Typing of HLA antigens in a child population with nephrosis syndrome.

Using the microlymphocytotoxicity test of Terasaki, HLA--A, B, Cw antigen typing was done for 37 antigens in 28 children with steroid-sensitive nephrosis and five children with steroid-resistant nephrosis. Evaluation was made by the chi 2 test as corrected by the method of Yeates. In steroid-sensitive nephrosis the frequency of HLA--B 8 antigen was 33.3% and in the resistant cases 40% as against 18.5% in the controls. The difference, however, was not significant, the B 12 and B 13 antigens were more frequent in both steroid-sensitive and resistant nephrosis, though not significantly.

The effect of antidiuretic hormone, indomethacin and naproxen on prostaglandin synthesis of experimentally infected and healthy kidneys.

The authors studied the effect of indomethacin and naproxen on the changes of renal prostaglandin E and F2 alpha concentration in experimental kidney infection, as well as the action of arginine-vasopressin in healthy rats. Naproxen proved to be an effective inhibitor of prostaglandin synthesis, as did indomethacin. In control animals an increased prostaglandin E and F2 alpha synthesis was observed caused by arginine vasopressin. It is supposed that ADH--depending on its concentration--has a metabolic modulator role in prostaglandin synthesis, which raises the possibility of a self-regulatory mechanism of water reabsorption.

Indomethacin and gentamycin treatment in experimental kidney infection.

Experiences in experimental renal infection of rats treated with gentamycin, indomethacin, and by the combination of both drugs are reported. Urine osmolality of animals treated with combined therapy normalized after the first dose of indomethacin; their antibody titers against the pathogenic bacteria, as compared to the controls, decreased significantly. When drugs were administered separately the above immunologic changes could not be observed. Indomethacin did not inhibit the antibacterial effect of gentamycin and, administered alone, it increased the renal concentration ability.

Urinary osmolarity in late stage nephritis and nephrosis.

Urinary osmolarity was studied with the DDAVP test in 30 children in the late stage of glomerulonephritis or nephrosis syndrome, and in 12 control children. A restriction of the concentrating ability of the kidney was demonstrated in the cases accompanied by residual symptoms, in comparison to the healthy children and to the patients in lasting remission. In the cases examined histologically, tubular damage corresponding to the functional change could be demonstrated.

Plasma exchange and immunosuppressive therapy in a paediatric patient with systemic lupus erythematosus.

A case is reported of systemic lupus erythematosus (SLE) associated with steroid-resistant nephrotic syndrome. Serial plasma exchanges (PE) combined with prednisolone treatment induced complete normalization of the immunological findings: the anti-DNA antibody, antinuclear antibody, LE cell phenomenon and circulatory immune complexes became negative. The prostacyclin (PGI2) production-supporting activity in the plasma increased to the control range; inhibitors of PGI2 production were eliminated. The creatinine clearance normalized, the urinary protein excretion decreased significantly, and the facial erythema disappeared. Continued treatment with chlorambucil + low-dose prednisolone led to a complete and stable remission of the nephrotic syndrome, and the C3 complement normalized. The low level of PGI2 production-supporting activity in the plasma may be explained by the inhibitor of PGI2 production. PE + immunosuppressive therapy might have beneficial effects on the immunological changes and PGI2 metabolism, and also on the remission of SLE-nephrotic syndrome.
