Immunoreactivity for S-100 protein in dendritic and in lymphocyte-like cells in human lymphoid tissues.

S-100 protein is an immunohistochemical marker for a subset of dendritic cells, the interdigitating reticulum cells (IDRCs), which are mainly located in T-dependent areas of lymphoid tissues. In the present study we have investigated the distribution of S-100-positive cells in lymph nodes, spleen, thymus and peripheral blood of normal subjects. Immunoreactivity for S-100 protein was demonstrated in large cells with dendritic morphology and in small lymphocyte-like cells present in the lymph node paracortex, thymic medulla, splenic periarterial lymphatic sheaths (PALS) and in peripheral blood. S-100-positive lymphocyte-like cells were frequently detected around high endothelial venules (HEV) and were present in numbers comparable to those of S-100-positive IDRCs. Immunoelectron microscopy confirmed the existence of positive cells with lymphoid morphology and revealed that the intracellular distribution of the immunoreaction product was similar in lymphoid and dendritic cells. Further characterization of S-100-positive cells demonstrated that both lymphoid and dendritic cells were unreactive with a large panel of monocytic and macrophage markers.

The Omenn's syndrome: histological, immunohistochemical and ultrastructural evidence for a partial T cell deficiency evolving in an abnormal proliferation of T lymphocytes and S-100 +/T-6 + Langerhans-like cells.

A 7 month old female infant was affected by a rapidly fatal familial disease highly reminiscent of Omenn's syndrome. She presented with widespread eczematous lesions, hepatosplenomegaly, superficial lymphadenopathy, peripheral blood lymphocytosis, eosinophilia and hyper-IgE. An axillary lymph node was involved by a marked proliferation of T-3 +/T-10-- lymphocytes admixed with S-100+/T-6+/Leu-3a+/Ia + reticular cells which lacked typical LC granules; cell suspension study revealed that 90%-96% of the lymph node cells were T-11+/T-3+ lymphocytes characterized by low expression of Leu-3a and T-8 antigens and by high expression of Ia antigens (52%). Peripheral blood T lymphocytes exhibited a similar distribution of surface phenotypes. The patient died of interstitial pneumonia and an autopsy was performed. The thymus was markedly atrophic and completely devoid of lymphocytes. The peri-arteriolar lymphoid sheets of the spleen were poorly developed and were mainly composed of T-8+ lymphocytes. The mediastinal nodes were rudimentary and were populated by T-3+/T-10+ lymphocytes with low expression of Leu-3a and T-8 antigens. Our results raise the possibility that Omenn's syndrome is a peculiar primary immunodeficiency in which, despite early thymic involution, some abnormal T lymphocytes still develop in the peripheral lymphoid organs. Antigenic triggering of these cells might result in prominent proliferations of T lymphocytes and Langerhans-like cells which lead to the clinical manifestation of the disease.

Lectin I of Ulex europaeus as a marker for a subset of histiocytic tumours of the lymph node.

We describe four lymph node based tumours in which numerous neoplastic cells and some mitotic figures were characterized by staining affinity for Lectin I of Ulex europaeus (UEA-I). The patients had no vascular or epithelial tumours and presented symptoms suggestive of a systemic lymphoproliferative disease. Histologically, the tumours were composed of large, cohesive, cells which were mainly located in the paracortex. UEA-I reactivity was more evident in the Golgi area and was present in large mononucleated cells often arranged to delimit vascular-like spaces. The neoplastic cells were weakly muramidase-positive in one case, and were ANAE+/AP+ in two other cases. Large dots of UEA-I reactivity were detected in S-100+/muramidase-negative Langerhans-like cells present in one case of Letterer-Siwe disease. UEA-I staining was consistently negative in 20 cases of B cell- or T cell lymphoma and in 9 other cases of histiocytic lymphoma. It is suggested that UEA-I+ tumours of the lymph nodes are part of a distinct subset of histiocytic malignancies whose neoplastic cells present some morphological and phenotypic properties normally associated with endothelial cells.

Peripheral T cell lymphoma in adults: morphological and phenotypical study of four cases.

In the present study we investigated the lymph node morphology and distribution of cell surface phenotypes in four cases of adult peripheral T cell lymphoma. Histologically, the tumors were classified as T zone lymphoma, T cell lymphoma with large multilobated nuclei and T cell immunoblastic sarcoma. In the T zone lymphoma the neoplastic lymphocytes were E+ (90%) and exhibited intensive focal staining for acid phosphatase (93%) and acid esterase (92%); the phenotype distribution revealed low expression of the T-3 antigen (49%), selective expression of the T-4 antigen (72%) and poor expression of T-6 (10%) and T-10 antigens (22%). Some of these features are present in normal and in neoplastic immature T cells. In the remaining three cases the majority of lymph node cells were E+ (59-75%), T-3+ (67-80%) and T-8+ (43-55%). A distinctive feature of the T cell immunoblastic sarcoma was the presence of high percentages of DR+ cells (62%; 63%). Thus our results indicate that the morphological heterogeneity of peripheral T cell lymphoma is also paralleled by a variety of surface phenotypes and that phenotype studies may provide a useful contribution to identification and accurate classification of peripheral T cell neoplasms.

S-100+ lymph node neoplasm. Report of a case with histological AL and immunological features intermediate between T cell lymphoma and malignant histiocytosis.

A 16-yr-old white female was affected by continuous fever, pancytopenia with relative increase of T-8 lymphocytes, severe bone marrow hypoplasia, generalized lymphadenomegaly and splenomegaly. A first lymph node biopsy, obtained at the onset of the disease, was involved by a paracortical tumor with some S-100+ "lymphocyte-like" cells in the neoplastic areas; in the cell suspension, 70-80% of cells were E4+/E37+ lymphocytes with prevalent expression of the T-8 phenotype (52%). A second lymph node biopsy, obtained five months later, was involved by a diffuse proliferation of S-100+ cells with high mitotic activity; in the cell suspension, the majority of cells were E-/T-11+/T-3+/T-8+. At the TEM level, the neoplastic cells were characterized by regular or indented nuclei with finely dispersed chromatin and by regular or indented nuclei with finely dispersed chromatin and by irregular cytoplasmic profiles with thick pseudopodia-like projections. The possibility is discussed that this neoplasm may share some similarities with the T-gamma lymphoma being part of a poorly described group of tumors with intermediate features between T cell lymphoma and malignant histiocytosis.

T-lymphocyte subsets in human lymph nodes: relative increase of OKT-8+ cells in neoplastic and reactive B-cell proliferation.

Cell suspensions obtained from 54 human lymph nodes involved by different pathological conditions were characterized by conventional markers and by the OKT-3, OKT-4, OKT-8, OKIa-1, and OKM-1 monoclonal antibodies. In 18 cases of reactive lymphoid hyperplasia, the majority of lymph node cells were mature T lymphocytes (E-RFC = 56 +/- 9%; OKT-3+ = 63 +/- 10%); among T-cell subsets, OKT-4+ cells were 49 +/- 8% whereas OKT-8+ cells were 21 +/- 8% (T4/T8 = 2.7 +/- 1.1). This distribution of T-cell phenotypes was not similar in the different histological types of reactive lymphoid hyperplasia. In fact, an increase in the percentage of OKT-8+ cells (25 +/- 9%; P less than 0.05) and a decrease in the values of the T4/T8 ratio (2.1 +/- 1.0; P less than 0.05) were observed in 9 cases of reactive lymphoid hyperplasia of follicular type when they were compared to the mixed and sinus types. In 13 lymph nodes involved by B-cell lymphoma, the percentage of T lymphocytes was markedly reduced (E-RFC = 21 +/- 12%; OKT-3+ = 27 +/- 18%) and the percentage of OKIa-1+ cells (51 +/- 15%) was significantly (P less than 0.01) increased as compared to reactive nodes; in addition, in these cell suspensions, an increase in the relative proportion of OKT-8+ cells (T4/T8 = 1.4 +/- 0.7; P less than 0.01) could also be demonstrated. Finally, a clear prevalence of OKT-4+ cells on OKT-8+ cells was demonstrated in 5 cases of tuberculous lymphadenitis (T4/T8 = 3.9 +/- 1.5; NS) and in 18 cases of Hodgkin's disease (T4/T8 = 4.2 +/- 2.0; P less than 0.01). In tuberculous lymphadenitis, a significant increase (P less than 0.01) in the percentage of OKM-1+ cells could also be demonstrated.

Hairy cell leukemia: absence of natural killer activity and of interleukin 1 release in OKM-1+ spleen hairy cells.

Phenotypic characterization by monoclonal antibodies of spleen cells obtained from three spleens involved by hairy cell leukemia (about 90% of cells with intracytoplasmic tartrate-resistant acid phosphatase) indicated that the hairy cells are OKT-3-, OKM-1+, and OKIa1+. Neoplastic cells obtained from two lymph nodes involved by B-cell lymphoma were found to be OKT-3-, OKM-1-, and OKIa1+. Functional characterization of the hairy cells present in the spleen indicated that they lack NK activity and do not release Interleukin 1. In fact, spleen hairy cells were not lytic against K-562 tumor cells in a short-term 51Cr-release assay and did not release Interleukin 1 in culture supernatants even after LPS stimulation. In normal individuals, both these cell functions are expressed by subpopulations of OKM-1+ cells.

Severe deficiency of natural killer activity in the peripheral blood of patients with hairy cell leukemia.

Natural killer (NK) activity against K-562 tumor cells was evaluated in peripheral blood leukocytes (PBL) obtained from untreated patients affected by hairy cell leukemia (HCL). NK activity present in PBL from 10 HCL patients was at least six-fold lower (p less than 0.01) than that present in PBL from 15 healthy donors. Decreased NK activity in HCL PBL was not due to dilution of the NK effector cells by the neoplastic cells; in fact, NK activity of PBL from 4 HCL patients with less than 5% circulating neoplastic cells was still five-fold lower (p less than 0.01) than that present in normal PBL. Partial characterization of the NK defect in HCL patients indicated that: (A) defective cytotoxicity was not dependent on the duration of the assay; (B) HCL PBL added to normal PBL during the assay did not exert suppressor activity; (C) the NK activity of HCL PBL could be potentiated in vitro by interferon; and (D) low levels of NK activity were associated with reduced numbers of circulating monocytes (p less than 0.01) and of large granular lymphocytes (LGL) (p less than 0.01). In conclusion, our results indicate that the low levels of NK activity present in the peripheral blood of HCL patients may be related to reduced numbers of circulating effector cells.

The human thymus in ageing: histologic involution paralleled by increased mitogen response and by enrichment of OKT3+ lymphocytes.

In the present study, based on 24 individuals aged 5-61 years, the histological pattern of the thymus has been determined and correlated to the thymus lymphocyte in-vitro responses to phytohaemagglutinin (PHA) and concanavalin A (Con A). It has been found that lymphocytes from histologically involuted thymus were significantly more responsive to PHA and Con A stimulation than lymphocytes from histologically normal thymus. These data indicate that thymus age-dependent involution is associated with increased lymphocyte in-vitro reactivity. In addition, in 10 cases the relative proportions of the thymus lymphocyte subpopulations have been determined by the use of monoclonal antibodies OKT3, OKT4, OKT6 and OKT8. These results have been correlated to the histology of the organ and to the mitogen responsiveness of thymus lymphocytes. In normal thymus the majority of the cells were OKT6+, while there were lower numbers were OKT3+. However, in involuted thymus, OKT3+ lymphocytes prevailed over OKT6+ cells. Furthermore, a direct relationship between the proportion of OKT3+ lymphocytes and the response to mitogens has been observed. These findings indicate that the increased mitogen response noticed in histologically involuted thymus is accompanied by an OKT3+-cell enrichment which is paralleled by a depletion of OKT6+ cells.

Increased monocyte phagocytosis and decreased lymphocyte mitogen reactivity in colorectal cancer patients.

In the present study we have evaluated monocyte phagocytosis and T lymphocyte in vitro reactivity to mitogens in peripheral blood samples from 14 colorectal cancer patients, from 21 nonneoplastic control patients, and from 22 normal donors. Monocyte and lymphocyte functions were tested before and 1 and 6 months after surgery. Our results indicate the existence of increased monocyte phagocytosis and decreased mitogen reactivity in untreated patients with advanced tumors. These abnormal responses persisted and were even more pronounced after surgery and chemotherapy.

Tissue T-lymphocytes in untreated Hodgkin's disease: morphologic and functional correlations in spleens and lymph nodes.

In the present study the number of E-RFC and the in vitro response to PHA and ConA were determined for lymphocytes obtained from spleens and lymph nodes of untreated HD patients. The results were correlated with tissue involvement and with the histological pattern of the disease. Our investigation has revealed: (1) involved spleens and lymph nodes contained a higher percentage of T-lymphocytes than uninvolved tissues. The differences was statistically significant for lymph nodes. (2) A statistically significant correlation was not found between T-lymphocyte numbers and histology. (3) Lymphocytes from uninvolved spleens were significantly more reactive to both mitogens than cells from involved spleens and control non-neoplastic spleens as well. (4) Similar results were obtained with lymph node lymphocytes; however, a significant difference was observed only when cells were stimulated with ConA. (5) The difference in proliferative response between uninvolved and involved tissues was maximally expressed in MC and progressively decreased in NS and LP. (6) T-cell enrichment does not alter the difference between uninvolved and involved spleens. (7) Cells from uninvolved and involved spleens, cultured for 24 hours, maintained unchanged their proliferative capacity. In this study evidence is provided that in HD T-lymphocytes from uninvolved tissues are characterized by an increased reactivity in mitogens, whereas cells from involved tissues are normally responsive.

Natural killer activity in spleens and lymph nodes from patients with Hodgkin's disease.

Natural killer (NK) activity against K-562 myeloid cell line was evaluated in 21 spleens and 14 lymph nodes from patients with Hodgkin's disease (HD). NK activity of eight HD-involved (HD+) spleens [556 lytic units (LU)] was found 5-fold higher than that of 13 HD-uninvolved (HD-) spleens (112 LU) (p less than 0.01). Moreover, NK activity of HD+ spleens was significantly different (p less than 0.05) from that of three spleens involved by non-HD lymphoma (100 LU). NK activity of four spleens from nonneoplastic patients (250 LU) was intermediate between those of HD+ and HD- spleens. Lymph node cells were about 10-fold less cytotoxic. NK activity of seven HD+ lymph nodes (43 LU) was 3-fold higher than that of three lymph nodes involved by non-HD lymphoma (8 LU). However, these differences were not statistically significant. Our data are compatible with increased NK activity in HD+ tissues as well as with depressed NK activity in HD- tissues. The observation that NK activity of peripheral blood leukocytes from nine HD patients (116 LU) (p less than 0.05 only at 100:1 effector:target cell ratio) may support the latter interpretation. Partial characterization of effector cells in HD+ and in HD- spleens indicated that, in both instances NK cells were nonadherent and almost equally distributed between the erythrocyte-negative and -positive cell fractions. Finally, NK activity of both HD+ and HD- spleen cells could be further potentiated in vitro by interferon.

PHA and ConA lymphocyte response in normal, hyperplastic and neoplastic human thymus: morphologic and functional correlations.

In the present work the in vitro response to PHA and ConA was determined for thymus lymphocytes from normal, hyperplastic, and neoplastic conditions as well as for peripheral blood lymphocytes from normal control donors. Our investigation has revealed these results: (1) lymphocytes from normal thymus are less responsive to both mitogens than lymphocytes from peripheral blood and abnormal thymus. (2) Lymphocytes from thymic benign lymphoid hyperplasia show an increased response only to PHA regardless of the extent of hyperplasia. (3) Lymphocytes from thymic neoplasia display an increased response to both mitogens regardless of the histologic pattern of the tumor studied. (4) Thymic lymphocytes from patients with myasthenia gravis have a mitogenic response higher than that from thymic lymphocytes from nonmyasthenic subjects. (5) In patients with myasthenia gravis, lymphocytes from neoplastic thymus show a response to PHA higher than that observed for lymphocytes from hyperplastic thymus. It seems clear that two conditions maximally enhance mitogenic response of thymus lymphocytes: thymomas and myasthenia.

In vitro formation of eosinophil-like colonies by lymph node cells from a human lymphoma.

In vitro culturing of lymph node cells from a human non-Hodgkin lymphoma gave rise to several colonies of eosinophil-like cells. Eosinophil colonies originated from cells that during the first week of culture had a fibroblast appearance and were adherent to plastic. The tissue culture was sacrificed after 14 days. At that time each colony was formed by 20-50 cells with intracytoplasmic peroxidase-positive and eosinophilic granules. Cells comprising the colonies exhibited different degrees of differentiation. Some of the cells (26.6%) were mature eosinophils, the majority (66.8%) resembled eosinophil myelocytes, and some other (4.6%) had a fibroblast appearance. One or two multinucleated giant cells were often present in the center of most of the colonies. These cells contained up to 10 nuclei, which were arranged in a "ring form" or centrally located; giant cells with a single, central, large, multilobed nucleus were also observed. Cells belonging to other myelopoietic lines could not be identified in the tissue culture. Histological examination of the lymph node revealed extensive presence of eosinophils at various degrees of maturation but absence of other myelopoietic lines.

Cell selection in the thymus of mice treated with escherichia coli lipopolysaccharide (LPS).

The present data indicate that injection of LPS induces a decrease in thymus weight with selection of thymocytes more efficient in killer and helper activities. It has been reported (12) that two T-cell types may cooperate in the GVH reaction: the effector T1 and the amplifier T2. The maturation from T1 to T2 occurs mainly in the periphery, whereas immature cortex thymocytes differentiate to T1 within the thymus (13). Our results, showing an LPS-dependent enhancement of thymocyte killing activity, suggest that LPS selects thymocytes mostly of the T1 type. LPS-treated thymocytes are more efficient in the reconstitution of the anti-SRBC response. These data can be explained considering that LPS, like cortisone (14), enriches the thymus with more immunocompetent helper cells. Alternatively it may be suggested that LPS selects in the thymus cell populations characterized by high proliferative activity. Unpublished observations showing that LPS-treatment in vivo increases the in vitro response of thymocytes to Con-A are consistent with this interpretation.
