[Multi-media computer-based chart: modern tool of clinical record keeping].

AIM: To design the program of computer multimedia case history (MCH) for collection, storage and analysis of information about the patient including diagnostic images; to test MCH for application in telemedical consulting and control over valid conduction of the treatment protocol. MATERIAL AND METHODS: Brief representation of information is reached by integrated multilevel placement of the data on common time axis. Dynamics of the selected parameters is represented as normalized diagrams in the same system axes. Electron tables are filled either by hand or import of the data from individual electron file of all the information about the patient. Diagnostic images and conclusions are obtained from local computer bases of the diagnostic units. Inexpensive commonly used computers allows one to input digital micro- and macrophotos, x-ray images, etc. Monitor information is available from the universal computer net directly from the monitors Geolink-M. RESULTS: MCH was tested in the Hematological Research Center of RAMS. In cooperation with Bryansk diagnostic center N 1 telemedical consulting and some data bases were perfected. The MCH system joints routine activity of the clinic without any problems raising quality of the patients management, facilitating the access to therapeutic and diagnostic information and its analysis. On-line breaking the limits of the controlled parameters and protocols is detected automatically. These events are emphasized by "blocking questions" which must be worked out by the user. The information on the patient can be fully stored on CD-ROM in line with the MCH program providing organization of the data. CONCLUSION: The system is introduced which collects medical information from various sourses and builds up comprehensive representation of the data on the same time axis. The system provides control over implimintation of the protocols of the patients' management, gives a convenient access to medical information, ensures its reliable storage and availability in case of the patients transfer to other medical institutions, facilitates analysis of clinical cases, conduction of medical consulting.

[Use of open method of treatment in diffuse purulent peritonitis in children].

Open method of treatment--laparotomy--was used in 22 children with diffuse purulent peritonitis, aged from 1 to 14 years. This permitted to drain adequately the abdominal cavity after the operation, to abolish the intoxication during 2-3 days in the most observations, to eliminate the enteroparesis. The single repeated abdominal cavity sanation was sufficient in 19 (90.5%) children, in 2 she was applied twice. Laparostoma was closed in 17 (80.9%) patients on the 2-3 th day, in 4--on the 4-7 th day after the operation. One child (4.5%) died.

[Bone marrow hematopoiesis in those who worked in the cleanup of the aftermath of the accident at the Chernobyl Atomic Electric Power Station with a history of acute radiation sickness (based on myelogram data)].

On the basis of myelograms an evaluation was done of medullary hemopoiesis in 15 subjects who had been taken ill with acute radiation sickness (ARS) as a result of Chernobyl accident in 1986. 6-12 years following the exposure to ionizing radiation the bone marrow of ARS convalescents still shows a tendency toward hypocellularity (the number of myelokaryocytes in the examinus ranged from 40 to 80 x 10(9)/l), high index of neutrophil maturation (0.99) was recordable, which fact was related to depletion of granulocyte reserve and increase among the elements of granulopoiesis of share of proliferating cells, together with a rise in the number of lymphoid elements (19%).

[Multidrug resistance determinants in acute myeloid leukemia developed in persons exposed to ionizing radiation due to the Chernobyl accident].

The results of multidrug resistance determinants expression analysis on leukemic cells of 56 acute myeloid leukemia (AML) patients by immunophenotyping are presented. Of these, there were 21 persons exposed to ionizing radiation due to the Chemobyl accident with radiation-associated AML and 35 patients with spontaneous leukemia. The aim of this study was to determine if transport proteins (P-glycoprotein, LRP, and MDR1), apoptosis-related proteins (Fas, Bcl-2, Bax, p53, and Bcl-X(L)), and topoisomerase IIalpha expression in AML patients with the history of radiation exposure differed from those in spontaneous AML cases. Leukemic cells in patients with radiation-associated diseases compared to spontaneous AML more often overexpressed antiapoptotic oncoprotein Bcl-2 (12/21 vs. 6/35, p < 0.005) and less often demonstrated expression of Fas receptor (12/21 vs. 30/35, p < 0.05). Moreover, leukemic cells were simultaneously Fas negative and Bcl-2 positive in 4 out of 21 patients exposed to ionizing radiation but none of spontaneous cases had similar phenotype (p < 0.05). Leukemic cells in patients with radiation-associated AML compared to spontaneous cases more often were P-glycoprotein positive (12/20 vs 9/31, p < 0.05). P-glycoprotein overexpression significantly correlated with resistant disease in patients with radiation-associated AML (r = 0.47, p < 0.05), but was not a prognostic variable for the treatment outcome in terms of overall survival. Defects in pathways of drug-induced apoptosis and function of pump, that actively effluxes drugs could contribute significantly to developing drug resistance in radiation-associated AML.
