[Fibroblastic rheumatism of the elderly. Histopathological, ultrastructural data and problems of differential diagnosis].

In order to contribute to the knowledge and the nosography of fibroblastic rheumatism, the authors report a case occurring in 68 years old patient. The differential diagnosis of the disease rests on histological and ultrastructural aspects of the dermal and articular tissues where numerous myofibroblasts are observed. On that score, among the acquired adult fibromatoses, fibroblastic rheumatism seems closer to Dupuytren's disease than to sclerodermis.

[Value of synovial biopsy in the diagnosis of arthropathies associated with hyperlipoproteinemias].

In two patients suffering from hyperlipoproteinemias (HLP) type II and also having arthralgias, a biopsy of the synovial membrane of the knees was done. In both cases sodium urate crystals were found. On the basis of this data, the authors emphasize the need for systematic use of biopsy of the synovial membrane in all cases of HLP with articular manifestations. In this way, it was possible to establish the nosographic autonomy of arthropathies combined with HLP.

Psychological aspects of rheumatoid arthritis vis-à-vis osteoarthrosis.

Using objective techniques (M.M.P.I; M.H.P.A.; ANAM; S.T.A.I.), a transverse investigation of two groups of arthritic patients (35 rheumatoid arthritis and 30 oesteoarthrosis) was carried out. The Rheumatoid Arthritis (RA) patients evidence greater depression (p less than 0.02), while the osteoarthrosis (OA) patients had higher levels of manifest aggressivity (p less than 0.01) and more somatic preoccupation (p less than 0.02). The RA group had a higher incidence of loss of a family member as a preceding life event (p less than 0.05), and a greater incidence of psychopathological problems in infancy (p less than 0.05). These findings were used to construct an interpretative hypothetical model which represents a starting point and a stimulus for future research.

[Chyliform effusions in rheumatoid arthritis. Cytochemical and ultrastructural studies of the foam cells].

The authors studied the cell composition and the cytochemistry and ultrastructure of the frothy cells in two periarticular chyliform effusions in rheumatoid arthritis. Compared with the polymorphonuclear cells, the mononuclear cells were very numerous with many frothy cells. Cholesterol crystals, identified by X-ray diffraction, were detected in fluids by polarised light, but only cytochemically and with the transmission electron microscope in the spumous cells. The latter contained not only neutral fat, but also hydrophobic lipids and phospholipids, but no cholesterol esters. A theory has been advanced that once phagocytosis has been accomplished, cholesterol crystal metabolism is not rapid and is too insufficient for cholesterol or its esters to accumulate in the cytoplasm. The polymorphonuclear leukocytes play only a small role in the phagocytosis of crystals and this probably plays a role in the absence of acute inflammation in structures which contain chyliform effusions.
