One-stop postmenopausal bleeding clinics reduce patient waiting times and theatre costs.

BACKGROUND: Postmenopausal bleeding (PMB) is a common problem and reason for referral to gynaecology clinics. AIMS: The aim of this study was to compare patient management and outcomes from a newly developed one-stop clinic for women with PMB with traditional gynaecology outpatient clinics. METHODS: Retrospective studying running from January to July 2003 comparing the one-stop clinic with four traditional consultant-led outpatient gynaecology clinics also seeing women with PMB running concurrently in the same hospital. RESULTS: In the study period, 95 and 51 women were seen in each type of clinic. There was no difference in patient demographics but the time from referral tfirst consultation was shorter in the PMB clinic (p < 0.00 ) and women had fewer visits (p < 0.001). The mean time from first consultation to definitive treatment or discharge was also shorter (p < 0.001). Fewer hysteroscopies were generated from the PMB clinic (p < 0.0001) and yet there was no difference in the rates of abnormal histology between the two groups. CONCLUSIONS: This study demonstrated that one-stop investigation of PMB, compared to traditional outpatient clinics, reduced waiting times and theatre costs by reducing the number of hysteroscopies.

Field evaluation of cypermethrin and cyfluthrin against dengue vectors in a housing estate in Malaysia.

Cynoff 25ULV (cypermethrin 25 g/l) and Solfac UL015 (cyfluthrin 1.5% w/v) were evaluated against the sentinel sugar-fed adults and 4th-instar larvae of Aedes aegypti in a housing estate endemic of dengue in Malaysia. The impact of both pyrethroids on field populations of Aedes albopictus and Aedes aegypti larvae was monitored weekly using bottle containers. Both Cynoff 25ULV and Solfac UL015 showed adulticidal effects and larvicidal effects. This field trial using Cynoff 25ULV against dengue vectors showed its potential for use in dengue vector control programs.

Breast carcinoma in Pakistani females: a morphological study of 572 breast specimens.

OBJECTIVE: To study the morphological features in breast carcinoma which have proven prognostic value. METHODS AND SETTING: A retrospective analysis of 572 mastectomy specimens received over a period of three years at the department of pathology, The Aga Khan University Hospital. RESULTS: A total of 572 mastectomy specimens were analyzed which were received over a period of three years. Most of the patients were in the 5th and 6th decades of life. The mean age at diagnosis was 48 years. The most common tumour was infiltrating ductal carcinoma (81%). Tumour size was > 2 cms. in 80% of the cases. According to Modified Bloom and Richardson system most of the tumours were in grade II (65%) followed by grade III (24%). The number of cases with > 3 lymph node metastasis was significantly higher (70%) in tumours of > 2 cms size. High grade tumours also showed increased number of lymph node involvement. CONCLUSION: In Pakistani females breast carcinoma occurs at a younger age group. They are of large size at the time of presentation and show more frequent axillary lymph node metastasis. Infiltrating ductal carcinoma is the most common type of tumour with predominance of high grade lesions.

Identification of a critical motif responsible for gating of Kir2.3 channel by intracellular protons.

Protons are involved in gating Kir2.3. To identify the molecular motif in the Kir2.3 channel protein that is responsible for this process, experiments were performed using wild-type and mutated Kir2. 3 and Kir2.1. CO2 and low pHi strongly inhibited wild-type Kir2.3 but not Kir2.1 in whole cell voltage clamp and excised inside-out patches. This CO2/pH sensitivity was completely eliminated in a mutant Kir2.3 in which the N terminus was substituted with that in Kir2.1, whereas a similar replacement of its C terminus had no effect. Site-specific mutations of all titratable residues in the N terminus, however, did not change the CO2/pH sensitivity. Using several chimeras generated systematically in the N terminus, a 10-residue motif near the M1 region was identified in which only three amino acids are different between Kir2.3 and Kir2.1. Mutations of these residues, especially Thr53, dramatically reduced the pH sensitivity of Kir2.3. Introducing these residues or even a single threonine to the corresponding positions of Kir2.1 made the mutant channel pH-sensitive. Thus, a critical motif responsible for gating Kir2.3 by protons was identified in the N terminus, which contained about 10 residues centered by Thr53.

Field evaluation of Vectobac G, Vectobac 12AS and Bactimos WP against the dengue vector Aedes albopictus in tires.

The efficacy of three formulations of Bacillus thuringiensis var. israelensis was studied against Aedes albopictus in discarded tires. The formulations were: Vectobac G (corn cob formulation), Vectobac 12AS (aqueous suspension), and Bactimos WP (wettable powder formulation). Both Vectobac G and Vectobac 12AS were effective for 24 hr with more than 80% mortality. Both Vectobac formulations were significantly more effective than Bactimos WP for 24 hr after treatment (P < 0.0005). A week after treatment, Vectobac 12AS was significantly different than Bactimos WP (P < 0.05). However, Vectobac G did not differ significantly from Bactimos WP (P > 0.05); two weeks after spraying there was no significant difference among the various formulations (P > 0.05).

Do thinner rigid gas permeable contact lenses provide superior initial comfort?

To test the hypothesis that thinner rigid gas permeable (RGP) contact lenses provide superior initial comfort, RGP lenses of 3 center thicknesses, 0.08, 0.12, and 0.16 mm, in otherwise matched parameters, were worn for 30 min by 17 unadapted subjects in a controlled, double masked, randomized study. Comfort ratings (0 to 100) after 30 min of wear were 42 +/- 30, 55 +/- 27, and 57 +/- 28 for the 0.08-, 0.12-, and 0.16-mm thick lenses, respectively (p = 0.04, multiple analysis of variance (MANOVA)), and 93 +/- 13 for the hydrogel control lens. The thinnest RGP lens was significantly less comfortable than its thicker counterparts (p = 0.03, Univariate F-test). There were no differences among the test lenses in edge shape, front surface wettability, or static lens fittings, and small differences in movement, vertical decentration, and front surface geometry were not significantly related to comfort. We hypothesize that the greater flexibility of the thinnest lens resulted in greater deformation of the lens during a blink cycle, causing transient peripheral lens lifting and interaction with the upper eyelid, thereby reducing comfort. In conclusion we found that thinner RGP lenses do not provide an initial comfort advantage and that very thin and, as a corollary, very flexible, RGP lenses can actually be less comfortable initially than stiffer, but otherwise matched, designs.

Use of a new glass microfibre histamine release method to study the modulation of the host response in human schistosomiasis mansoni. Individuals with different degrees of exposure to the disease show differing antibody biological function.

A new glass microfibre histamine release method was used to study the modulation of the host response in human schistosomiasis mansoni to improve our understanding of the role of basophils in the development of immunity in schistosomiasis mansoni. The histamine release from umbilical cord blood basophils sensitized with sera from Sudanese individuals infected with Schistosoma mansoni was measured. Schistosomiasis sera (n = 113) were able to passively sensitize basophils and induce a positive histamine release in response to whole worm homogenate (WWH) (chi 2 = 40.5, P < 0.0001) and soluble egg antigen (SEA) (chi 2 = 16.3, P < 0.0001). However, worm antigen induced significantly higher histamine release in adults than egg antigen (Z = 4.83, P < 0.0001). Basophil cell sensitivity to WWH was inversely related to the intensity of infection. A correlation was observed between basophil cell sensitivity and IgE antibodies in response to WWH. Chronically infected canal cleaners (n = 16) showed a significant increase in basophil cell sensitivity 3 months after praziquantel treatment (Z = 1.73, P < 0.05). Normally exposed adults (n = 29) showed a significant decrease in basophil cell sensitivity 1 year after treatment. When serum fractions were used, chronically infected canal cleaners showed a significant increase in histamine release after IgG removal (P < 0.05) Comparison between the different study groups with regard to resistance and susceptibility to reinfection, their basophil sensitizing ability and antibody response showed a difference in the biological function of the IgE to WWH in the different groups. Direct histamine release results from S. mansoni infected individuals from outside the endemic area (Danish) showed the possibility of using the glass microfibre histamine release method in the diagnosis of schistosomiasis.

Relationship between Breteau and House indices and cases of dengue/dengue hemorrhagic fever in Kuala Lumpur, Malaysia.

The relationship between the Breteau index, the House index, and the occurrence of dengue/dengue hemorrhagic fever in the 6 zones of Kuala Lumpur was studied throughout 1994. Cases of dengue/dengue hemorrhagic fever varied between zones and between months, ranging from 0 to 21 cases. In most of the zones in Kuala Lumpur, the occurrence of dengue/dengue hemorrhagic fever has no relationship with the Breteau and House indices. Cases of dengue/dengue hemorrhagic fever occurred in all zones despite the low Breteau and House indices.

Determination of mineral, parathyroid hormone, and 6-keto-prostaglandin-F1 alpha levels in pregnant women with hypertension and pre-eclampsia.

Determinations of total calcium, total magnesium, calcium ion, parathyroid hormone and 6-keto-prostaglandin-F1 alpha levels were carried out on 84 blood samples from 4 groups of women categorised as non-pregnant normotensive (NNP), pregnant normotensive (NP), pregnancy-induced hypertension (PIH) and pre-eclampsia (PE). PIH was clinically diagnosed when the diastolic pressure was more than 90 mmHg and was only hypertensive during pregnancy while PE was with additional proteinuria after 20 weeks of gestation. Compared to NNP women, total calcium and parathyroid hormone levels were of lower levels (p < 0.05) in NP women while in PIH women, total calcium and 6-keto-prostaglandin-F1 alpha levels were also lowered (p < 0.05). Compared to NNP women, PE women's levels of total calcium, calcium ion and 6-keto-prostaglandin-F1 alpha decreased (p < 0.05) while parathyroid hormone level increased (p < 0.05). When compared to the NP women, PE women had decreased levels (p < 0.05) of total calcium as well as calcium ion and increased level (p < 0.05) of parathyroid hormone. Calcium ion was found to be negatively correlated (NNP : r = -0.883, p = 0.008/NP : r = -0.931, p = 0.000) while parathyroid hormone was positively correlated (NNP : r = 0.904, p = 0.013/NP : r = 0.913, p = 0.000) with mean arterial pressure.

Field studies on populations of Aedes albopictus and Toxorhynchites species in bamboo pots in Malaysia.

Between April 1987 and March 1988, populations of immature Aedes albopictus and Toxorhynchites spp. in bamboo pots were sampled weekly. Populations of Ae. albopictus and rainfall varied from month to month. During the heavy rainfall months of September and October 1987, larval counts of Ae. albopictus were high, between 30.8 and 49.2 larvae per week compared to 16 larvae per week during the low rainfall month of January 1988. A higher population of Toxorhynchites spp. was associated with a low population of the vector.

Evaluation of haematuria as an indirect screening test for schistosomiasis haematobium: a population-based study in the White Nile province, Sudan.

Haematuria elicited in the history, seen macroscopically or detected by reagent strips, was used as an indirect screening test for Schistosoma haematobium infection in Um-Hani Irrigation Scheme in the White Nile province, Sudan. These approaches were used separately or combined in different sequences. Reagent strips alone detected 81% of cases and 88% of those who excreted 50 egg/10 ml of urine or more. The sequence of observation of gross haematuria followed by screening with reagent strips and then taking history of haematuria had the highest sensitivity of all the orders, 0.87, and it saved 18% of reagent strips. If history and inspection were done first, followed by reagent strips, the sensitivity would be 0.86 and 47% of strips would be saved. The specificity of haematuria as a diagnostic index for schistosomiasis, however, was low.

Seasonal population patterns of Spalangia endius Walker (Hymenoptera: Chalcidoidea) at a refuse dumping ground in Malaysia.

A study on population patterns of the parasitoid Spalangia endius Walker at a dumping ground near Kuala Lumpur city showed that the percentage of S. endius adult emergence varied seasonally. During the relatively heavy rainfall months of August and November 1988, and January, March, and April 1989, the population of S. endius adult emergence were low (0-14.2%) compared to the less rainy months of July, September, and December 1988, and May 1989 (29.3-39.6%). This information could be useful in formulating strategies to reduce house fly population at the refuse dumping ground through integrated pest management programs.
