Transport of a hydrophobic biosynthetic precursor by lipophorin in the hemolymph of a geometrid female moth which secretes an epoxyalkenyl sex pheromone.

Previous experiments with a geometrid species, Ascotis selenaria cretacea, have suggested that a pheromonal C19 3,4-epoxy-6,9-diene is biosynthesized from the corresponding 3,6,9-triene produced outside a pheromone gland and transported to it via hemolymph after association with lipophorin. In order to clarify this transport, high-density lipophorin (HDLp) in the female moths showing two bands (apoLp I with ca. 250 kDa and apoLp II with ca. 80 kDa) on an SDS-PAGE was purified by KBr equilibrium density-gradient ultracentrifugation, and the association of the triene was confirmed by GC-MS analysis of a solvent extract from the isolated protein. Next, the role of HDLp was revealed by a topical application of the deuterated trienyl precursor to the abdomens of the females. The trienyl precursor was associated with HDLp. In their pheromone glands, the triene and the deuterated epoxy pheromone were detected, indicating movement of the triene via the hemolymph. Experiments with male moths of A. s. cretacea and female moths of Bombyx mori showed the same association of HDLp with the triene topically applied. This result suggested that the adult females of A. s. cretacea did not develop HDLp specialized in the triene transport. Furthermore, the topical application of a mixture including the trienyl precursor and two other related hydrocarbons showed equal amounts of association by HDLp but selective delivery of the precursor to pheromone glands in the A. s. cretacea females.

Ultramultiple roundtrips of surface acoustic wave on sphere realizing innovation of gas sensors.

A thin beam of wave usually diverges due to diffraction, which is a limitation of any device using such waves. However, a surface acoustic wave (SAW) on a sphere with an appropriate aperture does not diverge but is naturally collimated, realizing ultramultiple roundtrips along an equator of the sphere. This effect is caused by the balance between diffraction and focusing on a spherical surface, and it enables realization of high-performance ball SAW sensors. The advantage of ball SAW is most fully appreciated when applied to a very thin sensitive film for which the multiple-roundtrip enhances the sensitivity, but the attenuation loss is not very large. It is exemplified in a hydrogen gas sensor that realizes a wide sensing range of 10 ppm to 100% for the first time, and realizes relatively fast response time of 20 s without heating the sensitive film.

Chromosomal regions with quantitative trait loci controlling cadmium concentration in brown rice (Oryza sativa).

A novel mapping population consisting of 39 chromosome segment substitution lines (CSSLs) was used to locate the putative quantitative trait loci (QTLs) for cadmium (Cd) concentration in brown rice (Oryza sativa). The mapping population carried a single chromosome segment of 'Kasalath' (indica) in each line overlapping with neighbouring segments in a 'Koshihikari' (japonica) genetic background. The parents and CSSLs were grown in pots filled with Cd-polluted soil until grain filling. The brown rice of three of the 39 CSSLs had significantly lower Cd concentrations than that of Koshihikari, and the brown rice of a further three had significantly higher concentrations. On the basis of graphical genotypes of CSSLs, putative QTLs controlling the Cd concentration in brown rice were detected on chromosomes 3, 6 and 8. Each of the CSSLs was nearly isogenic to Koshihikari, which is the most popular rice cultivar in Japan: they carried > 90% of the Koshihikari genetic background. Therefore, the development of a new Koshihikari with less Cd concentration in brown rice would be feasible in the near future.

A case of a large inguinoscrotal bladder hernia secondary to benign prostatic obstruction.

Large bladder hernias protruding into the scrotum are rare, with 23 cases having been reported previously in Japan. We report a case of a patient with a bladder hernia secondary to benign prostatic obstruction who demonstrated a unique voiding procedure. The patient manually compressed his scrotum at micturition to facilitate bladder emptying. He underwent subcapsular prostatectomy, followed by inguinal hernia repair. Postoperatively, the voiding procedure and urinary flow returned to normal. We should pay attention to symptoms of bladder hernia in the follow-up of patients with bladder outlet obstruction.

Involvement of FcRgamma in signal transduction of osteoclast-associated receptor (OSCAR).

Osteoclasts regulate homeostasis of bone development. A defect in osteoclast development results in osteopetrosis. Recently, the involvement of several molecules in osteoclast development has been found. Osteoclast-associated receptor (OSCAR) is one of such molecules critical for osteoclast differentiation. However, it remains unclear how OSCAR transduces signals for osteoclast differentiation. Here, we found that the FcRgamma chain, a signal transducing adaptor molecule for Fc receptors, is associated with OSCAR and is involved in the cell surface expression of OSCAR. Furthermore, FcRgamma is required for signal transduction by OSCAR. These findings suggest that the FcRgamma-mediated signal transduction by OSCAR is involved in osteoclast differentiation.

[Bladder neck contraction after radical prostatectomy: morbidity and risk factors].

We retrospectively analyzed the morbidity and risk factors of bladder neck contraction (BNC) after retropubic prostatectomy. Of 99 consecutive patients who underwent radical retropubic prostatectomy between 1995 and 2001, 10 (10%) developed anastomotic stricture after surgery. BNC was diagnosed 7 months on average after the surgery. Nine patients were successfully treated by cold-knife incision, but 5 required additional incision against the recurrence of BNC. None of them revealed newly diagnosed stricture or incontinence. Thirteen potential risk factors including age, stage, PSA, neoadjuvant treatment, pathology, intraoperative blood loss, operation time, operator, nerve sparing, extravasation of urine, marginal status, the year of operation, and the method of ureterovesical anastomosis were evaluated using univariate and multivariate analysis. The BNC rate was increased in patients with longer (more than 4 hrs) operations and excessive bleeding (more than 1,000 ml) (p=0.0027, respectively). The year of operation (before 1998, p=0.0015), the method of ureterovesical anastomosis (p=0.0017), operator experience (less than 5 cases, p=0.0056), and neoadjuvant treatment (p=0.09) were also risk factors. In multivariate analysis, the year of operation (p=0.03) and operator experience(p=0.04) were strong predictors of BNC. The skill levels of surgeons and institutes are expected to decrease BNC.

[PSA change and morbidity of prostate cancer after dietary intervention of low-fat and high-soybean-protein diet for patients with high serum PSA level].

A total of 164 patients with a high serum prostate specific antigen (PSA) level were enrolled in a supervised program of dietary intervention with a low-fat and high-soybean-protein diet. The primary endpoints were the evaluation of PSA change and morbidity of prostate cancer in this population. In the evaluable cases, the average serum PSA level had decreased 0.5 ng/ml after 3 months, 0.3 ng/ml after 6 months, and increased 0.1 ng/ml after 1 year. The change after 3 months was statistically significant (p=0.018, paired t-test). The average PSA doubling time was 444 months. During this study period, 9 of the patients developed prostate cancer. These findings indicate that this dietary intervention method was temporarily able to decrease serum PSA level, and indicates the possibility of prevention of prostate cancer by dietary intervention. However, more detailed analysis is necessary to determine why PSA did not keep decreasing.

Usefulness of urinary NMP22 to detect tumor recurrence of superficial bladder cancer after transurethral resection.

BACKGROUND: In a prospective study we compared the usefulness of urinary nuclear matrix protein 22 (NMP22) with that of urine cytology and other urinary markers in the monitoring of superficial bladder cancer after transurethral resection (TURBT). METHODS: The subjects were 156 patients, comprising 99 patients with superficial bladder cancer in whom TURBT was planned (untreated group) and 57 patients without tumors in the bladder who had been followed up after TURBT (follow-up group). RESULTS: Among the 156 patients, who were monitored for 11-26 months (median, 21 months), recurrence was observed in 51 patients (33.0%). At the time of recurrence, the sensitivities of NMP22, basic fetoprotein (BFP), and bladder tumor antigen (BTA) tests, and urine cytology were 18.6%, 23.3%, 9.3%, and 7.0%, respectively. The factors affecting the sensitivity of NMP22 were tumor size and urinary WBC. The size of recurrent tumors was significantly smaller (P<0.05) than that of the initial tumors. Based on receiver operating characteristic (ROC) curves calculated from the data of patients with recurrence, the ideal cutoff values at recurrence were recommended to be 5.0 U/ml for NMP22 and 6.0 ng/ml for BFP. Using these cutoff values, the sensitivities of NMP22 and BFP were 48.8% and 44.2%, respectively. CONCLUSIONS: Because the size of recurrent bladder tumors is usually smaller than that of the initial tumors, the cutoff values of urinary markers should be reduced to detect these tumors. We recommend 5.0 U/ml as a cutoff value of NMP22 for detection of recurrence of bladder tumor.

Analysis of spurious bulk waves in ball surface wave device.

We analyzed the acoustic waves propagating in a sphere to establish a useful guideline for the design of NDE apparatus and ball surface acoustic wave (SAW) device exploiting the diffraction-free propagation of SAW on a sphere. First, we calculated the laser-generated acoustic displacements both under ablation condition and under thermoelastic condition and verified experimentally the validity of the calculation. Next, the acoustic waves excited by out-of-plane stress and those excited by in-plane stress were compared. The results showed that when the out-of-plane stress was applied, the relative amplitudes of the bulk waves to that of the SAW were larger and the number of bulk waves was larger than that when the in-plane stress was applied, while the SAW had similar waveforms in each case. The ratio of the relative amplitude of the bulk waves for the out-of-plane stress and the in-plane stress was 3.1:1 at phi(1)=90 degrees and 1.67:1 at phi(1)=0 degrees. The large amplitude for the out-of-plane stress can be explained by wide directivities of bulk waves. Consequently, we found that it is necessary for ball SAW device to select a piezoelectric material and form of interdigital transducer so that the in-plane stress becomes dominant.

Angiomyolipoma with regional lymph node involvement: a case report and literature review.

A 28-year-old man without tuberous sclerosis, who complained of pollakisuria, consulted to our hospital for a left renal mass. Abdominal computed tomography revealed a solid mass without a lipid component, 10 cm in diameter, in the left kidney and with regional lymphadenopathy. Renal arteriography showed a hypervascular mass, demonstrating multiple tumor stains and aneurysms. Left radical nephrectomy and perihilar lymph node dissection were performed for an anticipated diagnosis of malignant tumor in November 2001. The histopathological diagnosis was an angiomyolipoma with lymph node involvement. Immunostaining for myogen markers was positive in the renal mass and lymph node tumors. He was free from disease ten months after surgery.

Renal oncocytomatosis in a long-term hemodialysis patient treated by laparoscopic surgery.

A 50-year-old female, who had been on maintenance hemodialysis for 22 years, consulted our clinic because of a left renal mass detected incidentally by ultrasonography. Computed tomography (CT) demonstrated a solid hypervascular mass, suggesting a renal cell carcinoma (RCC), in the left atrophic kidney. Left hand-assisted laparoscopic nephrectomy (HALN) was performed. The histopathological diagnosis was renal oncocytomatosis. Renal oncocytomatosis in a long-term hemodialysis patient is extremely rare. We report the first case of renal oncocytomatosis in a long-term hemodialysis patient treated with hand-assisted laparoscopic nephrectomy.

An organ-sparing treatment using combined intra-arterial chemotherapy and radiotherapy for muscle-invading bladder carcinoma.

OBJECTIVE: We describe the results of an organ-sparing approach for the treatment of non-metastatic, invasive bladder carcinoma. MATERIAL AND METHODS: Twenty-three patients (mean age 71 years; age range 47-87 years) with bladder carcinoma of clinical stage T2-T3N0M0 and histologically proven muscle invasion were examined between 1992 and 1998. The median duration of follow-up was 30 months. The treatment protocol for intra-arterial chemotherapy consisted of methotrexate 30 mg/m(2) and cisplatin 50 mg/m(2) in 7 patients and cisplatin 50 mg/m(2) in 16 patients, administered in three cycles via catheters inserted in the internal iliac arteries. Concomitantly, 41.4 Gy of radiotherapy was given to the lesser pelvis. Transurethral biopsy and urine cytology were performed after the completion of treatment; patients were followed observationally if residual tumor was absent, and underwent radical cystectomy if it was present. RESULTS: At the end of treatment, 18 patients (78%) showed a complete response (CR) and the bladder was spared in all cases. Radical cystectomy was performed for 4 non-CR cases, with the result that 2 cases had residual superficial cancer and the other 2 had muscle-invading cancer histologically. Among the patients with a CR, 2 experienced intravesical recurrence. Overall, 2 patients died of cancer, 5 died of other causes and 2 died during treatment. The 5-year disease-specific survival rate was 70.3% and the overall survival rate 46.4%. CONCLUSIONS: A bladder-sparing approach for the treatment of muscle-invading bladder carcinoma which utilizes combined intra-arterial chemotherapy and radiotherapy may arrest the decline in quality of life induced by urinary diversion and yield equivalent therapeutic benefit to that of radical cystectomy.

[Metachronous primary malignant lymphoma of the bilateral tests: a case report].

We report a case of metachronous malignant lymphoma of the bilateral testes. A 62-year-old man presented with a mass in the right scrotal contents. Physical examination revealed a solid painless mass in the right scrotal contents measuring 4 cm in diameter. He underwent right high orchiectomy. The histological examination confirmed non-Hodgkin's lymphoma of diffuse, large-sized cells of the B cell type. Computed tomography of the abdomen revealed paracaval lymphandenopathy at stage IIE according to Ann Arbor classification. Chemotherapy was initiated with cyclophosphamide, adriamycin and vincristine. Eleven months after the initial operation, the patient complained of left scrotal swelling, and subsequently underwent left high orchiectomy. The histological examination revealed the same pathology as observed in the right one scrotal contents. He was free from recurrence at 15 months after the second operation.

[A low-fat and high soybean protein diet for patients with elevated serum PSA level: alteration of QOL and serum PSA level after the dietary intervention].

Considerable epidemiological evidence has indicated a relationship between diet and prostate cancer. Many studies have shown positive associations between dietary fat and prostate cancer, and inverse associations with soybean intake. To prevent prostate cancer, we have tried dietary intervention for patients showing an elevated prostate specific antigen (PSA) level. A total of 96 patients who did not show any evidence of prostate cancer by transrectal ultrasonography and/or prostate biopsy despite high serum PSA level were placed on a supervised dietary intervention program consisting of a low fat and high soybean protein diet. The primary endpoint was the alteration of quality of life (QOL) and PSA after 3 months of intervention. Response to the questionnaire indicated that 88% of the patients desired to join such a study, and that 90% of the patients continued the dietary control after 3 months of supervision. About 60% patients adhered strictly to the dietary menus, and 90% of the subject planed to continue. Less than 20% of the patients complained of deterioration of dietary QOL. The average serum PSA level decreased from 6.9 ng/ml to 5.6 ng/ml after 3 months, which was statistically significant (p = 0.01). This dietary intervention almost maintains the patient's QOL, decreases the serum PSA level, and indicates the possibility of prevention of prostate cancer by dietary intervention.

Transitional cell carcinoma of the renal pelvis in a patient with cyclophosphamide therapy for malignant lymphoma: a case report and literature review.

Cyclophosphamide is considered to be a bladder carcinogen and there are many reports of secondary bladder cancer, while only a few cases of upper urothelial cancer have been described. A 59-year-old man, who had received cyclophosphamide therapy for malignant lymphoma, was suffering from gross hematuria and consulted our institute. Computerized tomography (CT), intravenous pyelography (IVP) and retrograde pyelography (RP) revealed a left renal pelvic tumor. Urinary cytology showed class V and radical left nephroureterectomy was performed. Histopathological diagnosis of the left renal pelvic tumor was transitional cell carcinoma, invading the renal parenchyma. He is free from recurrence eight months after surgery. To our knowledge, this is the third case of cyclophosphamide-induced upper urothelial carcinoma reported in Japan, and the twelfth reported in the English literature.
