Metastasis of primitive neuroectodermal tumor to the breast.

We performed mammography and sonography on a 49-year-old woman who had a mass in her left axilla that had been present for 1 month and who had undergone excision of a primitive neuroectodermal tumor (PNET) on her back 1 year before. Mammography revealed 2 adjacent, dense nodules in the left breast and enlarged lymph nodes, without a fatty hilum or internal microcalcifications, in the left axilla. Sonography showed 2 round to oval, markedly hypoechoic nodules in the left breast and enlarged, markedly hypoechoic lymph nodes, without microcalcifications, in the left axilla. We then performed sonographically guided core biopsy. Histopathologic analysis of the specimens confirmed the presence of PNET in the left breast and axillary lymph nodes. The patient was then treated with chemotherapy. To our knowledge, this report is the first to describe radiologic findings of metastasis of PNET to the breast.

Fine needle aspiration cytology of small cell variant of anaplastic large cell lymphoma. A case report.

BACKGROUND: Small cell variant of anaplastic large cell lymphoma (ALCL) is a rare and still underrecognized variant of ALCL. In contrast to the classic type of ALCL, which is sometimes mistaken for carcinoma or sarcoma, the small cell variant may be misdiagnosed as a reactive lesion without a high index of suspicion. CASE: We present a case of small cell variant of ALCL occurring in a 14-year-old female. It posed great difficulty in the fine needle aspiration (FNA) diagnosis due to the rarity of large, atypical cells. A subsequent lymph node biopsy demonstrated the typical histologic and immunophenotypic findings of the small cell variant of ALCL. CONCLUSION: This case indicates that small cell variant of ALCL should always be considered in the FNA diagnosis of unusual-looking, heterogeneous lymphoid proliferations dominated by atypical small or medium-sized lymphoid cells, particularly in young patients.

Squash smear findings of spherical amyloid in pituitary prolactinoma. A case report.

BACKGROUND: Pituitary prolactinoma containing spherical amyloid is rare. Squash smear findings of spherical amyloid in pituitary prolactinoma are characteristic. CASE: A 25-year-old woman presented with a pituitary tumor. The intraoperative squash smear showed abundant, homogeneous, eosinophilic material with multiple prominent striations. The scattered atypical cells in the periphery contained an irregular, bizarre nucleus; coarse chromatin; occasional prominent nucleoli; rare cytoplasm; and prolactin immunoreactivity. CONCLUSION: The characteristic squash smear findings should be helpful in the diagnosis of this rare tumor.

Human papillomavirus type 59 identified in a verrucous cyst of the flank.

The verrucous cyst is a non-plantar epidermoid cyst with histopathological features of human papillomavirus (HPV) infection, including papillomatosis and hypergranulosis of the cyst lining. We report the first case of a verrucous cyst demonstrating not only the histopathological and immunohistochemical features of HPV infection, but also homology with HPV type 59 on HPV genotyping. A 28-year-old male developed a palpable mass in his right flank. Histological examination revealed an intradermal cyst lined by an acanthotic and papillomatous squamous epithelium with prominent keratohyaline granules and squamous eddies. The keratinocyte nuclei were positive for papillomavirus antigens on immunohistochemistry and HPV genotyping demonstrated a homology to HPV type 59, a high-risk genital type. Although we only experienced a single case with such a finding, we suggest that it may be necessary to subject patients with verrucous cysts to a closer follow up for better characterization of their clinical behavior.
