[Catalytic autoantibodies as a new molecular instrument in rheumatological practice].

AIM: To compare clinicopathogenetic value of DNA-hydrolizing autoantibodies or DNA-abzymes in patients with systemic lupus erythematosus (SLE) and rheumatoid arthritis (RA). MATERIAL AND METHODS: We studied sera from 180 patients with SLE, 180 RA patients and 128 healthy donors matched by age and gender; assessed catalytic and cytotoxic activity of DNA-abzymes in patients with different variants of SLE and RA course. RESULTS: The highest catalytic and cytotoxic activities of DNA abzymes were observed in SLE patients. In SLE catalytic and cytotoxic activities of DNA abzymes ranged widely and their mean values depended on SLE activity in patients with systemic lesions. DNA-abzymes in RA patients showed lower catalytic and cytotoxic activities in relation to substrate DNA and target cells than in SLE. DNA-abzymes occurred most frequently in patients with high activity of RA, slow-progressive and lingering course of RA, especially in early development of visceral (extra-articular) pathology. Characteristic for DNA-abzymes in RA and SLE is the phenomenon of wide-range fluctuations due to factors determinating probability of induction of function of Ab-mediated catalysis and, therefore, incidence rates of DNA-abzymes, probably catalytic autoAb of the other specificity in a population of patients with systemic autoimmune diseases. CONCLUSION: The data indicate the validity of DNA abzymes use in clinical practice for realization of diagnostic and therapeutic programs in SLE and RA.

[Catalytic autoantibodies--a new molecular instrument in cardiology and ophthalmology].

AIM: To develop a conceptual model of using catalytic autoantibodies as diagnostic and monitoring tools in organ-specific autoimmune disorders. MATERIAL AND METHODS: A total of 99 patients (56 males and 43 females aged 21-52 years) with autoimmune myocarditis (AM) and 198 patients (77 males and 121 females aged 8-79 years) with autoimmune uveitis (A U) participated in the study. AM patients were examined for anticardiomyosin and anti-DNA autoantibodies (ACM, ADNAab), AU patients - for autoantibodies to S-antigen, IRBP, redopsin, phosphocine, autoDNA. RESULTS: AM patients had double level of DNA-binding autoantibodies. In 1/3 of them there was hydrolysing DNA and cytotoxic activity. In AU patients maximal titers were in Behcet's disease, sympathic ophthalmia, generalized uveitis and viral uveitis. CONCLUSION: Autoantibodies with different specificity and function including DNA-abzymes can be additional diagnostic and prognostic markers.

[Immunogenetic HLA markers of chronic viral hepatitis].

AIM: To study possible immunogenetic HLA markers of chronic viral hepatitides. MATERIAL AND METHODS: Using the reaction of complement-dependent cytotoxicity by Terasaki, we analysed distribution of leukocytic HLA antigens (loci A, B and C) in 179 patients with chronic viral hepatitides B, C and D in Russians and Kazakhs living in the Astrakhan Region. RESULTS: In the Russian population we discovered a significant positive association of CVHB with HLA-B18, HLA-B35, HLA-B40, HLA-Cw3 antigens, and negative one--with HLA-A2. In Kazakhs with CVHB there was a positive association with HLA-A3, HLA-B18 and negative one--with HLA-A11. Alleles HLA-A10, HLA-B35, HLA-B40 and HLA-Cw3 mark CVHC in Russians. HLA-Cw4 specificity acts as protector in development of chronic HCV-infection. A correlation was found between carriage of some specificities and haplotypes of HLA and activity of chronic HBV and HCV infection. A high risk of chronic delta infection in Russians is associated with HLA-B8 and HLA-B35, in Kazakhs--with HLA-B35 and HLA-D40. There are significant associations between CVHB, CVHC, chronic delta infection and some HLA haplotypes. CONCLUSION: A universal role of HLA-B35 specificity in development of CVH irrespective of hepatotropic virus and patients' nationality is shown.

[Cytokines as a diagnostic and therapeutic tool in patients with myocarditis].

One of the leading causes of autoimmune myocardial lesion in patients with myocarditis is disbalance between pro-inflammatory and anti-inflammatory cytokines, leading to the loss of functional tolerance to certain myocardial autoantibodies, and activation of autoreactive T-lymphocytes, which are able of myocardial cytotoxicity. The authors have conducted studies, which have shown that malignant myocarditis is characterized by abrupt increase of TNF-alfa and IFN-alfa basal serum levels, while at the same stage of the disease in patients with benign myocarditis only moderate increase of TNF-alfa level is observed, and IFN-alfa concentration is within normal limits. Abrupt increase of the cytotoxic activity of lymphocytes, activated by IL-2 (LAK-cells), is a feature of malignant myocarditis. In this category of patients the studies have found pronounced disbalance between cytokine serum levels, with abruptly increased levels of both cytokines which activate cytotoxic reactions (TNF-alfa, IFN-alfa, IL-1 alfa, IL-1 beta, IL-12), and cytokines which stimulate humoral immunity (IL-4 and IL-6), and increase of antimyocardial autoantibody concentration. Further research into the role of cytokines in myocarditis pathogenesis would allow grounded cytokinotherapy and application of immunomodulators, activating cytokine production.

[Glial tumors of the brain: classification, present-day aspects of immunopathogenesis and immunogenetic diagnostics].

Glial tumors of the brain present a complicated problem in neurosurgery due to the peculiarities of their growth biology, their prevalence, infiltrative growth and diagnostic difficulties. The complexity of treatment, which includes surgical removal of a tumor, radiotherapy, chemotherapy, and immune correction, makes the problem even more topical. Modem classification of glial neoplasms is of no less importance, because treatment depends on the histologic structure of a tumor. Thereupon, the authors of the article pay special attention to the modem WHO classification of gliomas. As immune diagnostics and treatment are indispensable in modern neurooncology, application of neurospecific oncologic markers is being widely introduced into clinical and scientific practice in many scientific centers. However, there are no objective tests to verify the diagnosis of astrocytic glioma with reliable accuracy. Hereditary syndromes associated with glial tumors have been distinguished and are being studied. The most interesting and promising direction is research into genetic anomalies in patients with glial processes.

[Achievements and prospects of clinical abzymology].

Catalytic autoantibodies (abzymes) are autoantibodies that are potentially ready to realize certain effects in the organism, first of all antibody-mediated catalysis and cytotoxicity. Natural abzymes with protolytic (protabzymes) and DNA-hydrolyzing DNA-abzymes) activity are of the greatest interest. The most impressive example of the catalytic activity of protabzymes is hydrolysis of specific proteins, revealed in patients with autoimmune diseases, such as bronchial asthma (vasoactive intestinal neuropeptide), autoimmune thyroiditis (thyroglobulin), multiple sclerosis (myelin basic protein), and autoimmune myocarditis (cardiomyosin). The pathogenic role of DNA-abzymes is not quite clear yet. However, it has been proven that they present a powerful regulator of apoptosis and other cytotoxicity mechanisms in systemic autoimmune diseases and tumors. The most promising is use of abzymes as illness activity markers, and as therapeutic agents capable of catalyzing specific proteins or activating antitumoral chemotherapeutic preparations.

[Introduction to medical abzymology: the present and the prospect].

The last decade of the 20th century was marked by appearance of a new field at the interfaces between modern immunology, biochemistry, molecular biology, and pathology. The name of this field, abzymology, descends from the terms "antibody" and "enzymes". The new direction is concerned with obtaining and study of abzymes, catalytic antibodies. This new class of biocatalyzers may be obtained artificially, by immunization of animals with transition state analogs, or by induction of anti-idiotypic antibodies to enzymes. By now, these approaches have allowed biocatalytic conversions of dosens of biochemical reactions using antibodies. Many of them do not have natural enzymatic "analogs". This have provided a real possibility to obtain "catalytic vaccines", moleculas of immunoglobulins, capable of destroying such harmful conpounds as pesticides, toxic agents, and narcotics. Abzymes may also be obtained from human and animal serum in various pathologic conditions. Nowadays it is obvious that the highest level of catalytic activity is observed in autoimmune conditions. Most likely, this aspect of abzymology may be considered from the point of view of prospects in diagnostics, as well as to be one of the aspects of studying abzymology.

[DNA-abzymes in rheumatoid arthritis: pathogenetic and clinical significance].

AIM: To study possible pathogenetic role and clinical significance of DNA-hydrolysing autoantibodies (autoAB) or DNA-abzymes in patients with rheumatoid arthritis. MATERIAL AND METHODS: Prevalence of DNA-abzymes and their catalytic activity were studied in 400 patients with rheumatoid arthritis (RA) and 88 healthy donors matched by age and gender. RESULTS: Associated with DNA-binding autoAB DNA-hydrolysing activity was detected in 41.5% cases of RA. DNA-abzymes were maximally active in men with rheumatoid factor (RF) and women without RF, while it was minimal in men without RF and women with RF. By catalytic activity there was no significant differences between patients with RF and without it. The highest catalytic activity of DNA abzymes was detected in patients with distinct extraarticular pathology. DNA-abzymes were also active in patients with x-ray stage III-IV of the disease in association with high prevalence of catalytic autoAB. DNA abzymes were also active in patients with RA activity stage II and III. CONCLUSION: It is possible to use DNA-abzymes in clinical practice for monitoring of the disease activity in RA.

[Tumor necrosis factor alpha and interferon alpha in patients with myocarditis.].

AIM: To elucidate the role of tumor necrosis factor alpha (TNF alpha) and interferon alpha (INF alpha) in pathogenesis of infectious-immune myocarditis (M) and myocarditic cardiosclerosis (MCS). MATERIAL AND METHODS: Patients with infectious-immune myocarditis (n=27) and myocarditic cardiosclerosis with symptoms of heart failure (n=19). Blood levels of TNF alpha and INF alpha were measured by immune enzyme analysis. RESULTS: Mean levels of INF alpha and TNF alpha in patients with M (75+/-47 and 304+/-102 rg/ml respectively) were significantly higher than in patients with MCS and healthy donors (31+/-14 and 83+/-39; 38+/-18 95+/-58 rg/ml, respectively, p<0.05). Levels of INF alpha and TNF alpha significantly differed between patients with benign and malignant course of M (99+/-46 and 354+/-100; 39+/-10 227+/-42 rg/ml, p<0.05). Phytohemagglutinine induced TNF alpha production by leucocytes in patients with malignant M (1432+/-515 rg/ml) was higher (p<0.05) while in patients with benign M (131+/-54 rg/ml) lower (p<0.01) than in healthy donors (255+/-98 rg/ml). Patients with malignant compared with those with benign course of myocarditis had lower ejection fraction (25.9+/-12.1 and 42.5+/-11,2%, respectively, p<0.003) and higher inhospital mortality (16.6 and 0%, respectively). CONCLUSION: It is most probable that factors of regulation of INF alpha and TNF alpha production occupy an import place among mechanisms of malignant transformation of myocarditis.

[The use of fermented milk drink with topinambur for the treatment of Helicobacter infection in the liquidators of Chernobyl nuclear power plant accident].

For estimation of anti-helicobacter activity of new fermented milk drink with Helianthus tuberosus called 23 liquidators of crash after-effects of Chernobyl Nuclear Power Plant were observed. Helicobacter pylori were determined in faeces by PCR-method, immune system readings--by first-level methods. The drink was applied in 200 grams portions during 18 days. After therapy appearance of Helicobacter pylori in faeces was reduced for 3 times. Reading of HCT-test, increased up to 16% in the first test before therapy, was normalized after therapy. Listed figures prove anti-helicobacter activity of fermented milk drink with Helianthus tuberosus.

Anti-DNA autoantibodies reveal toxicity to tumor cell lines.

Cytotoxicity of anti-DNA autoantibodies from sera of SLE and CLL patients was assayed on permanent cell lines L929, HL-60, Raji, and K562. L929 cells appeared to be the most sensitive to antibody treatment. DNA-hydrolyzing properties of the same autoantibody preparations were analyzed in parallel. The data obtained outlined the correlation between cytotoxicity and DNA-hydrolyzing properties of these autoantibodies. It was shown that treatment of the cells with cytotoxic anti-DNA autoantibodies induced internucleosomal DNA fragmentation and Annexin V binding to the cell surface characteristic of apoptotic pathway of cell death. A time-dependent profile of antibody-mediated toxicity to L929 cells suggested recruitment of at least two distinct mechanisms of cell death. The first peak of cell death observed in 3 h of incubation was completely inhibited by preincubation of cells with caspase inhibitor YVAD-CHO, while the second increase in cell mortality (18-30 h) persisted. Possible mechanisms for anti-DNA autoantibody cytotoxicity are discussed.
