Paraumbilical perforator-based pedicled abdominal flap for extensive soft-tissue deficiencies of the forearm and hand.

A new paraumbilical-based pedicled abdominal flap was used in 11 patients with extensive soft-tissue defects of the forearm and hand. With a relatively narrow pedicle, large flaps up to 5- x 14-cm can be raised. Another advantage of this flap is the comfortable position of the hand and forearm for the patient. The main disadvantage is the conspicuous abdominal scars like the other pedicled abdominal flaps.

Do absorbable sutures exacerbate presternal scarring?

We investigated cosmetic outcomes of the midline sternotomy incision. A randomized clinical trial was conducted in 60 patients who underwent surgery through a midline sternotomy incision. Patients were divided into groups A (n=30) and B (n=30). In addition, the incision line was also divided into 2 regions (upper and lower halves) in each group. In group A, the upper half of the skin was closed with absorbable 4-0 braided polyglycolic acid sutures (Sentesorb, Boz; Ankara, Turkey), and the lower half was closed with 4-0 nonabsorbable monofilamentous polypropylene suture (Monoplene, Boz), and vice versa in group B. Scar width and height were measured and photographed at the 6th postoperative month. In both groups, the lower part of the incision showed inferior cosmetic results, regardless of the suture material (P <0. 05). On the other hand, the upper part of the incision in group A (the area of absorbable polyglycolic acid sutures) was significantly more hypertrophic. We conclude that monofilament nylon sutures diminish the risk of hypertrophic scarring, in comparison with absorbable sutures. In the lower half of the sternotomy scar, increased tension and relative mobility of the skin over the xiphoid process lead to inferior cosmetic results, regardless of the suture material used.

The seven flap Z-plasty revisited.

Although multiple Z-plasties are widely used for burn contractures, the seven flap Z-plasty procedure has not gained wide acceptance in plastic surgery practice. However, the technique has the advantage of achieving more elongation than other Z-plasty techniques. The technique safely performed with satisfactory results in 31 cases.

Unusual combination of limb malformations in the same patient: brachydactyly with syndactyly and postaxial polydactyly of the hands and postaxial oligodactyly of the feet.

We report on a case with polydactyly, syndactyly and brachydactyly of the hands and oligodactyly of the feet, but no other anomalies and normal chromosome analysis. We compared the findings in our case with those of brachydactyly B, Fuhrmann syndrome and Haas-type syndactyly. However, it was not possible to suggest a syndrome diagnosis in the present case.

Diabetic ketoacidosis and rhino-orbital mucormycosis.

Mucormycosis often develops in immunocompromised patients, particularly in patients with diabetic ketoacidosis. Unless early diagnosis and treatment is established mucormycosis leads rapidly to death. A 38-year-old woman was admitted to the hospital with a severe diabetic ketoacidosis. Her clinical status improved in 4 days as a result of aggressive medical treatment. She has complained left cheek pain on the 10th day and had a swelling of her left cheek, facial edema, a black eschar on the palate and nasal cavity in association with visual disturbance and total ophthalmology in a short time. CT scan revealed left orbital cellulitis and pansinusitis. Excessive surgical treatment was performed and liposomal amphotericin-B, 4 mg/(kg day) was applied. Extensive fungal invasion of the orbit and the sinuses was demonstrated in the pathological species and Rhizomucor species were yielded with culture. Repeated superficial debridement was also performed. After 10 weeks, she was discharged with suggestion of insulin treatment and liposomal amphotericin-B with progressively decreasing doses. At the 13th month following the presentation, the patient was free of disease as confirmed by serial imaging and under good glycaemic control with insulin treatment. Although mucormycosis is a fatal infection, early diagnosis and aggressive treatment may decrease mortality.
