Effect of a patient management system on outcomes of total hip and knee arthroplasty.

Five hundred fifty-three patients undergoing hip and knee reconstructive procedures in one institution that used a patient management system were compared with a retrospective group of 340 patients undergoing similar procedures in the same institution. All procedures were performed by one surgeon and the same patient management team. Measures of length of stay, discharge disposition, and hospital charges were recorded for all patients in each subgroup of total hip arthroplasty, revision total hip arthroplasty, total knee arthroplasty, revision total knee arthroplasty, unicompartmental knee arthroplasty, and bilateral procedures. The length of stay and hospital charges were reduced significantly in all groups, whereas the percentage of patients discharged to home was unchanged. There was no significant difference in complication rates between the two groups.

A human recombinant haemoglobin designed for use as a blood substitute.

The need to develop a blood substitute is now urgent because of the increasing concern over blood-transmitted viral and bacterial pathogens. Cell-free haemoglobin solutions and human haemoglobin synthesized in Escherichia coli and Saccharomyces cerevisiae have been investigated as potential oxygen-carrying substitutes for red blood cells. But these haemoglobins cannot be used as a blood substitute because (1) the oxygen affinity in the absence of 2,3-bisphosphoglycerate is too high to allow unloading of enough oxygen in the tissues, and (2) they dissociate into alpha beta dimers that are cleared rapidly by renal filtration, which can result in long-term kidney damage. We have produced a human haemoglobin using an expression vector containing one gene encoding a mutant beta-globin with decreased oxygen affinity and one duplicated, tandemly fused alpha-globin gene. Fusion of the two alpha-globin subunits increases the half-life of this haemoglobin molecule in vivo by preventing its dissociation into alpha beta dimers and therefore also eliminates renal toxicity.

Sensitivity for Telemed diagnosis of myocardial infarction by use of 12-lead electrocardiogram derived from Frank XYZ leads.

Another comparison of sensitivity for diagnosis of myocardial infarction in conventional and derived 12-lead ECGs processed by the computer diagnostic program of the Telemed Cardio-Pulmonary System is reasonably consistent with the original trial of these two methods of recording ECG signals reported earlier by Dower and Machado. Other similarities and disparities are cited, together with brief comments about the theoretical and clinical implications of those findings and the relationship to a more rigorous analysis from this laboratory.

Rubella-vaccinated students. Follow-up in a public school system.

In a 7 1/2-year follow-up evaluation of the duration of rubella-vaccine-induced immunity of students who received either HPV-77 DK-12 or Cendehill vaccine, both groups showed a continous decline in hemagglutination-inhibition antibody from seven weeks after vaccination but a lower decline between 4 1/2 and 7 1/2 years after vaccination. However, at 7 1/2 years only 16 students (8%) receiving the Cendehill vaccine and one student (0.5%) receiving the HPV-77 DK-12 vaccine lacked detectable antibody. Despite the persistence of antibody titers, there was evidence of subclinical rubella among both groups of vaccinated students. These results emphasize the importance of continued evaluation of the conditions of persons receiving rubella vaccine.

Pseudovitelliform macular degeneration.

Three patients with vitelliform-like macular lesions had normal electroooculogram (EOG) light-peak/dark-trough ratios, unlike typical cases of vitelliform dystrophy (Best macular dystrophy). Leakage of fluorescein dye from perifoveal cappillaries implicated an increased permeability of these vessels as the probable cause for the vitelliform-appearing lesions. All patients with vitelliform lesions demonstrating normal EOG ratios should have an evaluation by fluorescein angiography to rule out a diagnosis of what we prefer to call pseudovitelliform macular degeneration.

Evaluation of influenza A/New Jersey/76 split-product virus vaccine in a summer camp.

An evaluation of two doses of split-product A/New Jersey/76 virus vaccines was conducted at a boys' summer camp. Two doses of either 200 chick cell-agglutinating (CCA) or 400 CCA units of vaccine were administered four weeks apart. There were no significant side effects. Of 60 subjects, 59 developed a significant titer of serum antibody after two doses. There was no correlation between titer of antibody and total dose of vaccine. Younger subjects developed a higher titer than older subjects. A single dose of vaccine was insufficient to achieve a protective level of antibody.

Elevated arterial pressure and postexertional ST-segment depression in middle-aged women.

Of ten healthy women, 43 to 61 years of age, four exhibited 1 mm. or more of upsloping ST-segment depression after maximal exercise, using the Bruce multistage treadmill protocol; the other six did not. Cardiac output (direct Fick) was not different in the two groups, wither at rest or during exercise. The women with ST-positive responses were older (56 vs. 51 years) and heavier (relative weights 109 per cent vs. 102 per cent), and their resting mean systemic pressures were higher. Their systemic and pulmonary mean arterial pressures during the last five minutes of upright exercise were significantly higher (P less than 0.001) than those in the ST-negative group. Although the ratio of systemic to pulmonary mean arterial pressures was higher at rest, it progressively fell during exercise in the ST-positive group. Polarcardiographic display of the Frank ECG during the first 3 minutes of recovery after maximal exercise showed significant differences between the ST and T of the two groups. At initial recovery, MS-phi greatly exceeded 10.6 mV. which is sensitive PCG ischemic exercise criterion in the ST-positive group. Although the number of observations is limited, it is concluded that greater hemodynamic stress imposed on the subendocardium by elevated pressures, rather than by any significant functional evidence of restriction in coronary blood flow, probably explains much of the postexertional ST-segment depression after maximal exercise.

Polarcardiographic criteria for myocardial infarction in Chinese men.

Polarcardiograms (PCG) derived from xyz leads of the Frank electrocardiogram (ECG) were recorded in the supine position in 1264 initially healthy middle-aged Chinese men who had been under continuing medical surveillance and were re-examined seven years later. Polarcardiographic criteria for myocardial infarction (MI) were demonstrated in 97 men (7.7%), but only 15 of them showed diagnostic Q waves in the electrocardiogram (ECG). Polarcardiographic criteria for MI were independent of age, but ranged from 4.3% in 464 apparently normal men to 38% in 21 men with manifestations of ischemic heart disease. The possible association of polarcardiographic criteria and a history of smoking was limited to those with clinical evidence of heart disease. The "false positive" rate of 4.3% in clinically normal men was similar to that reported in younger Canadians and in Cretan population samples.

Low-level dynamic exercises for earlier cardiac rehabilitation: aerobic and hemodynamic responses.

Three different kinds of low-level exercise (one arm and two leg exercises) and a low-level treadmill test were designed for patients recovering from acute myocardial infarction; noninvasive circulatory and aerobic responses to these activities were evaluated in healthy subjects. Exercise systolic pressure-rate product (X 10(-2)) were, respectively, 92 +/- 16, 98 +/- 10, 106 +/- 22 and 129 +/- 17 for the four activities studied. Exercise metabolic rates were 1.56 +/- 0.16, 1.99 +/- 0.22, 1.68 +/- 0.25 and 2.74 +/- 0.32 METS (multiples of the observed resting oxygen requirements). Additionally, arteriovenous oxygen difference, cardiac output, stroke volume, mean systemic and mean pulmonary pressures were measured in four coronary patients as they performed the leg exercises. It is hoped that this study will provide a basis for applying these different forms of exercise to patients recovering from an acute myocardial infarction, even when they are confined to bed, in order to advance the onset of cardiac rehabilitation. In a similar way, the new low-level treadmill test may be indicated for the same patients to assess the safety of discharging them from the hospital and permitting unsupervised ambulatory activity at home.
