[Cannabis: experts agree more than they admit].

Cannabis is evermore present in society, whether within the general public or as a subject for scientific debate. Mass consumption of cannabis, for example, is stabilising around 22 percent of 18-year old who admit to having used it at least once during the previous month; however, this consumption rate falls off as they enter later adulthood. This article describes the emerging scientific consensus about the effects of this drug. The psychotropic effects of cannabis--the result of cannabinoids contained in its resin that activate specific receptors--include general euphoria, a mild release from inhibitions and, in certain cases, some distortion of sensory perception. Some patients also experience drowsiness, a stimulated appetite and anxiolysis, while others anticipate a more intense experience such as an altered state of consciousness. The toxicity of smoked cannabis and its acute, chronic secondary effects are described, as well as the problematic relationship between cannabis consumption and psychosis. The damages and toxic effects attributed to such consumption are presented via three, related themes: the growth of dependency, negative somatic consequences (including cognitive impairment and its consequences for driving an automobile, and damaging psychosocial effects. "Escalation theory" is criticized. In their conclusion, the authors cast doubt on the scientific grounds for penalisation of cannabis consumption, and recommend a "de-demonisation" of the drug. An analysis and discussion of the current penalties applied in Belgium are presented.

[Ethical questions about methadone].

The author approaches ethical questions raised by use of methadone in opiates addiction: reduction of drug phenomenon to a strict medical problem, illusion of technical mastery, responsibility of pharmaceutical engineering, medicine under repressive constraint, reduction of damages linked to prohibition. The communication ends on a question about management of drug problems which today encourages a healthy vision of consumption behaviour rather than a "good way of use".

[Particular enlightenments on crisis situations, recurrent crises, hospitalization and intervention networks].

This article proposes an in-depth look at certain clinical aspects that have been dealt with by a crisis centre based at a general hospital that is located in a popular district of Brussels. Following a brief history of the Saint-Pierre hospital, which allows the author to touch on the hospitality dimension of taking charge of psychosocial crises, the article focuses on recurrent crises (and their corollary, multiple hospitalizations) and analyzes their psychodynamic and institutional perspectives. The author then approaches the institutional framework as a stage where the different critical phases of the patient come into play. Supported by clinical examples, the author also examines the functions of recurrent crises with respect to the patient's emotional make-up and to his or her extensive hesitation when faced with "change". Finally, the author points out the advantages and disadvantages of the indispensable link with mobile intervenors.

[The role of methadone in the treatment of opiate addicts: current findings in the literature].

We discuss the use of Methadone in the treatment of opiate addiction. Withdrawal and maintenance treatment should be considered separately. Only the latter remains controversial. Various programs exist, mainly in the USA, and the study of their characteristics could enable us to define more precisely their respective indication criteria. The integration of specific and non-concurrent maintenance programs in a diversified but coordinated health care organization for drug addicts appears as the more appropriate and realistic measure in our country. Considering the extension of HIV-infection among this population, it becomes an urgency.
