Diagnostic value of integrated ultrasonic backscatter in congestive cardiomyopathy.

Congestive cardiomyopathy is a fatal myocardial disease which can be diagnosed by clinical findings, electrocardiography, roentgenography and echocardiography. Tissue characterization and pathology have been determined recently using histogram methods to analyze ultrasonographs. In this study ultrasonic backscatter analysis for differentiation of normal and diseased myocardium was tested. Two-dimensional echo data and radio frequency signals of long-axis views of the left ventricular posterior wall of 10 healthy children and 14 patients with congestive cardiomyopathy in systolic and diastolic phase were digitized online into the computer memory. Cyclic variation was obtained in the control group but no variation was detectable in myopathic hearts.

[The value of laboratory tests for the early diagnosis of newborn sepsis].

In our study, laboratory tests were evaluated in the early diagnosis of newborn sepsis. The study consisted of 53 newborn infants with the diagnosis of suspected sepsis. Of the 53 newborn infants twenty seven were excluded from the study, because of negative blood culture. Remaining 26 newborns with positive blood culture and 15 healthy newborns comprised the study (sepsis group) and control group respectively. Total mature neutrophil count, immature/total neutrophil and immature/matür neutrophil ratio, toxic granulation changes, serum level of C-reactive protein and haptoglobin were found statistically higher in sepsis group than control group. Thrombocyte count and serum level of IgG were significantly lower in sepsis group. Average total white blood cell count, result of nitroblue tetrazolium tests erythrocyte sedimentation rate and serum level of IgM was similar in both groups. We emphasized that hematological scoring system, serum level of C-reactive protein and haptoglobins can be used in the early diagnosis of newborn sepsis.

[Intestinal parasites in primary schools of different socioeconomic status and environmental conditions].

Two hundred ninety-four stool sample obtained from primary schools of different social status and environmental conditions were examined for intestinal parasite. The age range was between seven and 13 years, 43% of the patients were females and 57% of the patients were males. In our study it has been observed that the parasite incidence was 29.3%. The predominant parasite was Giardia intestinalis (15.7%). Enterobius vermicularis (10.5%) and Ascaris lumbricoides (2.3%) have been followed respectively. There were three different parasites in the primary school of good social status and environmental conditions. In the contrary in the village school, there were five different parasites. In the conclusion parasite incidence was high in our region. In primary schools of different conditions there were no difference Giardia intestinalis but there were marked difference in the other parasites.

Neurologic complications in chronic renal failure: a retrospective study.

We have retrospectively examined 324 patients with chronic renal failure and evaluated the probable underlying causes of neurologic complications, laboratory data and therapeutic interventions. The common neurologic problems in our patients were alterations in consciousness (40.7%) and convulsions (35.1%). When BUN concentration was above 135 mg/dl and creatinine clearance was below 8 m/min/1.73 m2, alteration of consciousness was observed and when BUN concentration was 200 mg/dl and creatinine clearance was below 7 m/min/1.73 m2, abnormal convulsives appeared. Changes in deep tendon reflexes and pathologic reflexes were associated with hypertension. All of the patients with cortical atrophy using computerized cranial tomography aluminum hydroxide at least for 18 months, and six of them had hemodialysis. Fourteen patients who underwent dialysis developed convulsions and were thought to have disequilibrium syndrome. These findings are consistent with the suggestion that the metabolic and biochemical derangements associated with CRF may be particularly detrimental to the still developing CNS of the child.

Role of protein C in childhood cerebrovascular occlusive accidents.

A group of 23 children with cerebrovascular occlusion of unknown etiology were re-evaluated 6 months-2 years later. Plasma protein C levels were determined with the coagulation method and were low in 10 cases. The role of this deficiency and the necessity of this test in cerebrovascular occlusion is discussed.

Fetal substantia nigra grafts. Effect on dopamine receptors in the rat corpus striatium.

Effects of fetal substantia nigra grafts on the dopamine receptors in the corpus striatum in rats were investigated after the destruction of the nigrostriatal dopaminergic pathways with intraventricular 6-hydroxydopamine injections. The expected dopamine receptor denervation supersensitivity was demonstrated by a 53.7% increase of [3H]spiroperidol binding in rats with sham grafts compared with normal control-group rats. In contrast, rats with grafts showed a significant reduction of supersensitivity, with a 25% decrease in binding to the graft-bearing caudate when compared with the sham-graft group. A nonsignificant 15% decrease in binding on the nongrafted side was also observed. The fetal substantia nigra grafts thus reduced the denervation supersensitivity toward a normal level.

Evidence of a supraspinal opioid analgesic mechanism engaged by lateral hypothalamic electrical stimulation.

Brief trains of electrical stimulation were applied to the tails of rats to evaluate pain thresholds in the presence and absence of concurrent brain stimulation. Lateral hypothalamic (LH) stimulation, particularly in the dorsolateral medial forebrain bundle, elevated threshold for eliciting a post-stimulus vocalization response. Thresholds for eliciting a simple vocalization and motor response--both of which are organized at lower levels of the CNS than post-stimulus vocalization--were not significantly affected. This restricted form of analgesia was reduced by the opioid antagonist, naltrexone. Rewarding effects of stimulation in these LH sites, as evaluated in tests of self-stimulation threshold, were not affected by naltrexone. These results suggest that LH stimulation activates an opioid analgesic mechanism that is selectively active at a supraspinal level and diminishes the affective consequences of otherwise noxious stimuli.

Two-dimensional ultrasonographic findings of atelectatic lung segments.

Thirteen cases with pulmonary atelectasis were examined by two-dimensional sonography in our clinics between October 1990 and April 1993. Patients' ages ranged from 2 to 16 years. Five patients revealed atelectasis with pleural effusion and eight patients had only atelectasis. Two-dimensional sonography showed a solid mass indistinguishable from lung parenchyma in patients with atelectatic lung segment without pleural effusion while atelectatic segment was readily identified in those with pleural effusion. We conclude that sonography is a useful and non-invasive technique in differentiating cardiac from extracardiac masses, because atelectatic lung segment with moderate to large pleural effusion may mimic intrapericardial masses.

Cardiac functions in children with vitamin D deficiency rickets.

Nutritional deficiency of vitamin D is common in developing countries as a result of both inadequate diet and exposure to ultraviolet light. The most striking biochemical finding in this illness is hypocalcemia. Reduction in serum calcium level may affect ventricular contraction. The purpose of this study was to evaluate prospectively left ventricular function in a group of 27 infants diagnosed as having rickets. Electrocardiograms and echocardiographic studies were undertaken in all patients. A group of ten healthy infants was used as a control for the echocardiographic examinations. Patients were divided into three groups according to the biochemical classification of rickets. There were eight patients in group I, nine in group II, and ten in group III. Abnormal electrocardiographic findings were noted in four infants in group I, three in group II, and six in group III before treatment of the rickets. These changes resolved following treatment. Echocardiographic studies revealed left ventricular dysfunction in the pretreatment stage. The most striking echocardiographic finding is the increase in the ratio of interventricular septal thickness to left ventricular posterior wall thickness in eight patients from group III. This returned to normal after treatment of the rickets. This study has demonstrated echocardiographic evidence of left ventricular dysfunction in children with rickets. These abnormalities were not, however, sufficiently severe to be associated with clinical signs of cardiac failure. Cardiomyopathy may develop in rickets, especially in the third stage of the disease, and this finding may return to normal following adequate treatment of the rickets.

Computerized tomography findings of the posterior fossa in children: etiology and clinical correlation.

Of 1105 childhood cases who were evaluated by computerized tomography (CT) in a two-year interval, 93 who had posterior fossa abnormalities are reviewed. The cerebellar atrophies, either alone or accompanied by cerebral atrophy, were the most common morphological diagnoses. The clinical picture, etiology, and developmental state of the cases are discussed in relation to the CT findings.
