[Recurrent and peptic ulcers of anastomosis after vagotomy].

The results of surgical treatment in more than 700 patients clinical course and methods of diagnosis of recurrent and peptic ulcers of the duodenum after vagotomy are described. Some features of the course of peptic and recurrent ulcers of the duodenum depend on enzyme and acid producing function of the denervated stomach. Endoscopic examination has proved to be an effective method of diagnosis, as well as examination of the secretory function of the organs roengenological examination of the bowels. The patients of the "risk group" were singled out who needed prolonged follow-up or periodic treatment.

[Sorption in the treatment of acute peritonitis (experimental study)].

The data on the effective use of the new sorbent KAY and the carbon sorbent "Volna" in the treatment of acute generalized peritonitis were compared. Endosorption with sorbent KAY was conducted on an experimental model of generalized peritonitis. It produced a favorable effect on the functional morphology of the peritoneum by leveling many morphological signs of general intoxication of the organism and in this way creating the conditions for a favorable outcome of the postoperative period. The studies showed that the use of sorbent KAY in the treatment of acute generalized peritonitis facilitates active restoration of the function of vital organs, significantly raises the efficacy of treatment, and shortens the period of in-patient treatment.

[An improvement in the combined treatment of acute peritonitis].

The object of the research was to study regional lymph circulation in abdominal organs in experimental acute peritonitis and determination of the possibility of including lymphotropic therapy in the complex of measures for the management of patients with acute peritonitis. Regional lymph circulation in the abdominal organs was studied in 45 unbred rats, the effect of various lymphostimulators of regional lymph circulation was studied in 35 rats. Lymphotropic therapy was applied in the complex treatment of 106 patients with acute peritonitis in the clinic. The results of the study showed that lymph circulation in the abdominal organs is markedly disturbed in acute generalized peritonitis, in view of which application of lymph therapy in the complex treatment of patients in the postoperative period is pathogenetically substantiated. Lymph therapy improves microcirculation, promotes early restoration of gastrointestinal activity, reduces the expenditure of antibiotics to one half and one fourth, and shortens the term of inpatient treatment.

[Vascular structure and the microcirculatory bed of the stomach after selective proximal vagotomy].

The authors conducted experiments on 30 dogs for periods of 1-7 days to 12 months and established that a sharp disorder of microcirculation occurs in the postoperative period, particularly in the cardia and fundus of the stomach, due to denervation and devascularization which lead to stasis in the venous system, hypoxia, and ischemia. The microcirculatory disorders are restored in 6 to 12 months. The study showed that the use of selective proximal vagotomy in true gastric ulcers is not expedient.

[Diagnosis and treatment of recurrent peptic ulcers after vagotomy].

Under analysis were case histories of 41 patients with recurrent ulcer. An effective method of diagnostics is the endoscopic examination with an additional X-ray examination of the stomach and duodenum and secretory function of the organ. Gastric resection which is supplemented with selective gastric vagotomy depending on the state of gastric secretory function is the main method of surgical treatment of recurrent and peptic ulcers after vagotomy.

[Regional lymphotropic therapy].

The article deals with the results of experimental studies on 10 male and female dogs conducted under intrapleural hexenal anaesthesia. The rate of elimination of the lymphotrophic dye indigo carmine from the subserous depot in the wall of the gallbladder, stomach, and small intestine under normal conditions and in regional stimulation of lymphatic drainage was studied. The rate of total elimination increased 2.4 times in stimulation. On the grounds of these studies, regional lymphotrophic therapy was conducted in 417 patients treated at a general surgical hospital. As the result of this treatment the inflammatory process was arrested in a shorter time, the laboratory indices were normalized more rapidly, and the term of treatment was shorter than in patients of the control group.

[Effects of vagotomy on the lymphatic system of the small intestine].

Vagotomy in experiments induces compensatory dilatation of the lymph vessels of the small intestine, which is directed at correction of the microcirculatory shifts encountered in the postoperative period in the form of venous stasis and edema of the wall of the small intestine. Vagotomy leads to slowing down and terminal shift of digestion and exclusion of the duodenal lymph vessels from the process of absorption and transportation of fat. The restoration of the morphology and function of the lymph vessels in the late postoperative period is determined by the type of vagotomy. Microcirculation and the state of the lymphatic system improve more rapidly and fully after selective proximal vagotomy.
