Clinical trials on adjuvant chemotherapy for breast cancer.

Clinical trial initiated in 1975 at the Petrov Research Institute of Oncology (Leningrad) included 1228 patients with breast cancer Stages I, II, and III. Adjuvant chemotherapy in patients with Stages I-II (P T0-2N0-1M0) leads to decrease in mortality rate by 24.3% in the case of monochemotherapy (thiotepa, six courses, 200 mg each course) and 32.4% in the case of polychemotherapy (thiotepa, methotrexate, 5-fluorouracil [TMF], cyclophosphamide, methotrexate, 5-fluorouracil [CMF], six course). There is a trend to higher (by 7.8%-9.8% versus control) rates of adjusted survival in the groups of patients with relatively early stages of breast cancer (P T0-2N0M0) subjected to adjuvant mono- and polychemotherapy. In the group of breast cancer patients (P T0-2N0-1M0) who received adjuvant polychemotherapy (TMF, CMF schemes) the survival rates are higher by 12.0%-16.6% than in the control group during the sixth, seventh, and eighth years of the follow-up. Favorable effect of adjuvant chemotherapy manifested by diminished mortality rate and prolonged survival was statistically significant only in the group of patients younger than 50 years.

The results of clinical trial within the framework of CMEA on surgical methods of mammary gland carcinoma treatment.

The results of joint clinical investigation performed by 8 oncological centers within the framework of CMEA countries are reported. Out of 816 patients with mammary gland carcinoma (MGC) of lateral localization, stage T1-2N0-1aM0, classical radical mastectomy by Halsted was carried out in 230 patients, mastectomy by Patey in 318 patients and conservative operation of radical resection type--in 218 patients. Specific characteristics of the disease progression were studied considering the time of development and incidence rates of local relapse or distant metastases depending on the type of surgery and MGC stage. High rates of 5-year survival were found for all types of surgery, mastectomy by Patey proving somewhat superior.

[Adjuvant mono- and polychemotherapy in breast cancer (a randomized study)].

A clinical study started at the Institute in 1975 embraced 1,228 patients with breast tumors of stages I, II and III. Adjuvant monochemotherapy (six courses of 200 mg Thio-TEPA) was followed by a 24.3% decrease in the death rates for the patients with stage I-II tumors (pT0-2N0-1M0), whereas polychemotherapy (six courses of TMF and CMF)-by 32.4%. A 7.8-9.8% increase in corrected survival (as compared with control) was registered in those patients with less advanced malignancies of the breast (pT0-2N0M0) who had received mono- and polychemotherapy. In patients with pT0-2N0-1M0 tumors treated after the TMF or CMF modalities, survival rates showed a 12.0-16.6% increase, as compared with controls, after 6, 7 and 8 years. The beneficial effect of adjuvant chemotherapy (lower mortality rate matched by higher survival) was reliably established in patients under 50 only.

Antineoplastic Combined Chemotherapy Protocols↗

End results of treatment of breast cancer patients.

Data are presented on 3359 breast cancer patients treated at the Petrov Research Institute of Oncology of the USSR Ministry of Health. For the 20-year period (from 1955 to 1975) not large but statistically significant increase of the 5-year survival was noted which achieved 64.7% in 1971-1975. The rise of survival was observed in patients aged under 50, subjected to adjuvant chemotherapy (Thio-TEPA, 5-Ftoruracil). Development of distant metastases was the cause of death of breast cancer patients within the 15 years after the beginning of treatment in 68.8% of cases. Only 7.6% of patients expired from distant metastases in 15-20 years following operation.

[Characteristics of the therapeutic measures in breast cancer (based on data of the All-Union Center for the Study of the Effectiveness of Treatment of Cancer Patients)].

The paper deals with the evaluation of methods of treatment of 13,170 breast cancer patients. Stage I-II tumors were detected in 53.0% of cases. Surgery which still remains to be the basic method of treatment for breast cancer was used alone or in combination with radiation, chemo- and hormonotherapy in 77.6%. Radical treatment was given to 80.0% of patients under 55 years and 44.0% aged 75 and more. The number of cases who received symptomatic therapy decreased from 4.9 to 3.5%, while the percentage of combined (radiosurgical) treatment increased from 21.1 in 1968-1972 to 30.9% in 1974-1978.

[Pulsed laser radiation in the treatment of recurrences of breast cancer].

Radiation from a pulsed laser installation Pulsar - 1000 (wavelength - 1.060 nm. pulse duration - 1 ms) was used in the treatment of 87 recurrences of breast cancer in 31 patients. Fluence ranged 300-450 J/cm2 depending on the depth of recurrence. No recurrence of the first relapse was registered within six months - three years after exposure to laser radiation. Pulsed laser radiation was shown to be an effective means of treatment of localized relapse of breast cancer which does not involve musculature or thoracic wall. Laser therapy should be considered a method of choice in patients who have previously undergone radiation treatment as well as in cases of clusters of advancing relapse. Laser treatment of recurrent breast cancer may be conducted in conjunction with chemo - or hormone therapy.

[Long-term results of treatment of 3,359 patients with breast cancer].

The paper presents the data on 3359 cases of breast cancer treated at the Institute. A slight (4.7-7.6%) though significant increase in five-year survival was registered within 20 years (1955-1975). It had gone up to 64.7% by 1971-1975. A significant increase was observed in patients under 50 years who received an adjuvant postoperative chemotherapy (thiotepa and 5-fluorouracil). Mortality from distant metastases within the first 15 years after the beginning of therapy decreased from 89.5 to 68.8% and it was as low as 7.6% at later periods.

[Treatment of phylloid tumors of the breast].

Phylloid fibroadenoma of the mammary gland is a comparatively rare lesion--0.3% of all breast affections, its clinical course is generally known to be characterized by both the possibility of recurrences development after non-radical treatment and its transition into mammary gland sarcoma. The authors report the data on 56 cases of phylloid fibroadenoma of the mammary gland treated at the N.N. Petrov Research Institute of Oncology of the USSR Ministry of Health during the period of 1926 to 1973. Fifty one of these patients showed benign tumors, 5--malignant tumors. The clinical diagnosis of the tumor is rather complicated (the correct preoperative diagnosis was established only in 27.5% of cases). The use of non-contrast mammography and trepanbiopsy failed to appreciably improve the diagnosis. The differential diagnosis between the benign and malignant forms seems to be mostly difficult. Only surgery is advocated. The extent of surgical intervention depends on morphological features of the tumor, its size, rate of growth and the breast size. Among benign forms recurrences were noted in three patients (6.3%). Of 5 patients with malignant phylloid fibroadenoma two patients died due to metastases during two postoperative years.

[Densitometric analysis of the mammograms in benign breast tumors].

Densitometry of roentgenograms was performed in 48 patients with benign tumors of mammary glands (16-cysts, 16-fibroadenomas, 16-phylloid fibroadenomas). The analysis of densitograms indicated that densitometry provides a more objective characteristic of the structure intensity of the shadow and distinctness of the pathological focus contour. Densitometric analysis of the roentgenological picture of cysts, fibroadenomas and phylloid fibroadenomas along with features common to these neoplasms enabled the recognition of a number of symptoms determining differences in their densitometric picture.
