Structural visualization of expert nursing: Diabetes self-management education program.

In the Japanese clinical setting, expert nursing, and at the same time quality and safety assurance, are demanded. To provide nursing service with reliable assurance of nursing quality and safety, structural visualization of the thought, judgment and behavior processes of nursing practice and widespread application through development of IT applied systems are required. We have discussed and developed the structural visualization of programmed care in the field of diabetes self-management education, which is an area of expert nursing care. In this article, we report the programmed care for dietary therapy.

Structural visualization of expert nursing: Development of an assessment and intervention algorithm for delirium following abdominal and thoracic surgeries.

An assessment and intervention algorithm for delirium following abdominal and thoracic surgeries was developed based upon the current knowledge-base. The sources of information included literature and clinical expertise. The assessment and intervention algorithm was structured and visualized so that patient-tailored and risk-stratified prediction/prevention, assessment, and intervention could be carried out. Accumulation of clinical outcome data is necessary in the future validation study to identify the relative weight of risk factors and clinical utility of the algorithm.

Structural visualization of expert nursing: Dialysis patient education program "PD catheter management".

Structural visualization was implemented on catheter management education for peritoneal dialysis (PD) patients requiring expert nursing. An education program utilizing electronic images was proposed as a method of monitoring the exit site, which is of high importance in PD catheter self-management. Recording images of the exit site and utilizing them as part of the education program encouraged a common understanding of the exit site between nurses and patients, and helped to achieve a better quality of patient education. It is thought that visualization of nursing practice and the thinking processes of expert nurses which have not been expressly stated previously can become a foundation for more advanced nursing care throughout the profession.

Structural visualization of expert nursing: Dialysis patient education program "vascular access management".

Structural visualization was implemented on vascular access management education for dialysis patients requiring expert nursing. As listening to and recording the sound of the Vascular access is important, an education program involving the electronic recording of sounds was proposed. Recording the Vascular access sounds and utilizing them as part of the education program encouraged a common understanding of the sounds between nurses and patients and helped to achieve a better quality of patient education. It is thought that the visualization of nursing practice and the thinking processes of expert nurses which have not been expressly stated previously can become a foundation for more advanced nursing care throughout the profession.

Structural visualization of expert nursing: Expert nursing care for a patient undergoing outpatient radiotherapy.

In the past, almost all patients underwent radiotherapy during the hospital stay. So nurses took care of patients after radiotherapy in the hospital where patients could receive nursing care 24 hours a day. However, recently more and more patients have undergone radiotherapy at clinics as outpatients. This benefits them in enabling them to receive the therapy with improving QOL and maintaining their style of living--on the other hand this requires much self-care ability on their part and patient compliance of outpatient visits depends on their own positive decision-making. Nurses are considered to make decisions or take implementation of various kinds of intervention in the limited time of pre and post proceedings of the therapy when they accommodate patients directly. The process of their intervention has not so far been clarified so that the quality of the care depends on the degree of each individual's capacity or devotion. In addition, with the trend in decreasing hospitalized days, it is predicted that more and more patients will undergo outpatient radiotherapy. So it is significant to clarify the process for quality assurance of nursing.

Structural visualization of expert nursing: Expert nursing care for extravasation of anticancer agent.

Chemotherapy aims to cure cancer or inhibit the growth of cancer cells by administering the anticancer agent in blood vessels (BV) to the cancer body. So it is very important to inject the agent in BVs without fail because there is risk of extravasation which brings about the chemical necrosis of the tissue around. Thus, nurses involved in chemotherapy care should learn to take interventions for prevention from extravasation and immediate treatment for it, in case. In the interventions there are care practices with advanced thinking processes. Clarification of such thinking processes contributes to the visualization of nursing practice and the support for nurses to learn expert nursing practices. Also it is supposed to realize the nursing quality assurance for care receiver-patients.

Structural visualization of expert nursing: Hemodialysis patient education program "behavior modification program for hemodialysis patients".

Behavior modification programs (BMP) have been suggested to be useful for the self-management of hemodialysis (HD) patients. To provide more systematic care, we structured the procedure of the thinking process and care in BMPs as an algorithm. BMP developers produced a temporary algorithm based on previous studies, discussed it with nurses with BMP experience, and added and revised necessary items. As a result, an algorithm of BMP with high reproducibility that allows maintenance of consistent quality for the self-management of HD patients could be developed.

Structural visualization of expert nursing: Cancer pain management.

Nurses' knowledge and making decisions in Cancer Pain Management contribute much to the improvement of cancer patients' QOL. Based on the practice of a expert nurse involved in Cancer Pain Management, we have developed the algorithm, which clarified that nurses implement much of observations or complicated decision-makings in Cancer Pain Manage Hereafter it is a significant challenge to examine its consistency and validity to develop its system.

Structural visualization of highly-specialized practice on nursing and midwifery: Nurse-midwife's monitoring and caring during labor and delivery.

Structural visualization was implemented on nurse-midwife's care during labor and delivery, requiring highly-specialized practice on nursing and midwifery. From discussion about our former researches and literature review, and using an algorithm notion method, we made an algorithm of the monitoring and caring process by the nurse-midwife. It is thought that the visualization of nursing and midwifery practice and the thinking processes of expert nurse-midwives which have not been expressly stated previously can become a foundation for more advanced nursing care throughout the profession.

Structured visualization of expert nursing--An educational program for stoma self-care.

To provide an adequate educational program for patients with a stoma, we have developed an algorithm which was divided into three parts according to the period, i.e., the preoperative period, postoperative period, and rehabilitation period. It was composed of action nodes, thinking nodes, choice branches, assessment item tables, reference tables, and candidate logic tables. Wound Ostomy Continence Nurses (WOCNs) used this algorithm. The results indicated that the algorithm did not omit any important points involved in stoma care, and that it could therefore be successfully applied to patients with a stoma.

Structured visualization of expert nursing: Prevention of pressure ulcers.

We made an algorithm by extracting the caring elements for the patients to develop a program for pressure ulcer prevention. The algorithm consisted of observation of the patient skin as a beginning point, extraction of the risk factors for pressure ulcer development, intervention to each risk factor, and evaluation. The expert panel identified this algorithm as adequate enough to adopt to the patients.

To realize easy-to-understand description of nursing practice terminology for consumer.

In Japan it is an important issue to introduce Electronic Health Records (EHR) in over 60% of hospitals with more than 400 beds. Introducing E.H.R is supposed to help provide a safe medical environment as well as practice medicine by description in plain terms among professionals and among patients and medical professions. We've developed Nursing Pr Terminology understandable to consumers visualization of nursing action, and arranged them by level and category. However, it is still difficult to express the appropriate contents because they consist of expert decisions and processes. Hereafter, it is a very important challenge of ours to develop an easy-to-understand description of the Nursing Practice Terminology to supply safe and patient-centered medicine to consumers.

A clinical management system for patient participatory health care support. Assuring the patients' rights and confirming operation of clinical treatment and hospital administration.

We conducted a drastic change in our hospital information system to support patient participatory health care provided in the New Hiroshima University Hospital (HUH). The new information system in HUH (HU-MIND II-Hiroshima University Hospital Medical Intelligence and Notice Delivery system II) is designed as "clinical management system" (CMS). The core of this CMS is the electronic health record (EHR), which aims to assure both the patients' right to know, and the attendants' accountability. It is evident that the team practice including patients requires close communication. Data in the EHR are written not only by physicians, but also by all coworkers, which will enable them to realize the team communication and the ordering in a reliable way and to leave the evidence of conducted practices. Moreover, the bedside information systems were set-up at all 700 beds. Patients can access their anamnesis and future clinical care procedures themselves. Based on the demand outlined above, the new regulations of HUH are composed of 21 requirements, conditions of information collection, accumulation and use. Our focus was how to make patients' right compatible with attendants' accountability. As the data owners, patients have the facility to access their own data at their bedsides. They can view their own health condition and treatment program and can control the data flow.
