observed in the relationships to one another of lung size or either estimate of airway size, nor did the magnitude of the change in flow with changes in gas density show a consistent relationship to any of the size variables. The results indicate that there is a dissociation between tracheal dimensions and anatomic dead space as well as between airway size and lung size, suggesting that dysanapsis is a general phenomenon. This introduces additional sources of interindividual variability of maximum expiratory flow and measurements of density dependence of flow.

[Cefotiam passage into the maxillary sinus tissues].

Clinical study was made on cefotiam (CTM) and the following results were obtained. Tissue concentrations of CTM were determined 1 hour after intravenous injection (CTM 1 g) in chronic sinusitis and maxillary cyst. Concentrations of CTM were 16.53 micrograms/g, 13.26 micrograms/g in mucosa of the maxillary sinus, maxillary cyst, respectively. Antibacterial activity of CTM was measured on Escherichia coli, Staphylococcus epidermidis, Aerococcus, Acidaminococcus fermentans, Peptostreptococcus anaerobius; that were isolated from pus of maxillary sinus. The highest MIC on them was 3.13 micrograms/ml.
