Interleukin-1 receptor antagonist gene polymorphism in patients with rheumatoid arthritis in India.

BACKGROUND & OBJECTIVES: Interleukin-1 receptor antagonist (IL-1Ra) is a naturally occurring anti-inflammatory molecule that blocks action of IL-1. Polymorphism in IL-1Ra gene intron 2 results in differences in production of IL-1Ra. These polymorphisms are reportedly associated with autoimmune disease susceptibility in different studies. However, such data are lacking from India. We undertook this study to examine the IL-1Ra polymorphism as a susceptibility marker in patients with rheumatoid arthritis (RA). METHODS: DNA samples from 107 patients with RA and 111 healthy controls were used to study genotypes of the IL-1RA gene by PCR. Allelic frequencies and carriage rates were calculated and compared in both the groups. RESULTS: Among the 107 patients with RA, 93 were females and 75 per cent were seropositive for rheumatoid factor. The frequencies of IL-1RA alleles in controls were as follows: Allele 1 (IL- 1RN*1) was 83.33 per cent, IL-1RA allele 2 (IL-1RN*2) was 16.21 per cent and allele 3 (0.46%). In RA patients the allele frequencies were 84.11 per cent for IL-1RN*1, 14.95 per cent for IL- 1RN*2, 0.47 per cent each for IL-1RN*3 and IL-1RN*4. There was no difference in frequency of different alleles between the two groups. However, homozygosity for allele 2 was more frequent in controls (9.91%) as compared to patients (4.67%). INTERPRETATION & CONCLUSION: Our findings indicated that IL-1RA polymorphism was not a susceptibility marker in RA nor did it show any association with seropositivity, Sjögren's syndrome or subcutaneous nodules. Further studies with large sample need to be done to confirm these findings.

Suture foreign body granuloma masquerading as renal neoplasm.

Foreign body granulomas and pseudotumors due to retained surgical linen are well known in surgical practice. These lesions usually correspond to the actual size of residual foreign body and have characteristic presentation according to the anatomy involved. Renal suture granuloma is a rare postoperative complication of renal surgery due to persistence of sutures used to close the pelvicalyceal system/nephrotomy incisions and usually present as incidentally detected small mass lesions. This case of a suture foreign body granuloma presenting with hematuria, large peripheral mass lesion and characteristic computed tomography picture of renal cell carcinoma confounded the diagnosis and underwent laparoscopic radical nephrectomy. In retrospect, such lesions warrant the use of selective needle biopsy and intraoperative frozen section confirmation to clinch diagnosis. Mass lesions occurring in a previously operated kidney should have granuloma as a differential diagnosis.

Extending pedicle length in free jejunal loop grafts.

By mobilisation of the jejunal marginal artery with reverse flow, the authors present a novel way of increasing artery length available for anastomosis in free jejunal loop transfer for reconstruction in head and neck cancer surgery.

Primiparity: a risk factor for brachial plexus injury in the presence of shoulder dystocia?

The objective of the study was to assess the risk factors associated with obstetric brachial plexus injury. It was a retrospective analysis over a 7-year period, of women whose labours were either complicated by shoulder dystocia or had neonates who sustained brachial plexus injury. The 133 women included were divided into two groups: (1) Non-brachial plexus injury (Non-BPI) group: 106 women with labours complicated by shoulder dystocia. (2) Brachial plexus injury (BPI) group: 27 women whose neonates sustained BPI. Comparison of ante-partum, intra-partum and post-partum factors was done. In the BPI group, there were significantly more nulliparous women, with more use of oxytocin. The neonatal variables were similar in both groups. Mean birth weight was more than 4 kg in both groups. In the presence of similar neonatal variables, brachial plexus injury is more likely to occur in neonates of primiparous women in the presence of shoulder dystocia, if labour is accelerated.

Blood loss estimation in epistaxis scenarios.

Thirty-two members of staff from the Ear, Nose and Throat Department at Warrington General Hospital were asked to estimate blood loss in commonly encountered epistaxis scenarios. Results showed that once the measured volume was above 100 ml, visual estimation became grossly inaccurate. Comparison of medical and non-medical staff showed under-estimation was more marked in the non-medical group. Comparison of doctors versus nurses showed no difference in estimation, and no difference was found between grades of staff.

Intraoral papillary endothelial hyperplasia: case discussion with supportive histochemistry and immunohistochemistry.

An intraoral mass of eight months duration in a six year-old girl was diagnosed as papillary endothelial hyperplasia. Histologically, the tissue was characterized by the unusual endothelial cell proliferation, which is significant, as papillary endothelial hyperplasia resembles angiosarcoma and possible over-treatment thereafter. Clarification of this unusual lesion based on histological findings with supportive histochemical staining and immunohistochemistry in the light of clinico-pathological correlation is discussed. This lesion warrants better documentation of the clinical behavior with regular monitoring.

Prolonged feeding practice and its effects on developing dentition.

Muscular activity of the lips, cheeks, and the tongue are important factors, which not only guide the occlusal development but also affect the growth of the jaws. Bottle feeding has been known to cause a myriad of changes in the oro-facial development. But the adverse effects of feeding practices with respect to developing dentition has been a subject of controversy. Hence a study was designed with the aim and objectives to check the commonly adopted feeding practices in South Kanara, to compare the occlusion of 3- to 5-year-old children with breast feeding, bottle feeding or combination, and to check whether prolonged feeding has any adverse influence on developing dentition. A total of 250 questionnaires were distributed to parents of 3- to 5-year-old children of which 153 were chosen for recording the impression and for studying the occlusion under various parameters. The results of the study indicated definite potential hazards of prolonged breast and bottle feeding especially with a tendency to develop openbite and posterior crossbite. It also suggested that an early preventive and interceptive approach.

Holmium laser core-through urethrotomy for traumatic obliterative strictures of urethra: initial experience.

OBJECTIVES: To determine the safety and efficacy of holmium laser core-through urethrotomy (HolCTU) for the treatment of post-traumatic obliterative stricture of the urethra. METHODS: From June 2002 to March 2003, 29 men underwent the core-through procedure. All the patients had obliterative strictures of the bulbomembranous urethra. The length of the stricture was 2.5 cm or less, with good alignment between the two urethral ends. The holmium laser was used to vaporize the obliterative fibrotic tissue. The procedure was guided by a metal sound introduced through the suprapubic tract. Catheter removal and voiding cystourethrography were done at 6 weeks after the procedure and urethroscopy 1 month after catheter removal. RESULTS: At a mean follow-up of 15 months (range 10 to 19), the results were excellent in 19 (65.51%) of 29 patients. Nine (31.03%) had acceptable results; these patients failed to maintain good flow after HolCTU and required internal urethrotomy/endoscopic dilation once or twice for stabilization (maximal flow rate 15 mL/s or greater). Failure was seen in 1 patient (3.44%) who had recurrent obstruction despite repeated internal urethrotomy and finally required formal transpubic urethroplasty. The mean operating time was 40 minutes (range 30 to 90), with mean hospital stay of 8 hours (range 6 to 48). No perioperative complications occurred, except in 1 patient who developed small extravasation of fluid locally. All the patients were continent, and potency status was unaffected by HolCTU. CONCLUSIONS: HolCTU is a safe and simple treatment for obliterative urethral strictures. The procedure is less morbid than open repair with a slightly greater failure rate.

A systematic approach to the nasendoscopic examination of the larynx and pharynx.

Several nasendoscopic techniques have been described to improve the view of different anatomical subsites when assessing the laryngopharynx. A prospective study was undertaken to compare and evaluate the views obtained with each of the different techniques employed in the nasendoscopic examination of the upper aero-digestive tract. No visualization manoeuvres were found to be beneficial in improving the view of the tongue base; however, tongue protrusion did improve the view of the valleculae. Both the post-cricoid and the upper oesophageal sphincter were best seen when the trumpet manoeuvre was performed along with skin traction of the anterior neck. Pyriform apices were best viewed when the trumpet manoeuvre was coupled with head turn - a manoeuvre we believe not to have been recorded in the literature previously with respect to this topic. The authors present a systematic routine for examining the upper aero-digestive tract by nasendoscope on the basis of obtaining the best view for each anatomical subsite.

A randomized trial of local anaesthetic in treatment of quinsy.

A prospective randomized trial was undertaken in 30 adult patients with quinsy to determine the most efficacious method of local anaesthetic (LA) for aspiration, and incision and drainage of the abscess. Patients were randomized to receive either topical spray or spray and infiltration over the site of the collection. Pain was assessed on a visual analogue scale. Our results show, following analysis with Mann-Whitney U-test, a statistically significant lower mean pain scores for the infiltration group compared with the spray group during aspiration (P < 0.001), incision and drainage (P < 0.001), 30 min after incision (P < 0.01) and cumulative results during treatment (unpaired t-test P < 0.001). The authors recommend that infiltration be the method of choice of LA in aspiration and incision of peri-tonsillar abscess.

Dorsal buccal mucosal graft urethroplasty by a ventral sagittal urethrotomy and minimal-access perineal approach for anterior urethral stricture.

OBJECTIVE: To present the technique of dorsal buccal mucosal graft urethroplasty through a ventral sagittal urethrotomy and minimal access perineal approach for anterior urethral stricture. PATIENTS AND METHODS: From July 2001 to December 2002, 12 patients with a long anterior urethral stricture had the anterior urethra reconstructed, using a one-stage urethroplasty with a dorsal onlay buccal mucosal graft through a ventral sagittal urethrotomy. The urethra was approached via a small perineal incision irrespective of the site and length of the stricture. The penis was everted through the perineal wound. No urethral dissection was used on laterally or dorsally, so as not to jeopardize the blood supply. RESULTS: The mean (range) length of the stricture was 5 (3-16) cm and the follow-up 12 (10-16) months. The results were good in 11 of the 12 patients. One patient developed a stricture at the proximal anastomotic site and required optical internal urethrotomy. CONCLUSION: Dorsal buccal mucosal graft urethroplasty via a minimal access perineal approach is a simple technique with a good surgical outcome; it does not require urethral dissection and mobilization and hence preserves the blood supply.

Comparative evaluation of clinical efficacy of gallium and amalgam alloys in primary molars.

The present study was conducted to compare the clinical efficacy (marginal integrity and bulk fracture) of two alloys--Galloy (gallium based) and Aristaloy (amalgam based) in primary molars. 90 primary molars with initial caries formation were selected. 45 were restored with Galloy and 45 with Aristaloy in Class I and II preparations. They were evaluated at two-month intervals for 6 months. The results showed that Galloy performed at par with Aristaloy with regards to marginal integrity and bulk fracture. Hence Galloy can be considered as an interesting substitute in Pedodontic practice.

Maxillo- mandibular relationship--a cephalometric study.

An accurate assessment of apical base relationships using cephalometrics has been a perennial problem in orthodontics. The currently available literature suggests that reference points and planes are subject to change with treatment. Hence, an assessment of sagittal jaw relationship was done using five angular measurements and an attempt was made to determine the one which is statistically more accurate.

Endothelium-dependent brachial artery flow mediated vasodilatation in patients with diabetes mellitus with and without coronary artery disease.

AIM OF THE STUDY: Endothelial function as assessed by brachial artery flow mediated vasodilatation (FMD) has been shown to be impaired in patients with coronary artery disease (CAD). Since diabetes mellitus (DM) has been considered to be CAD risk-equivalent, we sought to determine whether endothelial dysfunction is present in patients with DM independent of presence of CAD. METHODOLOGY: One hundred and ninety eight individuals were included in the study and divided into four groups: Group 1--patients with risk factors for CAD, but no DM or CAD; Group 2--patients with DM but no CAD; Group 3--patients with CAD but no DM and Group 4--patients with both DM and CAD. Brachial artery FMD assessment was performed once in all subjects and FMD was calculated as percentage increase in brachial artery diameter in response to increase in brachial artery flow. RESULTS: Mean FMD was significantly higher in Group 1 (7.03 +/- 2.87%) compared to the other three groups. Mean FMD in Group 2 (5.51 +/- 2.12%) was similar to that in Group 3 (4.56 +/- 2.70%; p value 0.195) but significantly higher than that in Group 4 (4.26 +/- 1.93%; p value 0.038). There was no statistically significant difference in mean FMD in Group 3 and Group 4 (p value 0.65). CONCLUSION: Endothelial function as assessed by FMD is significantly impaired in diabetics compared to non-diabetics in absence of CAD. In addition, similar degree of impairment in endothelial function is seen in diabetics without CAD and non-diabetic patients having CAD, implying CAD risk-equivalence of diabetes.

Neurophysiology of pain: insight to orofacial pain.

This is a very exciting time in the field of pain research. Major advances are made at every level of analysis from development to neural plasticity in the adult and from the transduction of a noxious stimulus in a primary afferent neuron to the impact of this stimulus on cortical circuitry. The molecular identity of nociceptors, their stimulus transduction processes and the ion channels involved in the generation, modulation and propagation of action potentials along the axons in which these nociceptors are present are being vigorously perused. Similarly tremendous progress has occurred in the identification of the receptors, transmitters, second messenger systems, transcription factors, and signaling molecules underlying the neural plasticity observed in the spinal cord and brainstem after tissue or nerve injury. With recent insight into the pharmacology of different neural circuits, the importance of descending modulatory systems in the response of the nervous system to persistent pain after injury is being reevaluated. Finally, imaging studies revealed that information about tissue damage is distributed at multiple forebrain sites involved in attentional, motivational, and cognitive aspects of the pain experience.
