Biloma during steroid therapy for minimal change nephrotic syndrome.

A 27-year-old man, who had been on steroid therapy for 2 months for his nephrotic syndrome, suddenly developed intra-abdominal bile collection (biloma). He had no previous history of abdominal surgery, trauma, or any disease of the hepatobiliary system. The cause of the biloma formation was due, probably, to cholecystitis in the absence of calculi and a pinhole size perforation in the wall of gall bladder. It was assumed to be closely related to the high-dose steroid therapy over a prolonged period, which would likely suppress the repair process of the locally damaged biliary system.

Chromosomal structure of the halophilic archaebacterium Halobacterium salinarium.

The chromosomal structure of the extremely halophilic archaebacterium Halobacterium salinarium was examined. Sheared chromosomes prepared from the bacteria in the late exponential phase were separated into two peaks (peaks I and II) by sucrose gradient centrifugation, suggesting that the chromosomes consist of two parts differing in quality. The UV spectra of peaks I and II resembled those of DNA and eukaryotic chromatin, respectively. Electron microscopic observations revealed that the major component of peak I was protein-free DNA, while the major components of peak II were rugged thick fibers with a diameter of 17 to 20 nm. The rugged fibers basically consisted of bacterial nucleosome-like structures composed of DNA and protein, as demonstrated in experiments with proteinase and nuclease digestion. Whole-mount electron microscopic observations of the chromosomes directly spread onto a water surface revealed a configuration in which the above-described regions were localized on a continuous DNA fiber. From these results it is concluded that the H. salinarium chromosome is composed of regions of protein-free DNA and DNA associated with nucleosome-like structures. Peaks I and II were predominant in the early exponential phase and stationary phase, respectively; therefore, the transition of the chromosome structure between non-protein-associated and protein-associated forms seems to be related to the bacterial growth phase.

Extended transverse rectus abdominis musculocutaneous flap.

Blood circulation within the conventional TRAM flap is not generous, and the contralateral random portion of the flap may result in fat or skin necrosis. However, this random portion can be extended safely and used for reconstruction by including the superficial epigastric vessels and the superficial circumflex iliac vessels and by anastomosing either of these to the recipient vessels. We have experienced this extended TRAM flap in two patients without any complications.

Use of the latissimus dorsi and the serratus anterior muscles as a combined flap.

Flaps composed of the latissimus dorsi and the serratus anterior muscles have been used to repair extensive defects in 10 patients with no remarkable disabilities of shoulder function. The latissimus dorsi and serratus anterior muscles are consistently nourished through the subscapular-thoracodorsal vessels. Thus, the 2 flaps can be based on 1 vascular pedicle. If required, the ribs beneath the serratus anterior muscle, which are vascularized by the periosteal circulation, can be transferred with the muscle. The vascular pedicle of this flap is long and anatomically reliable. Care must be taken to avoid tension or torsion of the pedicle when positioning the flap.

Eyelid reconstruction in pachydermoperiostosis.

Our experience with an unusual case of pachydermoperiostosis is described. This case was accompanied by enlargement of the eyelids, excessive secretions, and difficulty in opening the eyes. Partial and serial excisions of the upper and lower eyelids were done to correct the thickened eyelids. As a result of the operations, this patient can open his eyes fully and the secretions have decreased.

Experimental evaluation of vascular pedicle in island flaps.

This study was undertaken to evaluate the influence of variations in the vascular pedicle on survival of island flaps. Abdominal island flaps were raised in 128 rats based on the left superficial epigastric vessels. Our results show that stenosis of about one-third of the original external diameter of the artery and vein of the pedicle in our model did not have any significant influence on the survival of the flap and ligation of the femoral artery distal to the branch to the flap did not produce any statistical difference in the viability of the flap.

Influence of dryness of the vessel on its patency.

A controlled experimental study of microvascular surgery in rabbits showed that arteries can tolerate dryness for at least 5 hours and veins can tolerate dryness for at least 2 hours. In clinical free-tissue transfer, therefore, drying of the vessel during the usual process of anastomosis is not a major concern.

A new type of prosthesis for augmentation rhinoplasty: our experience in 1600 cases.

Study of the elasticity of the skin of the nose, the direction of muscle movement and the mobility of the nasal cartilages shows clearly that the human nose is not simply a lump of flesh consisting of bone, cartilage, muscle and skin, but a subtle aesthetic combination of anatomical form and physiological function. A new basal prosthesis has been introduced which avoids the drawbacks of a rigid prosthesis of uniform hardness. By incorporating silicones of varying hardness and using a new design we have a choice of three different prostheses which can fulfill many of the desired aesthetic and functional requirements of reconstructive rhinoplasty. This new type of prosthesis has now been used in 1600 cases with satisfying results.

Effects of Pentoxifylline on flap survival.

Based on our experimental studies in rats and our clinical experiences, Pentoxifylline appears to have a potential application in the treatment of a skin flap in danger of necrosis. We believe that improving the deformability of red blood cells by increasing ATP content is an important factor in flap survival.

Our experiences with the free latissimus dorsi myocutaneous flap.

We have used 9 free latissimus dorsi myocutaneous flaps. All but 1 survived completely. This one showed superficial necrosis at the flap margin. Our operative techniques and experiences are reported here, with some of our results. A relatively large flap can be nourished by only one vascular pedicle, the subcapsular-thoracodorsal vessels. Though three are many vascular variations in the axillary region, the subscapular-thoracodorsal vessels are easily identifiable in our experience, and their diameters are always large enough for microvascular anastomosis. The bulk of this flap is one of its important features. Care must be taken in the design of the flap so that it fits exactly, and it should also be remembered that the volume of the flap will decrease greatly later on. The flap is remarkably simple, safe, and reliable. We believe it will be the method of choice in many microsurgical cases.

Ribonucleic acid polymerase mutation affecting glutamate synthase activity in and sporulation of Bacillus subtilis.

Spore formation of 15 rifampin-resistant (Rifr) mutants of Bacillus subtilis strain 168 was examined. As a pleiotropic effect of a Rifr mutation, glutamate synthase activity was lost in these mutants. Twelve of the 15 mutants examined formed as many spores as the parent, but the remaining 3 formed significantly fewer (1%) spores. One of the latter mutants characterized further (RF301) was blocked in its sporulation process at stage 0. Thus, it was concluded that a certain modification of ribonucleic acid polymerase may affect specifically the gene expression of glutamate synthase and also the sporulation process at the initial stage.
