Cationic comb-type copolymers for boosting DNA-fueled nanomachines.

For the better applications and developments of DNA nanomachines, their responding kinetics, output, and sequence-selectivity need to be improved. Furthermore, the DNA nanomachines currently have several limitations in operating conditions. Here we show that a simple addition of a cationic comb-type copolymer, poly(l-lysine)-graft-dextran, produces the robust and quick responses of DNA nanomachines under moderate conditions including physiologically relevant conditions even at very low strand concentrations (nanomoles per liter range) through hybrid stabilization and DNA strand exchange acceleration.

Influence of overweight on surgical complications for gastric cancer: results from a randomized control trial comparing D2 and extended para-aortic D3 lymphadenectomy (JCOG9501).

BACKGROUND: The impact of overweight on the outcome of gastrectomy with lymphadenectomy is controversial, and data from a well-controlled, randomized study are needed to identify a possible relationship. METHODS: We used data from 523 patients registered for a prospective randomized trial comparing D2 and extended para-aortic D3 lymphadenectomy to compare the effects of body mass index (BMI) and the extent of lymphadenectomy for the development of general or major surgical complications (anastomotic leakage, abdominal abscess, and pancreatic fistula). RESULTS: Seventy-seven patients were classified as overweight with BMI >or= 25, and 38 and 39 of these patients underwent a D2 or D3 lymphadenectomy, respectively. Among the 446 patients classified as nonoverweight with BMI < 25, 225 received D2 and 221 received D3 lymphadenectomy. Surgical complications, operation time, and blood loss were statistically significantly associated with BMI, and logistic regression analysis revealed that overweight directly affected the occurrence of surgical complications even after considering operation time and blood loss as intermediate factors instead of outcome variables. Among patients undergoing D2 lymphadenectomy, being overweight increased the risk for surgical complications and blood loss, whereas overweight was associated with only blood loss and operation time among patients receiving D3 lymphadenectomy. CONCLUSIONS: Overweight increased the risk of surgical complications in patients undergoing gastrectomy both directly and indirectly through operation time and blood loss. The impact of overweight on surgical complications was more evident in patients undergoing a D2 dissection.

[Rehabilitation method for churg-strauss syndrome from early stage: considering functional prognosis of mononeuritis multiplex].

We report our rehabilitation experience for two patients with Churg-Strauss syndrome (CSS). To improve their functional prognoses, we started rehabilitation program compensating for their activity of daily livings from their early stage. Also, we paid special attention to prescribed drug effect and adverse effect. Since patients with the mononeuritis multiplex have restricted weakness in the muscles ruled by a specific nerve, rehabilitation approaches including orthoses should start to improve the compensatory ability of normal adjacent muscles even in their early stages. In addition, since functional improvement may be gradual and take some years from onset, we should start rehabilitation in their early stage to improve their functional prognoses.

Fabrication and photocatalytic characterizations of ordered nanoporous X-doped (X = N, C, S, Ru, Te, and Si) TiO2/Al2O3 films on ITO/glass.

Transparent, ordered nanoporous TiO2/Al2O3 composite films doped with metal elements (Ru, Si, and Te) and nonmetal elements (N, C, and S) were fabricated by successive anodization and sol-gel process directly on glass substrates covered with a tin-doped indium oxide (ITO) film. The doping of ruthenium, nitrogen, carbon, and sulfur in TiO2 exhibited an enhanced effect on the absorbance, while the doping of silicon and tellurium showed little effect. Particularly, the N- and Ru-doped TiO2/Al2O3 films on ITO/glass developed an enhanced absorption red shift of 580 nm (-N) and 500 nm (-Ru). The nanoporous TiO2/Al2O3 composite film exhibited the highest photocatalytic activity in decomposing acetaldehyde under ultraviolet-light irradiation, with a value of 13 times in initial reaction rate or 7.8 times in quantum yield higher than a commercially available TiO2 material, Degussa P25. The ultraviolet-light photocatalytic activities of nanoporous TiO2/Al2O3 films were enhanced by the doping of nitrogen, carbon, and sulfur but slightly weakened by the doping of ruthenium, silicon, and tellurium. Particularly, the nanoporous N-doped TiO2/Al2O3 films exhibited effective photocatalytic activity on ultraviolet light decomposition of a highly toxic dioxin, HpCDD, and gave the highest decomposition rate of approximately 95% (via 7 h of irradiation) for the specimen with a dopant content of 1.7 wt % nitrogen.

Effects of changes in skiing posture on the kinetics of the knee joint.

This study was performed to investigate the effects of changes in the skiing posture on mechanical stress across the knee joint. The aim of the present study was to establish a safer form of skiing for the prevention of injury to the anterior cruciate ligament of the knee (ACL). Ten healthy volunteers were placed on a force plate on an artificial slope, and assumed forward and backward bending postures with a single leg by changing their centre of gravity. The knee flexion angles were limited to approximately 30 degrees by orthotics in each posture. Joint moments of the lower extremity were analyzed using a motion analysis system, and the muscle activities of the lower extremity were recorded by EMG and digitised to integrated EMG in each posture. In addition, extrusion of the lower leg by the boot was measured using a force sensor sheet inserted behind the calf. We found that the extension moment of the hip and ankle joints, and muscular activity of the hamstrings were increased in forward bending, whereas the extension moment of the knee and muscular activity of the quadriceps were decreased. Conversely, the muscular activity of the quadriceps, the flexion moment of the hip, extension moment of the knee joint, and pressure of the boot were increased in backward bending, whereas muscular activity of the hamstrings was decreased. The dependency on the hamstrings increased in forward bending in a skiing posture on a slope with slight knee flexion. Therefore, forward bending seemed to be an advantageous posture for the prevention of ACL injury. Conversely, in a backward bending skiing posture, the extension moment of the knee joint produced by muscle contraction of the quadriceps also increased imbalance in the hamstrings. The results of this study suggest that the internal force exerted by the quadriceps, in addition to the passive factor of extrusion of the lower leg by the boot, may contribute to non-contact ACL injury in a backward bending posture while skiing.

[Local recurrence of pancreatic cancer successfully treated with gemcitabine].

We report a patient for whom systemic chemotherapy using gemcitabine was effective against local recurrence of pancreatic cancer. A 58-year-old man underwent pancreatoduodenectomy for a pancreatic head cancer. The diagnosis was Stage IVb poorly-differentiated tubular adenocarcinoma, scirrhous type, pT4, PL (+), P0, H0, pN2. However, after 21 months, gastrointestinal bleeding occurred. Gastroscopy and CT examination revealed a mass at the cut-end of the pancreas invading the stomach. The serum CA19-9 level was found to be elevated. Systemic chemotherapy was performed with a regimen of gemcitabine 1,000 mg/m2/week for 2 weeks, followed by a week rest. The recurrent tumor in the stomach disappeared, and the mass at the cut-end of the pancreas became small. The serum CA 19-9 level regained the normal value. Two years after the diagnosis of recurrence, he returned to work, and his chemotherapy is being continued as an outpatient.

[Rehabilitation for gait and activity of daily livings at home: an informed form for Parkinson's disease patients].

Rehabilitation aims to provide patients and their supporters with the knowledge, skills and support necessary to maintain their autonomy, minimize disability and maximize the level of participation of individual. This may be the case in Parkinson's disease (PD) patients. However, to our knowledge, previous papers concerning rehabilitation for PD patients have been not easily to understand and not suit for usage in clinical activities. In this review, we provide comprehensive explanations for daily rehabilitation in their habitation for PD patients and doctors.

[A case of advanced gastric cancer responding to neoadjuvant TS-1/CDDP chemotherapy].

The patient was a 61-year-old man who was referred to our hospital with a complaint of epigastric pain. Upper gastrointestinal endoscopy and X-ray examination of the stomach revealed type 3 cancer in the gastric antrum, extending to the middle body. It was about 9 cm in diameter, and the biopsy specimen revealed moderately differentiated tubular adenocarcinoma. Abdominal CT scan showed marked enlargement of No. 3 lymph nodes along the lesser curvature of the stomach. Examination of the blood showed a hemoglobin of 10.2 g/dl, CEA 5.8 ng/ml, CA19-9 330.5 U/ml. For this gastric cancer, clinical Stage IIIA (cT3N1HOPOMO), neoadjuvant chemotherapy with TS-1/CDDP was planned. TS-1 (120 mg/day) was orally administered for 3 weeks followed by a drug-free-2-week period as the first course, and 93 mg (60 mg/m2) of CDDP administered by intravenous drip on day 8. There were grade 2 nausea and grade 3 appetite loss by intravenous administration of CDDP in the second course. An upper GI series revealed 33% reduction of gastric cancer, and laboratory studies CEA and CA 19-9 showed normal values. One month after the second course of chemotherapy, total gastrectomy, splenectomy and lymph node dissection D2 were performed. The pathological specimens showed no cancer cells in the surgically obtained stomach and lymph nodes, so the histological effect was Grade 3. The postoperative course was satisfactory, and he now attends outpatient department without any findings of recurrence 12 months after the operation. TS-1/CDDP chemotherapy produced a high response in this case, and it may be useful as neoadjuvant chemotherapy for advanced gastric cancer.

Improved performance of a DNA nanomachine by cationic copolymers.

We have reported previously that the cationic comb-type copolymer, PLL-g-Dex, accelerated DNA strand exchange reaction while stabilizing DNA duplexes. In this research, we studied the effects of PLL-g-Dex on a DNA-fueled nanomachine. The copolymer enabled rapid and secure operations of the DNA nanomachine which utilized the structural transition between intramolecular G-quadruplex and intermolecular double helix.

Intralimb incoordination in patients with ataxia.

Intralimb incoordination needs to be distinguished from interlimb incoordination, which is observed in Parkinson's disease (PD). With this aim, intra and inter-limb coordination impairment in the lower limbs were measured in 13 patients with cerebellar ataxia during pedaling of an ergometer with left and right pedals that can be rotated independently. Intralimb incoordination in patients with cerebellar ataxia was compared with interlimb incoordiation in patients with PD. We concluded that impairment of intralimb coordination in ataxia patients leads to inability to maintain pedaling amplitude and speed, but unlike in PD, right and left coordination was roughly preserved. Our method may be useful for assessing the severity of ataxia and detecting hidden parkinsonism in ataxic patients.

Classifying lower limb dynamics in Parkinson's disease.

To classify lower limb dynamics in patients with Parkinson's disease (PD), we conducted a clinical study by using pedaling exercise.Twenty-seven patients with idiopathic PD were included in this study. We measured rotational velocities of pedals during pedaling movements with a newly developed ergometer. The velocity waveforms exhibited different characteristics among patients, which could be categorized into four different clusters. In cluster 1, the amplitude on each side was constant and the relative phase was locked at 180 degrees. The pattern was the same as seen in normal subjects. In cluster 2, the amplitude on each side was constant, but the relative phase was locked at 90 degrees. In cluster 3, the amplitude on each side was modulated, and the relative phase drifted monotonously from 0 to 360 degrees during pedaling cycles. In cluster 4, the amplitude on each side was synchronously and irregularly modulated, and the relative phase fluctuated with intermittent spike-like decrement. In order to evaluate, the correlation between pattern and severity of PD, we divided 13 patients, who underwent measurement of pedaling patterns more than three times, into three groups, and found that the abnormal coordination pattern correlated with the presence of freezing phenomenon in patients with PD. Our clinical analysis may contribute in analyzing and classifying the dynamics of PD.

Design of artificial nucleic acid chaperones for DNA engineering.

We have shown that the comb-type copolymer consisting of a polycation backbone and hydrophilic side chains stabilizes DNA hybrids. Furthermore, the copolymers showed the activity to accelerate DNA strand exchange reactions between double helical DNA and its complementary DNA. The copolymer was considered to stimulate breakage and reassociation of base pairing and act as an artificial nucleic acid chaperone. The polymer's chaperoning activity was elaborated for rapid and precise judging of a subtle difference in DNA sequences. One base alternation out of 20mer DNA was quickly detected by the strand exchange assay employing the copolymer. From these, we conclude that the copolymer would be a useful material in DNA engineering that employs rapid and precise DNA folding.

[Successful treatment of gastric cancer accompanied by multiple liver metastases with TS-1 followed by curative gastrectomy affiliation].

Despite the improvement in the operative results for patients with gastric cancer, the prognosis of those with liver metastasis remains dismal. Multimodal therapy has attracted considerable attention as a breakthrough in the strategy for treating cases of highly advanced gastric cancer. We report the case of a 62-year-old female with gastric cancer accompanied by multiple liver metastases successfully treated by TS-1, a novel oral fluoropyrimidine derivative. One treatment course consisted of 4 weeks of TS-1 administration (100 mg daily) followed by a 2-week break. After 3 courses of treatment, an abdominal CT scan showed no evidence of liver metastases and gastroscopy revealed that the primary gastric lesion was reduced. Grade 1 toxicity (nausea and diarrhea) was seen but 6 days rest improved the condition. Distal gastrectomy was subsequently performed without any finding of residual tumor. TS-1 may have a promising role in neoadjuvant chemotherapy for gastric cancer.

[A clinical scoring system may identify good candidates for hepatic arterial infusion chemotherapy after hepatic resection for colorectal cancer].

The significance of hepatic arterial infusion chemotherapy (HAI) for the prognosis of metastatic colorectal cancer is controversial. We applied a clinical scoring system to find good candidates for HAI after hepatic resection. Ninety-four consecutive cases of hepatic metastases resected at a single institute were analyzed retrospectively. The cases were divided into 4 subgroups: those with a low risk score (0-2 points) with and without HAI and those with a high risk score (3-5 points) with and without HAI. The number of cases and the cumulative 5 year survival rate of each case were as follows: low risk score with HAI (n = 17) 64.1%, low risk score without HAI (n = 25) 27.4%, high risk score with HAI (n = 28) 16.9%, and high risk score without HAI (n = 24) 14.7%, respectively. These results suggest that hepatic resection with HAI seemed to contribute to the outcome for selected cases. The clinical scoring system may identify good candidates for HAI as well as predict recurrence after hepatic resection for metastatic colorectal cancer.

A neural mechanism for detecting the distance of a selected target by modulating the FM sweep rate of biosonar in echolocation of bat.

Most species of bats making echolocation use frequency modulated (FM) ultrasonic pulses to measure the distance to targets. These bats detect with a high accuracy the arrival time differences between emitted pulses and their echoes generated by targets. In order to clarify the neural mechanism for echolocation, we present neural model of inferior colliculus (IC), medial geniculate body (MGB) and auditory cortex (AC) along which information of echo delay times is processed. The bats increase the downward frequency sweep rate of emitted FM pulse as they approach the target. The functional role of this modulation of sweep rate is not yet clear. In order to investigate the role, we calculated the response properties of our models of IC, MGB, and AC changing the target distance and the sweep rate. We found based on the simulations that the distance of a target in various ranges may be encoded the most clearly into the activity pattern of delay time map network in AC, when the sweep rate of FM pulse used is coincided with the observed value which the bats adopt for each range of target distance.

Clinicopathological features and outcome of hepatic resection for liver metastasis from gastric cancer.

BACKGROUND/AIMS: The efficacy of operative resection of lesions metastatic to the liver from colorectal or neuroendocrine tumor is well established. However, the appropriate management of liver metastasis from gastric cancer is controversial. We analyzed the prognostic factors in patients who underwent hepatectomy for metastasis from gastric cancer. METHODOLOGY: Retrospective clinical and pathological study in Tokyo Metropolitan Bokutoh Hospital. Ten patients underwent hepatectomy for metastases from gastric cancer out of 1807 patients with gastric cancer between 1981 and 1998. INTERVENTIONS: Clinical investigation and histopathological examination of resected specimen. MAIN OUTCOME MEASURES: Survival, recurrence, liver metastases and lymph node metastases. RESULTS: The 1-, 3-, and 5-year survival rates of these ten patients were 50%, 30%, 20%, respectively. The median survival time was 25 months, and two patients survived longer than five years. The survival time tended to be longer, but not to a significant extent, in patients with no lymph nodal involvement at the primary site (P = 0.067). CONCLUSIONS: Even though it is rare, a survival time of 5-years can be achieved by resection of gastric cancer metastatic to the liver. These results suggest that a patient with liver metastasis from gastric cancer has a greater chance of surviving long-term if there is no lymph node metastasis at the primary site.
