1H NMR studies on human plasma lipids from newborn infants, healthy adults, and adults with tumors.

The 1H NMR spectra of the lipid region of human plasma from healthy adults, neonates, and patients with malignant and nonmalignant tumors have been recorded on a JNM-GX400 FT spectrometer operating at 399.6 MHz for protons. The chemical shifts of methylene and methyl groups of plasma lipids were measured with respect to the higher field component of the methyl proton resonance of the lactate molecule. The results show that there are changes in the chemical shifts of the methylene proton resonances among the plasma from healthy adults, adults with tumors, and neonates. The shifts observed in the case of cancer patients and neonates are in the direction opposite to the shift measured from the plasma of healthy adults. Thus, the observed changes cannot be explained by the activity in the cell proliferation of tissues which is high in the cases of both healthy neonates and patients with malignant tumors, but they most probably reflect the different lipoprotein compositions of neonates, healthy adults, and adults with tumors.

Trimethoprim resistance of Escherichia coli in outpatients in Finland after ten years' use of plain trimethoprim.

Development of trimethoprim resistance among Escherichia coli collected from urine samples in three areas in Finland was studied in 1978-1984. Three different trends of development of resistance were found: in the Turku area resistance has increased evenly during 1978-1982 from 5.4% to 10.1%, but thereafter a plateau seems to have been reached; in the Helsinki area resistance increased rapidly from 2.9% in 1980 to 11.1% in 1984, possibly due to the spread of a trimethoprim resistance transposon Tn7, which occurred significantly more often among E. coli strains in this area than in the Turku area; in the Rovaniemi area resistance has been at a plateau level, between 3.1 and 5.7%, during the whole study. No clear correlation between the consumption of trimethoprim and the level of resistance was found. The frequency of trimethoprim resistance in E. coli isolated from patients less than 65 years was 5.2% and in those isolated from patients greater than or equal to 65 years 14.7%.

Ampicillin concentrations in middle ear effusions in acute otitis media after administration of bacampicillin.

The penetration of ampicillin into middle ear effusions after oral administration of 800 mg bacampicillin to patients with acute otitis media was studied and compared with the serum levels. One hour after administration a mean (+/- SEM) concentration of 2.4 +/- 0.4 micrograms/ml ampicillin was found in the effusions and the mean serum level was 7.7 +/- 0.8 micrograms/ml. After twelve hours a mean concentration of 0.6 +/- 0.2 micrograms/ml was found in the middle ear effusions and 0.1 +/- 0.03 micrograms/ml in the serum. The high concentration of ampicillin in middle ear effusions after twelve hours suggests that a twice daily dosage of bacampicillin should be given in acute otitis media.

Acute Disease

Lysozyme activity and immunoglobulins in middle ear effusion fluid in acute purulent otitis media and in otitis media with effusion.

147 samples of punctured middle ear effusion fluid from cases of otitis media with effusion and 150 samples from patients with acute purulent otitis media were tested for lysozyme activity. In otitis media with effusion the concentration was 182.0 U/ml, in acute otitis 433.8 U/ml. The lysozyme concentration in otitis media with effusion depended upon the nature of the effusion. Serous fluid showed an activity of 124.8 U/ml and mucoid 311.6 U/ml, respectively. In culture-positive cases of acute otitis media the lysozyme level was 423.4 U/ml. Culture-negative cases showed about the same concentration, 438.3 U/ml. The possible role of lysozyme in defence systems of the middle ear is discussed.

[Acidocillin concentration in middle ear exudate].

Acidocillin levels were determined in the middle ear exudates and serum of patients with acute exudative otitis media after oral administration of the drug. Acidocillin rapidly diffused exudate into the ear and its level in the exudate in 1 hour was 0-1.7 gamma/ml. At the same time the serum levels ranges within 4 to 25 gamma/ml. acidocillin remained longer in the ear exudate than in the serum. In 12 hours 0.2 to 1.3 gamma/ml of acidocillin was still found in the exudates, while there was no antibiotic in the serum.

Sephadex G-200 chromatographies of the liver cell sap of rabbits, 1, 3 and 6 days after an injection of 3H-folic acid activity (FAA) into the circulation, were used in an attempt to study the binding of this labelled FAA to liver proteins. The labelled FAA was quickly accumulated in the liver, and in the cell sap it was grouped chromatographically to two maxima, which corresponded to the two maxima of microbiological FAA (L. casei). The maxima were eluated in the chromatography together with cell sap proteins. The possible coupling of FAA to proteins in the liver is discussed.

Gram-positive, motile, cluster-forming cocci as a cause of urinary infection.

One hundred and thirteen strains of motile, Gram-positive, catalase-positive, cluster-forming cocci were isolated from patients with urinary infection attending a private surgery. They constituted 1% of the total 11 302 positive cultures. The biochemical characteristics and the drug sensitivities of the strains are described. The significance of motility for organisms which cause urinary infections is pointed out.At the present time the organisms isolated are orphans in the controversial classification of staphylococci and micrococci.

Serum folic acid activity (L. casei) in Sephadex gel chromatography.

The folic acid activity (FAA) of the protein zone in DEAE Sephadex A-50 chromatography of normal serum showed the following distribution: most (about 40%) was eluted with protein zone IV (alpha-2-macroglobulin), about 33% with protein zones II-III (transferrin), and the balance with zone V (albumin). In healthy human subjects the variations were relatively small. The repeatability of the chromatography in an individual was good.Towards the end of pregnancy the binding of FAA to protein zones II-III increased, while that to zone IV decreased. The return to normal started a few days after delivery. The binding to zones II-III was greatly increased in patients with pernicious anaemia but no neuropathy. It became normal during B(12) therapy. If the patient had neuropathy, his FAA chromatogram was close to normal. During therapy the amount of unbound FAA decreased rapidly. The use of diphenylhydantoin increased the binding of FAA to protein zone I and to zones II-III. In these patients the binding to protein zone IV was slighter than normal.
