[Enzymatic debridement in venous ulcus cruris. Report of a multicenter clinical comparative study with various enzyme preparations].

In a multicenter trial (comprising 258 patients in eight centers) three topical preparations for enzymatic debridement, which are widely used in our country, have been compared in the treatment of leg ulcers. Preparation A is a combination of bovine fibrinolysin and desoxyribonuclease (but without antibiotic); preparation B is a combination of clostridiopeptidase and chloramphenicol and preparation C of trypsin and framycetin sulphate. The results showed no significant difference in the effects of treatment between the preparation. Preparation A however was much better tolerated; preparation B and C induced a lot of side effects. This means that antibiotics in preparations for enzymatic debridement are not of real advantage.

[Chondroid syringoma].

Chondroid syringoma is a rare benign appendage tumor of the skin. Histologically, the tumor is characterized by the presence of sweat gland elements surrounded by a cartilage-like material. The lesions are most frequently located around the nose. Clinically, they appear as solitary, firm, circumscribed, slowly growing intracutaneous or subcutaneous nodules. In the majority of cases the clinical diagnosis is either basal cell epithelioma or cyst. This paper calls attention to the characteristic clinical features of chondroid syringoma as observed in five patients.

[Proliferating trichilemmal tumor].

The proliferating trichilemmal tumor is a rare benign neoplasm of the outer roof sheath of the terminal hair follicle. It occurs almost exclusively in elderly women as a solitary tumor of the hairy scalp. In a woman 71 years of age a proliferating trichilemmal tumor developed during two years. A large cornu cutaneum of the parietal scalp was present. The clinical and histopathologic features can be mistaken for squamous cell carcinoma.

[Seborrheic pemphigoid (Schnyder)].

Seborrheic pemphigoid first described by Schnyder is considered to be a clinical variant of bullous pemphigoid which resembles pemphigus erythematosus. Ruptured bullae and erosions covered with crusts involving predominantly the median parts of the trunc characterize the clinical picture. Histologic and immunofluorescence findings substantiate the diagnosis of pemphigoid. The clinical, histopathologic and immunofluorescence features as well as the course of the disease in a 74 year old woman are described.

[Closure of facial defects using full-skin transplants from the face].

Full thickness skin grafts from inner or outer areas of the face to cover defects in the face following tumor excision achieve functional and cosmetic results comparable to those methods using flaps. In contrast to this method grafting of full-thickness skin can be performed under local anesthesia on an out-patient basis with low risk and inconvenience for the patient. The graft excision should be performed like a face-lifting: remove cosmetically disturbing folds and avoid visible scars. Localization of possible donor sites, pre-operative planning, surgical management, special features and results acquired with this technique are described.

Serum IgE-concentrations in patients with contact sensitization.

In 123 patients with contact sensitization (cs), of whom 23 had positive histories of atopy and/or positive prick tests (csa), the serum IgE-concentrations were determined and the following results were obtained: 1. A remarkable dispersal of IgE-concentrations was noted in cs--patients (1-3080 U/ml); 12% of them displayed IgE-concentrations of more than 1000 U/ml, even though they had no histories of clinical manifestations of atopy, liver of kidney diseases, or parasitic infestations. Their mean IgE-value of 396 U/ml +/- 60 differed considerably from the one of a comparable control group of healthy individuals. No statistically significant differences were noted between the IgE-levels, the age of our patients, the type and number of contact allergens, and the duration of skin lesions. 2. On an average, there were higher IgE-levels noted in cases of acute and extensive skin lesions than in cases of subacute to chronic, localised skin lesions. Controls of the serum IgE-concentrations and histologic examinations by means of immunofluorescence suggested that, depending on the individual patient, skin lesions might lead to an (unspecific?)activation of IgE-synthesising round cells. 3. In csa-patients with sensitization to metal, the skin lesions were of localised subacute to chronic character, while skin lesions of acute and extensive character were exceptional (5.9%).
