Hydrometrocolpos in neonate due to distal vaginal atresia.

Hydrocolpos is the result of vaginal obstruction and can become an emergency in the newborn period. The treatment of imperforate hymen is well defined, but the treatment of vaginal atresia is more complex. We encountered two cases of hydrocolpos secondary to distal vaginal atresia, that were operated on in the first days of life. One baby had distal atresia without persistance of urogenital sinus. Surgery combining abdominal perineal approaches and a posterior vaginoplasty was carried out. The second baby had hydrocolpos with persistance of urogenital sinus. Drainage through the sinus was unsuccessful because the baby developed sepsis by trapping urine in the uterus. Finally an abdominoperineal vaginal pull-through was successfully done. The embryology and literature are reviewed. The classification, indications and surgical technique are discussed.

The anterior perineal approach for pull-through operations in high imperforate anus.

Recent widespread interest in a new approach to imperforate anus is ample testimony to the difficulty of the classical operations and to the unsatisfactory results. In the early seventies, Mollard began to use an anterior perineal approach to high imperforate anus as an alternative to the sacrococcygeal pull-through. It entails a semicircular retroscrotal incision allowing direct access and visualization of the urethra and puborectalis sling. Since 1976 we have switched to the anterior perineal approach. We now have experience with 30 patients, 27 males and 3 females. Two of the females had complicated cloacal anomalies, and three teenage patients were reoperated for incontinence following previous unsuccessful pull-throughs. Of the 27 primary operations, there were 17 with an uneventful early postoperative course. There were two major and eight moderate or minor complications. There was no mortality. A 3-year follow-up is available in 14 patients. The results are good in 8, fair in 4 and poor in 2. The advantages of this technique are threefold: it allows clear visualization and precise identification of the puborectalis sling; the dissection proceeds parallel to and with constant vision of the urethra, avoiding injury to it; and it obviates the need to change the position of the patient during the procedure. This operation is easier and safer than the sacral approach, it avoids cutting through the puborectalis sling as recently described, and, finally, it respects the criteria established by Stephens.

Thymectomy for myasthenia gravis in children.

Myasthenia gravis is rare in childhood. In the last 12 years, we have treated eight children under 16 years of age for this disease. There were six girls and two boys, a ratio of 3:1. Age of presentation of symptoms ranged from 10 to 16 years (mean 11 3/4). There was no significant family history and there were no significant associated diseases. Antibodies were measured in six patients and were all negative. All patients were given cholinergic drugs and five had corticosteroids as well. Three underwent tracheostomies because of severe respiratory insufficiency. There was clinical deterioration or no improvement in spite of increasing doses of medication; hence, thymectomy was performed in all after an average duration of symptoms of 12 months (range 4 to 17). All thymectomies were done through median sternotomy. There was no postoperative morbidity or mortality. No thymoma was found. Four had lymphoid hyperplasia, in three, the thymus looked normal and in one it looked very atrophic. Dramatic amelioration with eventual complete recovery was seen in six, with an average follow-up of 7.3 years (3 to 11) after thymectomy. Two patients are only one year after thymectomy and are improving progressively. We recommend early thymectomy in myasthenia gravis of childhood on the basis of very encouraging results.

[Comparative in vitro tests of silver sulphadiazine and zinc sulfadiazine swabs on burn patients].

The antibacterial effect of silver sulphadiazine is compared with that of zinc sulphadiazine in three in vitro tests with 14 different bacterial colonies. Our results demonstrate that silver sulphadiazine is more efficient than zinc sulphadiazine as an antibacterial agent. As the local application of zinc promotes and accelerates wound healing, we suggest to apply first silver sulphadiazine and then zinc sulphadiazine in combined local therapy of burn wounds.

[Heterotopic autotransplantation of splenic tissue after traumatic rupture of the spleen - a solution after unavoidable splenectomy? (author's transl)].

Considering the increased incidence of infections and the danger of postsplenectomy sepsis (OPSI) a preservation of the ruptured spleen by suturing etc. is usually attempted today. When splenectomy is unavoidable autotransplantation of splenic tissue may preserve some degree of splenic function. We report our first successful case. 6 months after autotransplantation splenic tissue could be visualized by technetium 99 scintigraphy. Howell-Jolly bodies were absent in the peripheral blood and the immunoglobulins were normal.

[The incidence of congenital malformations. A five-year study carried out in a Paris maternity unit (author's transl)].

The congenital malformations arising out of 13,335 deliveries that occurred over the course of five years in the University Clinic of Gynecology and Obstetrics in the hospital of Saint-Antoine were recorded. Their overall frequency was 3.37%. Details are given of each malformation. Often malformations are linked to perinatal mortality (14.2% of stillbirths are malformed). There was no difference to be found between different ethnic groups. Pathological developments in previous pregnancies were not particularly frequent. Bleeding in the first trimester of pregnancy was significantly more frequent in congenital abnormalities.

[Pituitary agenesis in the rat: synthesis of testosterone in fatal testes].

Testosterone radioimmunological assays were made in testes of 19th day fetuses affected with pituitary agenesis. Testosterone was found very reduced but still present, indicating a basic synthesis, independent of hypophyseal stimulation. General apparatus of affected fetuses is grossly normal. Intervention of chorionic gonadotrophin is hypothesized.
