[The specialization and advancement of military physicians in radiodiagnosis].

The article deals with the questions of post-graduate training of army diagnostic radiologists. The authors make a detailed description of all stages of basic and enhanced training of radiologists and ultrasound diagnostic specialists. The article emphasizes the necessity to endorse the candidates for further specialized training depending on duration of their professional time-in-service period. Each stage of enhancing training is also described from the point of view of new professional skills acquired in the process of training.

[The causes of death in animals with hydrogen peroxide poisoning].

Hydrogen peroxide (HP) was administered to experimental rats at the level of LD100. The indices of external breathing, blood pressure, ECG, heart thrust were determined. It was found that the increasing tonus of the vagus nucleus was an essential feature of the pathogenesis of HP poisoning, inducing a series of pathological changes in animals. A certain role was played by extensive gas embolism. About 80% of the experimental animals survived after administration of atropine or lobeline.

[Plasmapheresis in the treatment of patients with hypertension].

The authors discuss the results of the use of plasmapheresis in multimodality treatment of patients with stage II essential hypertension. At present the prevalence, ever increasing accessibility and relative simplicity of plasmapheresis make it possible to use it for the treatment of essential hypertension. Analysis of the data obtained shows the high efficacy of plasmapheresis in essential hypertension. Since plasmapheresis turned out effective in combination with drug therapy and alone as well, it is concluded that plasmapheresis is not only an approach to overcoming resistance to hypotensive therapy but also an independent treatment modality that exerts a considerable effect on the immunologic and hemorheologic blood properties. The above-indicated allows recommending plasmapheresis for the treatment of patients suffering from essential hypertension, since it may lead to a long-term decrease of arterial pressure, to the enhancement of the sensitivity to hypotensive drugs, to the diminution of drug requirements, thereby ameliorating the course and prognosis of the disease in patients suffering from essential hypertension.

[Changes in the hemostasis system during long-term intravenous administration of nitroglycerin in the acute period of myocardial infarct].

Twenty seven patients were examined in the first hours of the onset of myocardial infarction for the hemostatic systems during a long-term intravenous administration of nitroglycerin (N). They were compared with 24 patients from a control group. N was found to reduce the signs of blood hypercoagulation, decrease platelet aggregation, increase fibrinolytic activity, and elevate antithrombin III levels. Hemostatic alterations were related to a hemodynamic response to nitroglycerin. Positive shifts in the hemostatic system were the most evident in the patients who exhibited higher cardiac output and lower signs of heart failure. In the patients without signs of heart failure, N deteriorated hemodynamic parameters, aggravated hypercoagulation and inhibited blood fibrinolytic activity.

[Effects of plasmapheresis on lymphocytic energy enzymes in hypertension].

Cytochemistry of peripheral blood lymphocyte enzymes was studied in 58 patients with State II essential hypertension (36 of these were treated with traditional hypotensive therapy and 22 with plasmapheresis + drug therapy). alpha-GPDH, LDH, and acid phosphatase activities were found reduced in these patients. The degree of the enzymic activity reduction (SDH included) depended on the disease standing and family history of patients. Plasmapheresis elevated the activities of energy enzymes and reduced arterial pressure, i.e. these parameters were in inverse correlation. In the patients treated by routine hypotensive therapy energy enzymes' activity was unchanged, despite the clinical effect.

[The functional heterogeneity of circulating polymorphonuclear leukocytes in acute myocardial infarct].

Cytofluorometric analysis, using immune complexes, consisting of human IgG and fluorescein isothiocianate (FITC)-labelled monospecific rabbit antibodies to human IgG, as fluorescent markers for polymorphonuclear leukocytes (PML), and microscopic analysis of modified NST-test probes with NADH2 or NADFH2 were employed to investigate PML functional heterogeneity of healthy individuals and patients with acute myocardial infarction. It was shown, that circular PML make up heterogeneous cell population in the following parameters: Fc gamma-receptors expression, oxidative derivates generation and cell oxidases activity and the degree of heterogeneity may change during nonspecific inflammatory process, which accompanies acute myocardial infarction.

[Treatment of patients with hypertonic disease with plasmapheresis].

The paper presents a discussion on therapeutic results obtained with plasmapheresis treatment in 16 patients with stage II essential hypertension. Thirty-three comparable patients entered a control group of conventional treatment. The test and control patients with stage II hypertension were examined for time-course changes in T- and B-lymphocytes counts, the activity of the energetic enzymes (alpha-glycerophosphate dehydrogenase, succinic dehydrogenase, lactate dehydrogenase), serum immunoglobulins and circulating immune complex levels. It was established that the standard antihypertensive treatment failed to restore normal parameters of immunity and to favor positive alterations in the activity of the enzymes in the blood lymphocytes, whereas therapeutic plasmapheresis was found to stimulate immunity, especially cellular one. This occurred in line with a rise in the levels of lymphocytic dehydrogenases.

[Difficulties in the diagnosis of brain tumors].

In 27 patients brain tumors were misdiagnosed for several years. Most mistakes were due to the incomplete analysis of the patients' complaints, anamnesis and peculiarities of the disease development. Most difficult were identifying the leading neurologic deficit, detecting the adjacent and remote signs. Using full sequence of both simple and sophisticated diagnostic techniques (PET, angiography, CT etc.) one can sharply reduce the incidence of diagnostic errors.

[Morphofunctional state of the gastric parietal and gastrin-producing cells in chronic gastroduodenitis in children].

Morphofunctional studies of parietal and gastrin-producing cells in 30 children with chronic gastroduodenitis with (10 patients) or without (20) recurrent erosions in the pyloroduodenal region showed functional inhibition of G cells and hyperplasia of P cells in the antral part of the stomach in children with recurring erosions. In these children, the increased surface area and density of parietal cells, the increased perimeters of secretory canaliculi's membranes, and the consequent elevated gastric juice acidity were probably due to P-cell hyperplasia in the gastric antrum and G-cell hyperplasia in the duodenal bulb. In children, gastroduodenitis with recurrent erosions should be considered a pathogenetic variant of duodenal ulcerous disease.
