[Complex assessment of vasomotor function of vascular endothelium in patients with hypertension].

Vasomotor function of vascular endothelium was studied in 62 patients with grade 1-2 hypertension with moderate and high added risk. Methods included study of brachial and middle cerebral artery endothelium dependent and independent vasodilation/vasoconstriction, measurement of plasma levels of nitric oxide metabolites (NO(n)-), endothelin-1, and antithrombin, as well as registration of their changes during vasomotor tests with calculation of integral indexes. Most patients with hypertension differed from controls by preponderance of vasoconstrictor over vasodilator reactions both in peripheral and cerebral vascular bed. At the same time patients with hypertension had pronounced dissociation between vasomotor responses of cerebral and peripheral vessels compared with subjects with normal blood pressure (p<0.05). Besides lowered basal level of NO(n)- and high concentration of endothelin-1 patients with hypertension were characterized by hyperreactivity of nitricoxidergic system, augmented lability of endothelin producing system, and impaired athrombogenecity of vascular endothelium. Complex assessment of vasomotor function of vascular endothelium by sequential vasoactive tests characterizes functional and metabolic activity of cerebral and peripheral vessels and can be used for improvement of risk stratification and monitoring of efficacy of treatment of patients with hypertension.

[Functional and biochemical characteristics of the vasodilator effects of nebivolol in patients with arterial hypertension].

AIM: To examine nebivolol-induced changes in endothelial vasomotor function and blood concentrations of nitric oxide (NO) metabolites in patients with arterial hypertension (AH). MATERIALS AND METHODS: The impact of 8-week nebivolol monotherapy on 24-hour blood pressure (BP) monitoring, ECG, autonomic heart regulation, changes in endothelium-dependent vasodilation (EDVD) and endothelium-independent vasodilation, on the plasma concentration of NO metabolites, and on the endothelial responsiveness index (ERI) were studied in 30 patients with stages I-II AH. RESULTS: Nebivolol provides a sufficient 24-hour BP monitoring, reduced heart rate, the incidence of supraventricular arrhythmias, and left ventricular myocardial mass with the optimization of autonomic cardiac regulation. The agent recovers NO-ergic regulation of endothelial vasomotor function in patients with AH by normalizing the basal level of NO and EDVD. The study of plasma NO metabolites before and during reactive hyperemia test with ERI calculation substantially extends an idea of the mechanisms of EDVD and supplements the diagnostic potentialities of this method. CONCLUSION: Dysfunction of the vascular endothelial NO-producing system occurs in patients with AH. In addition to its positive effect on 24-hour BP profile and myocardial morphofunctional parameters, nebivolol restores the NO-ergic regulation of endothelial vasomotor function in patients with AH, by potentiating NO expression, and modulates steady-state vasodilation.

[Early thrombotic complications after femoro-popliteal segment surgery].

The work analyses the causes of early postoperative thromboses which occurred after reconstructive operations on the femoro-popliteal segment in 293 patients. Early postoperative thromboses are a frequent complication in reconstructive operations on the vessels and develop in 27% of cases mostly in the first days after the operation. The most frequent causes of early thromboses were deficient distal arterial blood circulation, technical and tactical errors of the intervention, and insufficiently corrected postoperative therapy. It is pointed out that combination of reconstructive operations on the main vessels with lumbar gangliectomy has a favorable effect on the results of treatment and prevention of early thromboses. The use of microsurgical techniques reduces the probability of the development of early postoperative thromboses. In the absence of an effect of repeated operative interventions, in increase of ischemia and intoxication the limb must be amputated before gangrene develops.

[Pathomorphology of the endocardium and myocardium in the heart injury].

Light, polarization, luminescent microscopy, phase contrast, special staining methods and scanning electron microscopy were used for studying sections and fragments of the endocardium and myocardium from 123 patients who had died of heart contusion and ischemic heart disease and from 50 rats with experimental heart contusion. The morphological classification of heart contusion and differential-diagnostic table of myocardial alterations due to contusion of the heart and ischemic heart disease are proposed to facilitate pathomorphological diagnosis of heart contusion. The classification of pathomorphological signs in heart contusion is presented.

[Forensic-medical assessment of pathomorphological changes in the myocardium in blunt chest trauma with an isolated heart contusion].

A total of 37 autopsy cases were divided into two groups. Group 1 consisted of 17 victims who had died of closed blunt trauma of the chest with isolated heart contusion made by fists, feet in shoes. Group 2 (control) consisted of 20 cases of sudden death of ischemic heart disease because of atherosclerotic cardiosclerosis. Fragments of the heart containing endocardium were studied on the microscope JEIL-SM200 (Japan). Endocardium pathomorphology was represented by relief changes of the endocardium, structural arrangement of separate groups of muscular fibers above the endocardium surface, hemorrhages and blood clots, endotheliocyte desquamation, prelacerations, lacerations and destruction of the endocardium.

[A microthermal resistance method for determining the species specificity of protein].

Microthermal method was used for objective registration of the course of precipitation reaction in determination of species specificity of human and animal protein. This method allows one to carry out species identification using blood protein dilution with 0.9% solution of natrium chloride in correlation from 1:100 up to 1:100,000. Method is simple in use, it can be operated on the apparatus of home production. Analysis with two control tests may be performed within 10 min. The results of reaction are successfully registered as thermistograms subjoined to the expert conclusion.
