Metastasis of prostate adenocarcinoma to testis.

Metastatic carcinoma to the testis is very rare. Metastasis of prostate adenocarcinoma to testis was detected incidentally after bilateral orchiectomy for hormonal management of metastatic prostate carcinoma. The metastatic lesion was not identified in physical examination or in macroscopic dissection of the testis after surgery. Microscopy revealed an adenocarcinoma which, given the history of the patient and a positive immunohistochemical stain for PSA, was identified as metastatic prostatic adenocarcinoma.

The effect of nursing or rampant caries on height, body weight and head circumference.

In this study, the effect of nursing bottle caries and rampant caries on height, weight and head circumference was evaluated. For this purpose, 126 children, aged 3 to 5 years old, who have nursing or rampant caries were selected. One hundred twenty-six children with no caries and similar age and sex were matched as a control group. When the children who had rampant or nursing caries were evaluated in the direction of the mean weight, it was corresponding to mean weight between 25th and 50th percentiles. The mean percentile weight for control group were corresponding to mean weight between the 50th and 75th percentiles. Of the nursing or rampant caries children, 7.1% weighed less than 80% of their ideal weight, compared with only 0.7% of the control group children. When the children, who had rampant or nursing caries, were evaluated in the direction of the mean height, it was corresponding to mean height between 10th and 25th percentiles. The mean percentile height for control group were corresponding to mean height between the 25th and 50th percentiles. When the head circumference is evaluated, there was no significant statistical difference between the two groups. Since the height and weight of the control group showed a higher percentile category than the nursing or rampant caries group, (P<0.001), it can be stated that rampant or nursing caries may correlate with adversely affected growth of the body.

Carcinoma of the fourth part of the duodenum: report of a case.

Primary carcinoma of the duodenum is rare, accounting for only 0.35% of all gastrointestinal carcinomas, and carcinoma of the fourth part of the duodenum constitutes approximately 10% of duodenal carcinomas. Since the predominant symptoms of tumors in this part of duodenum are related to upper intestinal obstruction, the diagnosis is usually made late. We report the case of a 66-year-old man who presented with anemia, weight loss, and upper intestinal obstruction, and was found to have advanced primary carcinoma of the fourth part of the duodenum by upper gastrointestinal series and computed tomography (CT). A review of the literature indicates that survival, which is related to nodal status, the grade of the tumor, and surgical results, seems to be longer in patients with carcinoma in this part of the duodenum compared to those with carcinomas in other parts of the duodenum. Moreover, segmentary resection has been reported to achieve a favorable outcome.

The effect of toxic doses of cholecalciferol (vitamin D3) on the serum zinc levels in rats.

Zinc is a trace element important to bone mineralization as well as, in general, nutrition. It is known that cholecalciferol (vitamin D3) affects bone metabolism. In this study, toxic doses of vitamin D3 were injected subcutaneously (25 micrograms/d) to rats for 5 wk. It caused a significant increase in serum zinc levels (p < 0.02). On the other hand, no significant increase was detected in the other groups. Excessive amounts of vitamin D3 caused bone breakdown and increased the levels of zinc in blood.

Effect of toxic doses of cholecalciferol (vitamin D3) on alkaline phosphatase levels in serum and gingival tissues of rats.

Toxic doses of cholecalciferol (vitamin D3) were injected subcutaneously (25 micrograms/day) into rats for 5 weeks, causing a significant increase of serum alkaline phosphatase (ALP) levels (p less than 0.01). Gingival tissue alkaline phosphatase levels were also elevated (p less than 0.01). On the other hand, a significant difference was detected between serum calcium levels in experimental and control groups (p less than 0.02).

[Dinitrochlorobenzene test in patients with chronic nephritis].

Dinitrochlorobenzene (DNCB) test was applied to 22 patients with chronic nephritis. Only 22% of the patients gave positive results. This was concluded to be due to the high NPN levels. It was demonstrated that this inhibition is transitory and the normal responses appear 12 hours after the return of the NPN to normal levels. When it was considered that some patients with high NPN levels showed normal DNCB reactivity and delayed rejection time, the chronic nephritis patiens reacted to organ transplants despite their normal NPN levels. It was suggested that it is one of the components of NPN which should be responsible for the above mentioned inhibition.

Primary hepatic carcinoid tumor: dynamic CT findings.

Most carcinoid tumors involve the gastrointestinal tract or respiratory system. Primary hepatic carcinoid tumor is a rare entity. A 46-year-old woman presented with abdominal pain, vomiting, and diarrhea. She was diagnosed with primary hepatic carcinoid tumor based on radiologic and laboratory findings, namely a mass in the fifth segment of the liver and markedly elevated levels of 5-hydroxyindole acetic acid in the urine. Histologic and immunohistochemical findings of the resected liver segment showed a malignant carcinoid tumor of the liver. This case is of interest because of the rarity of this neoplasm. This report describes the case and reviews the clinical features, radiologic findings, and treatment in previously reported cases.

Burns in southern Turkey: electrical burns remain a major problem.

In Turkey, burns represent a relatively small number of injuries overall, but they continue to be a major public health problem. Our aim in this study was to identify risk factors that affect outcome in burn patients hospitalized in the southern part of our country, with special emphasis on electrical burns. The database for 109 burn patients who were admitted to our burn center from April, 2000, through August, 2001, was retrospectively analyzed. Electrical injury was the cause of burn in 23 (21%) of the 109 cases. The burn causes differed among age groups and between the sexes, with males constituting 95% of the electrical burn patients. The mortality rate for the electrical burn group was lower than the rate for the rest of the burn patients (1/23 vs 17/86, respectively; P <.001); however, the opposite was true for complication rate (10/23 vs 5/86, respectively; P <.001), cost of treatment (8351 US dollars vs 5122 US dollars, respectively; P =.009), and length of hospital stay (39.9 vs 26.2 days, respectively; P < 0.001). The rate of electrical burn injury in Turkey has changed very little in the past two decades. This underlines the need for stronger efforts aimed at prevention, such as better public education and strict regulations regarding the distribution and use of electricity.

Significance of p53 and bcl-2 immunoexpression in the prognosis of laryngeal squamous cell carcinoma.

This study was undertaken to determine whether p53 or bcl-2 immunoexpression has any significance in the prognosis of laryngeal squamous cell carcinoma. Eighty-four cases of laryngeal squamous cell carcinoma at different pathological stages and locations were studied. All patients were treated surgically first, and followed up for 6-96 months (mean, 35 months). p53 and bcl-2 immunoexpression in tumour cells, evaluated by image analysis and histoscore, were found in 71.4% and 46.4% of tumours, respectively. Glottic location was correlated with significantly longer survival than tumours at other locations. Pathological stage and histological grade of tumour were also significant predictors of survival. Neither p53 nor bcl-2 immunoexpression was found to be predictive of post-operative survival. Only transglottic tumours expressed significantly high levels of bcl-2. Both methods used to evaluate p53 and bcl-2 immunoexpression correlated well with each other. This study suggests that immunoexpression of p53 and bcl-2 is not a significant prognostic predictor in laryngeal squamous cell carcinoma.
