Loss of antibodies against hepatitis C virus in HIV-seropositive intravenous drug users.

A time-point survey of 262 ex-intravenous drug users (IVDUs) on methadone treatment showed serological evidence of hepatitis C virus (HCV) in 64%, hepatitis B virus in 74% and HIV in 36%. Analysis of previously collected frozen stored sera of the same patients showed that 31 further IVDUs had had anti-HCV antibodies in the past and had lost them in a time-dependent fashion. Most HCV seroreversion was detected in HIV-positive people: 20 out of 85 in the HIV-positive group versus 11 out of 18 in the HIV-negative group.

Hypohaptoglobinaemia as an epidemiological and clinical indicator for malaria. Results of two studies in a hyperendemic region in West Africa.

Hypohaptoglobinaemia is a common phenomenon in tropical countries, where it is probably due to malaria-induced haemolysis. Two studies were carried out in a hyperendemic zone of West Africa to test its specificity and usefulness as an epidemiological indicator for measuring malaria endemicity. The first study evaluated the prevalence of hypohaptoglobinaemia before and after courses of antimalarial chemotherapy of varying duration. The second monitored haptoglobin levels in an untreated population during a whole year to compare its seasonal variations with those of several classic indicators of malaria. These studies suggest that in regions where malaria is endemic the prevalence of hypohaptoglobinaemia could be as useful an indicator as the parasitic index but would be much easier to establish and to monitor.

Measurement of serum haptoglobin as an indicator of the efficacy of malaria intervention trials.

Serum haptoglobin levels were measured by an enzyme-linked immunosorbent assay in Gambian children who participated in 3 malaria intervention trials with untreated or impregnated bed nets. In one study, in which a significant effect on clinical malaria was observed, the mean serum haptoglobin level was significantly higher in the intervention than in the control group. In the other 2 studies, in which no significant protection was observed, mean haptoglobin levels were similar in intervention and control groups. Measurement of serum haptoglobin may provide a useful indirect measure of the effectiveness of malaria control programmes.

Acute HIV infection.

In this short review, clinical manifestations associated with primary HIV infection (PHI) and their impact on progression to AIDS will be described. High levels of viral replication of viruses with a restricted heterogeneity and decreased CD4/CD8 cell ratio are hallmarks of PHI. Within weeks to months following PHI, a marked decrease in viremia and an increase of CD4 cells which stabilise at levels below normal values occur. Early decrease of viremia to low levels, low proviral levels, stabilisation of CD4 cells to values close to the normal range postpone the development of AIDS. Tentative goal of early antiviral therapy in PHI might be, by decreasing viral load, to restore CD4 cell levels to normal value and to transform the majority of PHI patients into long-term non-progressors. Selection of therapeutic regimens, duration of antiviral treatment and validation of appropriate virological and immunological markers are the key issues in the context of PHI treatment. These issues and the results of therapeutic interventions with mono and bitherapy in PHI as well as combined drug regimens for future studies are presented and discussed.
