Treatment of IUD related menorrhagia by indomethacin.

Indomethacin, prostaglandin synthetase inhibitor was tested in 9 women wearing a lippes loop IUD. These women suffered from excessive bleeding with a mean menstrual blood loss of more than 100 ml. The effect was studied by quantifying menstrual blood loss in a double blind 4 periods crossover study. The mean percentage reduction during treatment was of 57 percent. No significant placebo effect was observed. The effect of indomethacin support the contention that IUD's associated menorrhagia, is connected to an abnormal prostaglandin synthetase in the endometrium.

Zona-free hamster ovum penetration assay as a screening procedure for in vitro fertilization.

Twenty couples with primary sterility due to mechanical factors entered our program of IVF. In addition to routine sperm evaluation, an SPA was performed in all cases. Whenever the SPA was pathologic, none of the human ova were fertilized in vitro, while 77% of the patients with a normal SPA had IVF of at least one human ovum. The good correlation between pathologic SPA and failure of IVF stresses the clinical value of this assay. The use of the SPA for patient selection for IVF is suggested.

Severe fetal distress with hydramnios due to chorioangioma.

A case is described of antenatal detection of a placental mass by ultrasound in a patient with hydramnios due to chorioangioma and who had a sinusoidal-like pattern and positive oxytocin challenge test (OCT). The importance of frequent fetal heart rate monitoring in cases of suspected chorioangioma is emphasized.

In vitro fertilization and embryo transfer--legal and religious aspects in Israel, patient selection, and a modified technique for oocyte collection.

In vitro fertilization (IVF) and embryo transfer (ET) have recently become an accepted treatment modality for patients with mechanically caused infertility. The first series admitted to our program comprised 42 patients with confirmed mechanical infertility. The method of patient selection and the techniques of laparoscopy, follicular aspiration and oocyte recovery are described. From 26 laparoscopies, a total of 39 oocytes was recovered. Normal cleavage of 10 oocytes was obtained; they were transferred into the uterus at the 4- to 16-cell stage. One pregnancy was obtained. An IVF and ET program involves certain ethical, legal and religious questions, which have special implications in Israel: foster mothers and the use of donor sperm are forbidden. Only married couples may enter the program. Indications, methods, success rates and method failure are discussed.

Conservative management of suspected prolactin secreting pituitary adenoma during pregnancy.

Seventeen women with prolactin levels of 100 ng/ml and above suspected of harboring prolactin-secreting pituitary adenoma, from the basis of this study. Ten patients had radiological signs of an adenoma while in 7 the radiological criteria for such a diagnosis were not fulfilled. Ovulation and pregnancy were induced with bromocriptine in all 17 patients. They were carefully observed during pregnancy and following delivery. All gave birth to full-term babies after uneventful pregnancies, except for one patient who experienced intrauterine fetal death at 31 wk of gestation. It is our policy that women with suspected intrasellar prolactin-secreting pituitary adenoma be allowed to conceive and give birth without previous surgical intervention. The patient should be closely followed during pregnancy for clinical symptoms of enlargement of the tumor, including periodic visual field examinations. In cases of neurologic or ophthalmologic complications, surgery or bromocriptine administration without interruption of pregnancy is advocated, or if lung maturity is achieved, delivery should be induced.

Establishment of a fetoscopy and fetal blood sampling program in Israel.

The development of fetoscopy and the ability to obtain fetal blood samples have made possible the prenatal diagnosis of diseases not diagnosable by amniocentesis. Foremost among these disorders are the hemoglobinopathies. We report our experience in establishing the first program for fetoscopy and fetal blood sampling in the Middle East. The results demonstrate that the experience and training required for a fetoscopy and fetal blood sampling program can be obtained in about one year. During that year, 23 couples who fetuses were at risk for a hemoglobinopathy learned the genetic status of their fetuses.

Two conceptions in a 45,X woman.

We present a 31-year-old woman with a 45,X chromosome constitution who had had two miscarriages. This report brings to 11 the number of presumably non-mosaic Ullrich-Turner syndrome patients who have achieved pregnancy. A review of the literature indicates an increased incidence of chromosome abnormalities and a high rate of fetal death in offspring of such patients. However, in light of the fertility in these patients, genetic counseling should be reevaluated and perhaps amniocentesis recommended in successful pregnancies.

Quintuplet pregnancies.

Five quintuplet pregnancies, following induction of ovulation with clomiphene and HMG-HCG, are the subject of this communication. In 3 women, pregnancy was associated with ovarian overstimulation, and two patients required emergency surgery because of torsion of an ovarian cyst. Two women aborted in mid-trimester, while the other 3 delivered by cesarean section at 33-35 wk gestation. 15 babies were born, weighing 700-2200 g. 8 infants suffered from respiratory distress syndrome and 5 babies were born with correctable malformations. 13 newborns survived, and all are mentally and physically well-developed. Four of the 5 women conceived again. The management of the patients during multiple gestation and the outcome of pregnancy are discussed.

The effect of intrauterine devices containing zinc and copper on their levels in serum.

Serum levels of copper and zinc were determined in 11 health women in whom Latex Leaf intrauterine devices (IUDs) containing copper and zinc had been inserted. Patients with low levels of serum copper or zinc before insertion usually had increased levels of either of these metals while using the device, but they did not exceed the upper limits of normal values. When the group was considered as a whole, the serum levels of zinc showed a slight tendency to increase with duration of IUD use, whereas there was no statistical difference between the serum levels of copper before and after insertion of the IUD.

Preliminary report on the use of the combined multiload contraceptive intrauterine device (ML Cu-250).

The multiload contraceptive intrauterine device (IUD) (ML Cu-250) is made of polyethylene and contains copper wire which is wrapped around the stem, providing 250 sq mm of exposed copper surface area. The IUD was inserted in 294 women comprising 1884 women-months of use. Four pregnancies and three expulsions occurred. In 13 instances the ML Cu-250 IUD was removed for medical reasons (side effects). The net cumulative pregnancy rate for a 12-month period after insertion was 3.9% and the expulsion rate was 1.6%; the medical removal rate was 8.2%. No perforations or tubal pregnancies occurred. Our results with the ML Cu-250 was compared with our previous experience with other IUDs. It seems that the comparatively low pregnancy, expulsion, and medical removal rates render the ML Cu-250 a good device for clinical use.

Sodium nitroprusside for aneurysm surgery in pregnancy. Report of a case.

A 25-year-old woman in the 7th month of pregnancy was operated upon for a ruptured cerebral aneurysm. Sodium nitroprusside was used to achieve effective controlled hypotension. Signs of foetal distress were transient. Subsequent delivery was spontaneous and uneventful. Development of the child at 2 yr was satisfactory. The various physiological aspects and the action of nitroprusside on both mother and foetus are discussed.
