[Endoscopic bile duct drainage--technique].

The authors describe the technique of endoscopic implantation of endoprosthesis in the bile duct. From 1978 to 1989 implantation of endoprosthesis was successful in 88.7% of 212 patients. One to three procedures per patient were required for primary implantation. 407 procedures were performed; primary implantation, replacement of endoprosthesis and implantation of additional endoprosthesis. The success rate increased from 55% in the first third of the period to 82.4% in the last third. Ten patients had been previously operated with Billroth II gastrectomy and one with Whipple's operation. Implantation was successful in seven of these patients.

Bile Ducts↗

[Endoscopic bile duct drainage--results].

212 patients were treated with endoscopic endoprosthetic drainage for bile duct obstruction. 137 patients had inoperable malignant obstruction, ten operable malignant tumor, 35 bile duct stones and 30 various benign strictures. Endoprosthetic stenting was successful in 188 patients (88.7%). The remaining 24 patients were treated with percutaneous transhepatic drainage or biliaryintestinal anastomosis, or received no treatment. In patients with successful implantation of endoprostheses, jaundice was relieved in 81% of the cases and pain in 89.6%. Cholangitis was treated with good results in 93.5%. Biliocutanous fistulas closed in two patients. Patency of the endoprostheses was 87 days on average. Replacement of endoprostheses or insertion of additional endoprostheses was performed 130 times. Complications occurred in 52 of 407 procedures (12.6%). Mortality related to the endoscopic endoprosthetic treatment was 4.2%.

[Endoscopic retrograde cholangio-pancreatography in suspected acute gallstone pancreatitis].

During the 5-year-period from July 1983 to July 1988, ERCP was performed within 48 hours after onset of clinical symptoms in 63 patients with suspected gallstone pancreatitis. In 53 patients this etiology was confirmed, and acute endoscopic papillotomy was done in 30 of these patients. ERCP seems to be a safe diagnostic procedure in acute gallstone pancreatitis, and papillotomy gives effective drainage and relief of the acute symptoms.

Acute Disease↗

[Common bile duct calculi after biliary surgery. Postoperative endoscopic papillotomy is the solution].

In 37 patients with retained bile duct stones after operation using a T-tube, endoscopic papillotomy (EPT) was used as the therapeutic procedure. In 27 patients the stones were successfully removed in one session. Eight patients needed two to six endoscopic attempts before the stones could be removed. In two patients we failed to solve the problem endoscopically; in both patients we could remove the stone through the T-tube channel using a Dormia basket. The endoscopic method of removing retained bile duct stones is safe and quick. Most of the patients can be discharged from the hospital the following day.

Routine double contrast barium enema and fiberoptic colonoscopy in the diagnosis of colorectal carcinoma.

The diagnostic efficacy of routine double-contrast barium enema and fiberoptic coloscopy for detection of cancer was retrospectively studied in 303 patients consecutively operated for colorectal adenocarcinoma in a 52-month period from January 1980. Double-contrast barium enema was performed in 180 patients with 184 carcinomas, 157 (85%) of which were revealed by this examination. The detection rate of carcinoma according to site was 89% between cecum and descending colon, 92% in sigmoid colon and 71% in rectum. Fiberoptic colonoscopy was done in 176 patients with 181 carcinomas, 163 (90%) of which were detected by the endoscopical examination. The detection rate of carcinoma according to site was 86% between cecum and descending colon, 90% in sigmoid colon and 95% in rectum. The two methods were equally effective in detecting carcinoma of the colon. Colonoscopy was superior to barium enema in detection of rectal carcinoma.
